The authors present one case of severe congenital dysmetria of the leg with agenesis of the foot which was treated by the Ilizarov method. After a clinical and radiographic description of the case, the type of surgery used to create a new foot and the results obtained are discussed.

Recently a novel and non-invasive technique of lower leg length measurement (knemometry) was introduced. Experiences made in 5 cases with Borggreve's rotation plasty have shown that in cases of congenital femural defects and exceptionally also in malignant bone tumors of the distal femur this method should be suggested to the patient as a possible therapy. By rotating and shortening of the leg, the ankle joint is converted to a new knee joint. With this method an active movement of the knee joint can be obtained, so that the fitting with a below knee prosthesis becomes possible.

Periosteal function of growing bones was studied in young sheep. Periosteal devascularisation and tibial plate implantation caused partial cortical necrosis and porosity of endostally vascularized cortex. Plate implantation induced epiphyseal activities. After 8 months differences in bone length of 2.5 mm were observed.

With special reference to skeletal growth, a 17-year follow-up study of a lower-leg replantation in a four-year-old boy is reported. The patient maintained good cosmesis and function; however, foot size on the affected side was 1.5 cm smaller than the contralateral side, and leg length was 1.2 cm shorter than on the normal side.
