Temporary biventricular support as a bridge to recovery: A justifiable strategy for small children with fulminant myocarditis.

Short-term mechanical circulatory support can be life-saving in the pediatric population with acute cardiogenic shock (ACS). However, recovery from MCS is a rare entity. MCS options are limited for low-body-weight children in Turkey. Over the last decade, extracorporeal membrane oxygenation (ECMO) has been the primary bridging modality for children with end-stage heart failure in our country. However, VA-ECMO may cause increased wall stress and oxygen demand, which may alter myocardial recovery. Here, we describe using a Levitronix CentriMag Systems for biventricular support as a bridge to rec

Neurodiversity and Intellectual Disability: Opportunities and Challenges for Functioning and Participation Across the Life Course.

There is increasing recognition that intellectual developmental disabilities are best understood through strengths, everyday functioning, and participation rather than through deficits or predicted limitations. The "F-Words" framework (ie, Function, Family, Fitness, Fun, Friends, Future) translates the World Health Organization's International Classification of Functioning, Disability and Health into language that is meaningful for clinicians and families. Using three common pediatric scenarios (eg, Down syndrome, fetal alcohol spectrum disorder, prematurity), this review shows how early couns

Managing NIF safety equipment in a high neutron and gamma radiation environment.

The National Ignition Facility (NIF) is a 192 laser beam facility that supports the Inertial Confinement Fusion program. During the ignition experimental campaign, the NIF is expected to perform shots with varying fusion yield producing 14 MeV neutrons up to 20 MJ or 7.1 × 10(18) neutrons per shot and a maximum annual yield of 1,200 MJ. Several infrastructure support systems will be exposed to varying high yield shots over the facility's 30-y life span. In response to this potential exposure, analysis and testing of several facility safety systems have been conducted. A detailed MCNP (Monte Ca

Experiences and coping strategies of community-dwelling middle-aged and older adults with physio-cognitive decline syndrome: A qualitative study.

BACKGROUND: Physio-Cognitive Decline Syndrome (PCDS) is a clinical characterized by concurrent declines in physical and cognitive functions that precede disability. Taiwan's rapidly aging population highlights declines in physical and cognitive functions as precursors to disability, posing key challenges in geriatric care. Research shows these declines begin in early middle age and progress with age. Understanding middle-aged and older adults' experiences and coping strategies for Physio-Cognitive Decline Syndrome (PCDS) is essential for developing effective interventions. PURPOSE: This study

Artificial intelligence-associated delusions and large language models: risks, mechanisms of delusion co-creation, and safeguarding strategies.

Large language models (LLMs) are poised to become a ubiquitous feature of everyday life, mediating communication, decision making, and information curation across nearly every domain. Within psychiatry and psychology, the attention has largely been on bespoke therapeutic applications, sometimes narrowly focused and often diagnostically siloed, rather than on the broader reality that individuals with mental illness will increasingly engage in agential interactions with artificial intelligence (AI) systems. Although the capacity of these systems to model therapeutic dialogue, provide companionsh

Imaging insights in veno-venous and veno-arterial extracorporeal membrane oxygenation (ECMO): CT protocols, underlying pathophysiology, and main complications.

Extracorporeal membrane oxygenation (ECMO) is a vital life support technique employed in patients experiencing pulmonary or cardiopulmonary failure. This procedure entails the use of a pump to replace heart function and an oxygenator to ensure adequate blood oxygenation. ECMO systems are categorized into two main configurations: veno-venous (VV) and veno-arterial (VA) circuits. VV-ECMO is employed for isolated respiratory failure, while VA-ECMO provides temporary mechanical circulatory support for patients with cardiogenic shock or cardiac arrest. A less common alternative, veno-arterial-venou

Palliative care in cardiovascular medicine.

Cardiovascular disease remains the leading global cause of morbidity and mortality. Although advances in prevention, diagnostics, and disease-modifying therapies have prolonged survival, many individuals now live longer with high symptom burden, functional decline, and complex decisional needs. Palliative care (PC) for adults with advanced cardiovascular disease can improve quality of life, support caregivers, and align treatments with patient values and goals. Core elements include symptom management, effective communication, shared decision-making, advance care planning, and integration of p

Multidisciplinary management of Duchenne muscular dystrophy from childhood to adulthood.

Duchenne muscular dystrophy (DMD) is a severe, progressive neuromuscular disorder characterized by muscle degeneration and weakness, leading to significant physical disability and multisystem complications. The management of DMD requires a coordinated, multidisciplinary approach to address the evolving needs of patients from childhood through adulthood, with a focus on patient quality of life. Early diagnosis and proactive interventions are critical to optimizing outcomes and may include physical therapy, respiratory support, cardiac monitoring, and orthopedic care. In children, the focus is o

Communication and Relationships in Pediatric Palliative End-of-Life Decision Making in the United Kingdom and Japan: A Cross-Cultural Scoping Review.

INTRODUCTION: Decision making in pediatric end-of-life care for children with life-limiting or life-threatening conditions (LLC/LTCs) is ethically complex and shaped by cultural values and health care systems. Although shared decision making has been increasingly emphasized, comparative evidence on how communication and relationships influence pediatric end-of-life decision making across cultural contexts remains limited. This scoping review compared communication and interpersonal relationships among health care professionals (HCPs), parents, and children in the United Kingdom and Japan. MET

Does (co-)variation between boldness and growth differ between sexes? A nuanced and powerful evaluation of the pace of life hypothesis.

A major goal of behavioral ecology is identifying sources of among-individual variation in behavior. Theory suggests that life history traits and personality should be correlated at the individual level, but despite much attention, it has little support. A potentially crucial aspect influencing life history and personality associations that has been overlooked is sex. Males and females of many species differ in energy requirements, energy allocation, and behavior, suggesting that sex may affect correlations between life history and personality differently; if so, not accounting for sex differe

Forced oscillation measurements in the first week of life and pulmonary outcome in very preterm infants on noninvasive respiratory support.

BACKGROUND: We aimed at investigating whether early lung mechanics in non-intubated infants below 32 weeks of gestational age (GA) are associated with respiratory outcome. METHODS: Lung mechanics were assessed by the forced oscillation technique using a mechanical ventilator (Fabian HFOi, ACUTRONIC Medical Systems AG, Hirzel, Switzerland) that superimposed small-amplitude oscillations (10 Hz) on a continuous positive airway pressure. Measurements were performed during regular tidal breathing using a face mask on days 2, 4, and 7 of life. Respiratory system resistance (Rrs) and reactance (Xrs)

[Conditions of use and access to care of sickle cell anemia patients. Analysis of the literature].

In order to identify the care access and life conditions and non-compliance risk factors of sickle-cell patients, we performed a literature survey on these subjects, and on other chronic pathologies and care systems used as references. The data showed a positive influence of good quality of life, social support and family environment on diabetes mellitus patients compliance, and the negative effect of poor socioeconomic status on adequate prenatal care. Concerning sickle-cell disease, there were few data on the non-compliance risk factors. However, the authors identified factors which influenc

2025 Korean Guidelines for Cardiopulmonary Resuscitation: Part 7. Pediatric basic life support.

Pediatric cardiac arrest primarily arises from asphyxia in infants and trauma in older children, contrasting with adult etiologies dominated by cardiac events. This underscores prevention as the cornerstone of pediatric basic life support, through injury mitigation like child restraint systems and water supervision, safe sleep practices including supine positioning on firm surfaces with caregiver smoking cessation to reduce sudden infant death syndrome, plus awareness of child abuse and adolescent suicide prevention. In hospitals, pediatric early warning systems (PEWS) enable early deteriorati
