BACKGROUND: Flat pigmented skin lesions in chronically sun-exposed face and scalp areas present diagnostic challenges. Particularly, solar lentigo, pigmented actinic keratosis and lentigo maligna show overlapping features. Erroneous laser treatment of lentigo maligna for cosmetic reasons will inevitably delay diagnosis and compromise patient safety. This study assessed the performance of a market-approved convolutional neural network (CNN) versus dermatologists in differentiating melanocytic from non-melanocytic face and scalp lesions (FSL).

Mohs micrographic surgery is a technique that offers surgeons and pathologists a means to examine the entire surgical margins of excised cutaneous malignancies in a more complete manner than conventional methods. Initially developed to treat nonmelanoma skin cancers, its use has been expanded to include histologic margin evaluation for treatment of malignant melanoma. Clinical studies demonstrate equivalent or better 5-year survival rates with Mohs surgery compared to conventional wide local excision. For surgical treatment of melanoma, it offers unique advantages for lesions requiring tissue

Dermatologic radiotherapy is based on the standard physical and radiobiological parameters. The radiation quality most often used in dermatology lies between 10 and 50 kV. Another important parameter is the half-value depth which should correspond to the depth of the tumor below the skin surface. In this way the skin is not over-exposed to radiation treatment. Indications for radiotherapy of malignant skin tumors include basal cell carcinoma, squamous cell carcinoma, severe actinic keratoses, lentigo maligna, lentigo maligna melanoma, Merkel cell carcinoma, and Kaposi sarcoma, as well as T- an

Spontaneous hair repigmentation of physiologically white or gray hair is a rare occurrence that may be associated with melanoma in elderly individuals. We present the first case of this phenomenon in a man. A gray-haired, 80-year-old man presented to dermatology clinic with a 3-cm lock of black hair on his vertex scalp that developed over 1 year. Punch biopsies showed an increase in junctional dendritic melanocytes with rare pagetoid cells and extension along the follicular outer root sheath epithelium and interfollicular epidermis, associated with prominent dendritic melanocytic hyperplasia a

Pigmented facial macules are common on sun damage skin. The diagnosis of early stage lentigo maligna (LM) and lentigo maligna melanoma (LMM) is challenging. Reflectance confocal microscopy (RCM) has been proven to increase diagnostic accuracy of facial lesions. A total of 154 pigmented facial macules, retrospectively collected, were evaluated for the presence of already-described RCM features and new parameters depicting aspects of the follicle. Melanocytic nests, roundish pagetoid cells, follicular infiltration, bulgings from the follicles and many bright dendrites and infiltration of the hai

BACKGROUND: Malignant changes in burn scars occasionally develop, the majority being squamous cell carcinoma. Malignant melanomas (MMs) arising in burn scars occur rarely.
OBJECTIVE: To present the case of a patient with a multiple MM developed at different burn scar areas.
METHODS: A case report and review of literature are given.
RESULTS: One was a large pigmented nodular lesion with central ulceration on right flank. It was widely excised and revealed lentigo MM. After that, we took notice another small pigmented lesion on right arm. It was also reported lentigo MM.
CONCLUSION: This cas

Mohs micrographic surgery (MMS) has been shown to achieve very low recurrence rates in skin cancer, and some studies suggest it may improve survival. We conducted a narrative review to assess the impact of MMS on the survival of patients with various skin cancer subtypes. Some retrospective studies suggest that MMS may enhance survival in patients with head and neck melanoma, lentigo maligna, lentigo maligna melanoma, invasive cutaneous squamous cell carcinoma (cSCC) (especially high-risk cSCC), and high-risk dermatofibrosarcoma protuberans, and, possibly, with certain malignant adnexal tumors

BACKGROUND: Benign lichenoid keratoses (BLKs) are solitary skin lesions which have been proposed to represent a regressive form of pre-existent epidermal tumours such as solar lentigo or seborrhoeic keratosis. However, the genetic basis of BLK is unknown.
OBJECTIVES: FGFR3, PIK3CA and RAS mutations have been shown to be involved in the pathogenesis of seborrhoeic keratosis and solar lentigo. We thus investigated whether these mutations are also present in BLK.
METHODS: After manual microdissection and DNA isolation, 52 BLKs were screened for FGFR3, PIK3CA and RAS hotspot mutations using SNaP

Seborrheic keratosis is a common benign skin tumor that affects most individuals, particularly later in life. Despite its high prevalence, the underlying biology of seborrheic keratosis remains poorly understood. In this study, we performed whole-exome sequencing and spatial transcriptomics on a cohort of seborrheic keratosis and solar lentigo lesions to gain insight into their molecular landscape. Our results reveal a high mutational burden driven by solar UV damage, with recurrent somatic variants observed in genes such as FGFR3, NOTCH1, and TP53. Spatial transcriptomic analysis uncovered a

There is no doubt that ultraviolet radiation (UVR) contributes to the generation of acquired lentigines in human skin, as indicated by the term solar lentigo. A growing number of recent epidemiological and mechanistic studies, however, strongly suggest that in addition to UVR, other environmental factors contribute to lentigines' formation as well. We therefore here introduce the term 'environment-induced lentigo' (EIL) to refer to acquired pigment spots of human skin. In this view point, we (i) summarize the existing evidence to support a role of environmental toxicants other than UVR in the

In dermatologic oncology several good indications for radiotherapy are known. Precancerous lesions can be treated by grenz rays, as e.g. large senile (actinic) keratoses, lentigo maligna or large lesions of Bowen's disease or Queyrat's erythroplasia. Well-known indications are also mid-sized basal cell and squamous cell carcinomas, especially in the face of elderly patients. The lentigo-maligna melanoma should no longer be considered a radioresistant tumor, and it must be stressed that larger lesions can be successfully treated by radiotherapy. Dermato-radiotherapy shows also excellent palliat

Many advances have been made in cryosurgery during the past decade. Important considerations are the high cure rate, good cosmetic result, convenience, and cost effectiveness. In an era of budget restraints with regard to medical care and in a time of concern with exposure to bloodborne pathogens, cryosurgery offers unique solutions. Cure rates for skin cancer in the area of 97 to 98 percent, for actinic keratosis, 98 percent, and for lentigo maligna, 96 percent, should encourage its use in more patients. The effectiveness in freezing lesions of leukoplakia, dermatofibroma, hemangioma, keloid,

INTRODUCTION: Facial pigmented lesions pose significant challenges for diagnosis and treatment planning due to their anatomical topography and complexity. Traditional methods like dermoscopy have limitations, and while reflectance confocal microscopy (RCM) offers in-vivo cellular resolution, it is hindered by shallow penetration. The recently developed line-field confocal optical coherence tomography (LC-OCT) combines the benefits of OCT and RCM, providing deeper penetration and three-dimensional cellular imaging.
OBJECTIVES: This study aims to assess the ability of LC-OCT in displaying morph

INTRODUCTION: Differentiating early melanoma from other flat pigmented lesions on the head and neck is challenging both clinically and dermoscopically, partly due to the wide differential diagnosis and the lack of specific diagnostic algorithms.
OBJECTIVES: To review publications covering the dermoscopic features of pigmented macules on the head and neck.
METHODS: Embase and PubMed (Medline) database from January 2015 to January 2021 were searched using a four-step search. Keywords used were dermoscopy/dermatoscopy or epiluminescence microscopy, lentigo maligna, lentigo maligna melanoma, lic

PURPOSE: BRAF mutations are present in two thirds of cutaneous melanomas and many of the rest have NRAS mutations. However, cutaneous melanoma is a heterogeneous disease with many clinicopathologic subtypes. Of these, the majority fits into four categories: superficial spreading, nodular, lentigo maligna, and acral lentiginous melanoma (ALM). Thus far, there is very limited data combining BRAF and NRAS mutation analysis to explore differences between cutaneous melanoma subtypes. The aim of this study was to address this issue.
EXPERIMENTAL DESIGN: The frequency of BRAF and NRAS hotspot mutati
