Two-week nifedipine treatment for pulmonary hypertension complicating chronic obstructive lung disease.

The effects of nifedipine on hemodynamics and blood gases were studied in 10 patients with pulmonary hypertension secondary to chronic obstructive lung disease. Two different sets of test data were recorded. The first set of readings was taken immediately before and during the initial hour after sublingual administration of 20 mg of nifedipine. The second set was taken after 2 weeks of therapy with 3 X 10 mg per day. After the acute sublingual dose, a significant decrease in pulmonary arterial pressure and pulmonary vascular resistance was recorded: 13% and 26% respectively. This decrease was

[Influence of disturbed lung hemodynamics on the early bronchial obstruction in mitral disease (author's transl)].

32 subjects affected with mitral stenosis were investigated. All the subjects were nonsmokers, and none had any histories of lung disease. Mean pulmonary artery pressure (PAp) and mean wedge pressure (Pc) were obtained by right catherization and compared with a number of sensitive tests for the detection of incipient bronchial obstruction: dynamic and static lung compliance (CLdyn, CLstat); maximal expiratory flows at 50% and 25% (MEF50%, MEF25%) of the forced vital capacity (FVC): closing volume (CV); airway resistance (RAW), and residual volume (RV). It was established that most sensitive te

Reversibility of chronic obstructive lung disease in infants following repair of ventricular septal defect.

The signs of obstructive lung disease dominated the clinical course of ten infants with ventricular septal defects and large left-to-right shunts. Airway obstruction in these patients can be attributed (1) to increase in large airway resistance as the result of compression by enlarged pulmonary arteries or cardiac chambers and (2) to increase in small airway resistance as the result of accumulation of peribronchiolar fluid. The rapid regression of the signs of obstructive airway disease following open heart repair of the ventricular septal defect indicates that the lung disease observed in the

The association of spirometric small airways obstruction with respiratory symptoms, cardiometabolic diseases, and quality of life: results from the Burden of Obstructive Lung Disease (BOLD) study.

BACKGROUND: Spirometric small airways obstruction (SAO) is common in the general population. Whether spirometric SAO is associated with respiratory symptoms, cardiometabolic diseases, and quality of life (QoL) is unknown. METHODS: Using data from the Burden of Obstructive Lung Disease study (N = 21,594), we defined spirometric SAO as the mean forced expiratory flow rate between 25 and 75% of the FVC (FEF25-75) less than the lower limit of normal (LLN) or the forced expiratory volume in 3 s to FVC ratio (FEV3/FVC) less than the LLN. We analysed data on respiratory symptoms, cardiometabolic dis

The Evaluation of Interstitial Abnormalities in Group B of the 2011 Global Initiative for Chronic Obstructive Lung Disease (GOLD) Classification of Chronic Obstructive Pulmonary Disease (COPD).

Objective In 2011, the Global Initiative for Chronic Obstructive Lung Disease (GOLD) classification categorized chronic obstructive pulmonary disease (COPD) patients into 4 groups. A report demonstrated that the mortality in Group B was higher than that in Group C. Ischemic heart disease and cancer were suggested to be the cause. The aim of the present study was to test the hypothesis that interstitial lung abnormalities (ILAs) are more prevalent in Group B than Group C and that they may be responsible for the higher mortality in Group B. Methods Patients were selected based on their pulmonary

Features of Heart Failure with Preserved Ejection Fraction in Patients with Chronic Obstructive Pulmonary Disease and Systemic Sclerosis-Associated Interstitial Lung Diseases.

Background/Objectives: This study aims to investigate the potential etiopathogenesis of HFpEF development and identify possible different phenotypes of HFpEF in patients with chronic obstructive pulmonary disease (COPD) and systemic sclerosis-associated interstitial lung diseases (SS-ILDs). It could help clinicians improve early HFpEF personalized detection and management. Methods: This study included 150 patients with chronic lung diseases (CLDs), such as COPD and SS-ILD, who were outside of exacerbation, had no history of chronic heart failure (CHF), and had a left ventricular ejection fract

Inhalation Technique Errors with Metered-Dose Inhalers Among Patients with Obstructive Lung Diseases: A Systematic Review and Meta-Analysis of U.S. Studies.

BACKGROUND: Metered dose inhalers (MDIs) are commonly prescribed for inhalation therapy, but correct use is critical to promoting effective medication delivery. This systematic literature review and meta-analysis evaluates the overall and step-by-step prevalence of errors among adults with obstructive lung diseases in the United States who used MDIs. METHODS: Electronic and manual searches conducted between 1979-2018 using PubMed, EMBASE, PsycINFO, Cochrane, and Google identified 10 articles that met the following inclusion criteria: (a) English language, (b) U.S. adults diagnosed with chroni

Proteo-metabolomic insights into the progression of chronic obstructive pulmonary disease and lung function decline.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is a progressive yet preventable respiratory disease, often remaining undiagnosed until significant lung function impairment has occurred. The early, asymptomatic stage, termed "pre-COPD", offers a critical window for intervention, yet its molecular characteristics remain poorly understood. METHODS: We performed integrated proteomic and metabolomic profiling of human lung tissues across the spectrum of COPD (n = 32), including non-diseased controls, individuals with pre-COPD, and patients at varying stages of established disease. Key mo

Electroencephalogram derived sleep microarchitecture in patients with interstitial lung disease and obstructive sleep apnea.

INTRODUCTION: Sleep disturbances are common in patients with interstitial lung diseases (ILD) and are often compounded by comorbid obstructive sleep apnea (OSA). Conventional polysomnographic indices may not fully capture disease-specific neurophysiological alterations. METHODOLOGY: We aimed to characterize EEG-derived sleep microarchitecture in ILD with comorbid OSA (ILD-OSA) and compare it with ILD without OSA and apnea hypopnea index matched non-ILD OSA (n = 20 in each group). Quantitative EEG analysis was performed using standardized preprocessing and data extraction protocols. Spectral p

Cluster analysis of obstructive lung disease phenotypes: the Framingham heart study and the Rotterdam study.

BACKGROUND: The traditional classification of obstructive lung diseases into asthma and chronic obstructive pulmonary disease oversimplifies their complexity. A hypothesis-free classification using routine clinical and laboratory characteristics may identify clinically important subgroups. METHODS: Hierarchical clustering was applied to 1473 Framingham Heart Study (FHS) participants with a history of asthma or airflow obstruction based on pre-bronchodilator spirometry. Variables included demographics, smoking history, body mass index, inflammatory markers, spirometry, and symptoms, excluding

Imaging Markers of Chronic Obstructive Pulmonary Disease on Computed Tomography of the Lungs: a Narrative Review.

Chronic obstructive pulmonary disease (COPD) is a heterogeneous disorder in which spirometry reflects airflow limitation but incompletely captures the underlying structural injury. Patients with similar spirometric impairment may have markedly different patterns of emphysema, small-airway disease, and pulmonary vascular involvement. Computed tomography (CT) can visualize these abnormalities in vivo and may detect structural changes before overt airflow limitation develops. This narrative review examined whether CT-derived imaging markers provide clinically meaningful information beyond spirome

Chronic Obstructive Pulmonary Disease and Inhaled Treatment Effects on Mortality in Patients with Lung Cancer.

Rationale: In patients with lung cancer, the impact of chronic obstructive pulmonary disease (COPD) diagnosis and subsequent management on mortality remains uncertain, because evidence supporting the efficacy of inhaled therapies in improving clinical outcomes in this population is limited. Objectives: This aim of this study was to assess whether COPD worsens outcomes in patients with lung cancer and to investigate whether inhaled treatments for COPD can improve these outcomes. Methods: This retrospective cohort study used the Korea Central Cancer Registry database from 2012 to 2019. Patients

Integrating Genetics, Transcriptomics, and Proteomics in Lung Tissue to Investigate Chronic Obstructive Pulmonary Disease.

The integration of transcriptomic and proteomic data from lung tissue with chronic obstructive pulmonary disease (COPD)-associated genetic variants could provide insight into the biological mechanisms of COPD. Here, we assessed associations between lung transcriptomics and proteomics with COPD in 98 subjects from the Lung Tissue Research Consortium. Low correlations between transcriptomics and proteomics were generally observed, but higher correlations were found for COPD-associated proteins. We integrated COPD risk SNPs or SNPs near COPD-associated proteins with lung transcripts and proteins

The prevalence of obstructive sleep apnea in interstitial lung disease: a systematic review and meta-analysis.

PURPOSE: To evaluate the overall prevalence of obstructive sleep apnea (OSA) in interstitial lung disease (ILD). METHODS: We performed a systematic search of the academic literature while adhering to the preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines on four scientific databases including EMBASE, CENTRAL, Scopus, and MEDLINE. We performed a meta-analysis to evaluate the prevalence and severity of OSA. Severity was defined by apnea-hypopnea index (AHI) as mild (AHI ≥ 5 to < 15/h), moderate (AHI ≥ 15 to < 30/h), and severe (AHI ≥ 30/h). RESULTS: From 139

[Research Progress of Treg/Th17 in the Treatment of Chronic Obstructive Pulmonary Disease with Lung Cancer].

Chronic obstructive pulmonary disease (COPD) and lung cancer are global high incidence and high mortality diseases, which seriously increase the socio-economic burden. Smoke exposure, genetic susceptibility and chronic inflammation are common susceptible factors. At present, abnormal inflammatory immune response plays an important role in the occurrence and development of the two diseases. In the process of immune response, tumor microenvironment (TME) is gradually produced, which is beneficial to angiogenesis and immunosuppression, and finally leads to immune escape of tumor cells, leading to

The EGFR-ADAM17 Axis in Chronic Obstructive Pulmonary Disease and Cystic Fibrosis Lung Pathology.

Chronic obstructive pulmonary disease (COPD) and cystic fibrosis (CF) share molecular mechanisms that cause the pathological symptoms they have in common. Here, we review evidence suggesting that hyperactivity of the EGFR/ADAM17 axis plays a role in the development of chronic lung disease in both CF and COPD. The ubiquitous transmembrane protease A disintegrin and metalloprotease 17 (ADAM17) forms a functional unit with the EGF receptor (EGFR), in a feedback loop interaction labeled the ADAM17/EGFR axis. In airway epithelial cells, ADAM17 sheds multiple soluble signaling proteins by proteolysi
