Histoid leprosy: case report.

Histoid leprosy is a rare but well-defined entity with specific clinical, histopathologic, and bacteriologic features. We present a case of histoid leprosy in an 84-year-old Egyptian male in view of the rarity of this condition. The patient presented with erythematous itchy discrete and coalescent papules that were distributed bilaterally and symmetrically on the front and back of the trunk. Before approaching us, he was initially misdiagnosed as a case of pityriasis rosea. There was no mucosal or facial affection and the patient's general examination was normal. De-novo histoid leprosy.

Histoid leprosy is a rare form of multibacillary leprosy with distinct clinical and histopathological features. It is a variant of lepromatous leprosy. It occurs in lepromatous patients, who relapse after dapsone monotherapy, in the presence of dapsone resistance or at times 'de-novo'. We describe here a case of histoid leprosy, clinically mimicking neurofibromatosis.

Leprosy: recognizing red flags.

A 28-year-old man from the Philippines presented with multiple papules and plaques symmetrically distributed on the arms and legs. This was associated with worsening paraesthesia of the hands and feet. A right common peroneal nerve decompression had been performed 2 years earlier. He was diagnosed with multibacillary leprosy on skin biopsy and subsequently treated with oral rifampicin, clofazamine and dapsone.

Dermatological disorders resembling leprosy.

Three dermatological conditions - epidermolysis bullosa dystrophica (EBD), granuloma multiforme (GM) and mycosis fungoides (MF) were diagnosed elsewhere as leprosy either clinically or histologically. Although the morphology of the lesions were suspicious of leprosy there were few striking clinical findings which were unfavourable. Leprosy is still an important disease that should not be missed. However, the recognition of these skin disorders is highlighted so that unnecessary and prolonged treatment for leprosy can be avoided in endemic countries.
