Differential diagnosis and management of liver tumors in infants.

During the first year of life, most of the liver neoplasms are benign in origin, but some of these histologically benign lesions may be challenging in their management. Although most hepatic hemangiomas can be safely observed until involution is documented, some patients will need treatment due to progressive hepatomegaly, hypothyroidism and/or cardiac failure. Large mesenchymal hamartomas may require extensive hepatic resection and an appropriate surgical plan is critical to obtain good results. For malignant neoplasms such as hepatoblastoma, complete surgical resection is the mainstay of cur

Diagnosis and treatment of hepatic disease in patients with HIV.

Liver involvement with opportunistic infections and neoplasms is a well-recognized component of AIDS, affecting most patients. The cause of hepatic disease in these patients may be divided into hepatitis, granulomatous disease, mass lesions, vascular lesions, hepatotoxic drugs, and nonspecific findings. With a rational approach, most patients with AIDS and liver disease can be diagnosed and treated in a cost-effective manner with low morbidity.

Cholangiocarcinoma associated with multiple bile-duct hamartomas of the liver.

The case of a 61-year-old woman with a surgically resected solitary cholangiocarcinoma of the liver is reported, where many discrete multiple bile duct hamartoma (MBDH) were also seen. The latter is a congenital lesion of the liver that potentially may be confused with widespread metastatic disease. The relationship between cholangiocarcinoma and MBDH was studied histologically by the use of an immunoperoxidase technique for cytokeratin. MBDH was strongly positive for cytokeratin, while the neoplasm showed this to a lesser extent, but a clear continuity between the MBDH epithelial cells and th

Computed tomography and magnetic resonance imaging evaluation of liver cancer.

Focal liver lesions occur commonly and with varying histology, each requiring radically different management. There are a multitude of imaging modalities currently being used for detecting and characterizing focal liver neoplasms. The primary modalities include ultrasonography, computed tomography (CT), magnetic resonance imaging (MRI), and rarely, angiography. CT and MRI have benefited from rapid technologic advances, and MRI, in particular, from the advent of new contrast agents.

[Macroscopic liver pathology in patients with dissociated cholestasis].

OBJECTIVE: To establish the prevalence of liver focal pathology in patients with increase of alkaline phosphatase and gamma-glutamil transpeptidase and normal bilirubin (dissociated cholestasis), and to analyze the related risk factors for such pathology. METHODS: All laboratory studies of patients admitted to an Internal Medicine Department were reviewed prospectively throughout a period of 9 months. For the purpose of detecting focal liver pathology imaging liver studies (echography and/or CT) were carried out in those in which biochemical analyses showed dissociated cholestasis. RESULTS:

[A Resected Case of Intraductal Pupillary Mucinous Neoplasm of the Pancreas Characterized by Recurrence in the Remnant Pancreas and Multiple Malignant Neoplasms in Other Organs].

A 74-year-old man underwent pancreaticoduodenectomy for intraductal pupillary mucinous neoplasm(IPMN)of the pancreatic head. Histopathological examination of the specimen revealed non-invasive intraductal papillary mucinous carcinoma( IPMC). Two years after surgery, recurrent IPMN developed in the remnant pancreas, and a liver tumor was identified in S8 of the liver. The patient underwent total pancreatectomy of the remnant pancreas and partial resection of S8. These tumors were histopathologically diagnosed as invasive IPMC and hepatocellular carcinoma, respectively. Five years after the rese

[Multiple hepatic abscesses due to actinomycosis: difficulties of diagnosis].

Authors present the case of a 65 year-old man without any remarkable previous illness, hospitalized because of having prolonged fever caused probably by an infection due to a tick bite. Tularaemia and bartonellosis based on serological results as well as neoplasm of the sigma with multiple metastases to the liver raised by ultrasonography and abdominal CT could be excluded with detailed investigations. Multiple abscesses of the liver were diagnosed, followed by a percutaneuos ultrasonography directed aspiration of one of the abscesses which resulted in 150 ml buffy pus. Microbiological examina

Magnetic resonance imaging of cystic and endocrine pancreatic neoplasms.

This article describes the appearance of miscellaneous cystic and endocrine neoplasms using magnetic resonance imaging (MRI). Magnetic resonance imaging is a useful diagnostic modality in the assessment of various pancreatic neoplasms. Pancreatic endocrine tumors are moderately low in signal intensity on T1-weighted fat-suppressed images and moderately high in signal intensity on T2-weighted fat-suppressed images and demonstrate homogeneous, ring, or diffuse heterogeneous enhancement on immediate postgadolinium gradient echo images. Cystic pancreatic neoplasms, including intraductal papillary

Hepatic Lymphoma Diagnosis.

Systemic hematopoietic disorders may present on liver biopsy, and rare cases of primary hepatic lymphoma (PHL) may be encountered. Hepatopathologists must be familiar with the full spectrum of hematopoietic disorders involving the liver and be prepared to exclude benign mimics. PHL, which is confined to the liver without extrahepatic involvement, can present as solitary or multiple nodules, raising consideration for carcinoma on imaging, or may mimic benign inflammatory conditions, posing a diagnostic challenge. This article describes clinical, morphologic, and immunophenotypic features of som

Hepatocellular carcinoma and other hepatic malignancies: MR imaging.

Magnetic resonance (MR) imaging surpasses all other imaging modalities in characterizing liver lesions by virtue of the exquisite tissue contrast, specificity for various tissue types, and extreme sensitivity to contrast enhancement. In addition to differentiating benign from malignant lesions, MR imaging generally discriminates between the various malignant liver lesions. Hepatocellular carcinoma constitutes most primary malignant liver lesions and usually arises in the setting of cirrhosis. Intrahepatic cholangiocarcinoma is a distant second and features distinctly different imaging features

Antemortem diagnosis of hepatic adenocarcinoma in a ewe.

A 4-year-old ewe with weight loss, anorexia, and high liver enzyme activities was diagnosed at necropsy as having cholangiocellular carcinoma that had metastasized to kidneys, adrenal glands, heart, lungs, mediastinum, and peritoneal surface of the diaphragm. Cholangiocellular carcinoma is an uncommon neoplasm of sheep. Evaluation of liver tissue obtained via ultrasound-guided percutaneous liver biopsy had suggested an antemortem diagnosis of adenocarcinoma of the liver.

Advancements in selective internal radiotherapy: Efficacy, safety, and future directions.

Selective internal radiotherapy (SIRT) has emerged as a viable treatment for both primary and secondary liver neoplasms, exhibiting favorable efficacy and low toxicity in clinical applications. This review synthesizes key studies that investigate the safety and efficacy of SIRT, particularly in hepatocellular carcinoma and intrahepatic cholangiocarcinoma, along with its application in treating liver metastatic malignancies. SIRT demonstrates superior time to progression, enhanced quality of life, and reduced toxicity compared with conventional therapies, including tyrosine kinase inhibitors an

Multifocal fatty sparing areas in a steatotic liver mimicking hepatic lesions: a case report and review of the literature.

BACKGROUND: Fatty liver disease is a major global health problem that can be challenging to diagnose, particularly when focal fatty-sparing mimics hepatic neoplasms. These imaging findings can result in undue concern and may prompt invasive procedures that could otherwise be avoided. Accurate differentiation between benign focal fat-related changes and malignant lesions requires careful correlation of imaging features with clinical and laboratory findings. CASE PRESENTATION: A 34-year-old Arab female with a history of hyperlipidemia and hypothyroidism was hospitalized for acute appendicitis.

Periampullary Carcinoma: Unusual Sites of Metastasis.

BACKGROUND: Periampullary neoplasms include carcinomas of the duodenum, ampulla of Vater, distal common bile duct, and pancreas. The aggressive course of these neoplasms is attributable to the delay in diagnosis, as patients have no symptoms until advanced stages of the disease. More than half of patients have distant metastasis at the time of diagnosis. The most frequent sites of metastasis are the liver, lymph nodes, peritoneum, lung, bone, kidney, and, rarely the skin. CASE REPORT: We report the case of a 45-year-old female patient with adenocarcinoma of the ampulla of Vater metastasizing

Peribiliary glands pathology in a large series of end-stage alcohol-related liver disease.

The peribiliary glands are intramural or extramural structures with multiple functions related to bile secretion. The peribiliary glands can develop cystic alterations in several conditions, such as alcohol addiction. Peribiliary cysts can enlarge till being radiologically visible and mimic cancer. We studied 217 consecutive explanted livers for end-stage alcohol-related liver disease from the Pathology Unit of the Liver Unit at the King's College Hospital in Denmark Hill, with particular focus on peribiliary glands. Our cohort consisted of 31 females and 186 males, with a median age of 51 and

Intraoperative ultrasound for liver tumor resection in children.

BACKGROUND: Primary hepatic neoplasms in children are rare tumors. All malignant and medically refractive benign primary pediatric liver tumors ultimately require surgical resection for cure. Accurate preoperative imaging including multidetector helical computerized tomography or magnetic resonance imaging (MRI) is necessary to determine resectability. In the literature intraoperative ultrasound (IOUS) has proven to be a vital adjunct to liver surgery in adults, but this is not well established in children. MATERIALS AND METHODS: Between April 2003 and November 2014, children (<18-y-old) with

Co-existence of hepatocellular carcinoma and hepatic tuberculosis.

BACKGROUND: Many case reports describe tuberculosis (TB) co-existent with a malignant neoplasm. However, the neoplasm in most of these reports is lung or breast cancer, with only two cases of liver cancer concomitant with TB reported in the literature. Although both TB and cancer are very common diseases, little attention has been given to the pathophysiologic and practical implications of their co-existence.
