Primary intestinal diffuse large B-cell lymphoma forming multiple lymphomatous polyposis.

Multifocal and skip involvement is quite a rare developmental pattern for primary gastrointestinal lymphomas. A 25-year-old male patient with diffuse large B-cell lymphoma of the small intestine, with macroscopic features and clinical aspects imitating Crohn's disease and attracting attention with cobblestone-like appearance, is presented herein together with the clinical and pathological features.Multiple ulcerated lesions were also observed infiltrating the serosa with polypoid appearance, 2.5 cm in largest diameter, within the resected jejunoileal specimen, which displayed patchy, healthy-a

[Marfan syndrome complicated with CD5+ CD10+ diffuse large B-cell lymphoma].

Marfan syndrome (MFS) is caused by mutations in the gene encoding fibrillin. A 35-year-old man with MFS visited a local physician because of a sore throat. His left tonsil gradually became swollen and he was referred to our department. Histopathological examination of tonsil biopsy specimens showed diffuse proliferation of lymphoma cells with large nuclei. The tumor cells showed CD5+, CD10+, CD20+, BCL-6+, and MUM-1-. Based on these findings, the patient was diagnosed with CD5+ CD10+ diffuse large B-cell lymphoma (DLBCL). Chemotherapy combined with rituximab was administered and complete respo

Coexistence of diffuse large B-cell lymphoma within pontocerebellar angle schwannoma.

We present an unusual case of diffuse large B-cell lymphoma within pontocerebellar angle schwannoma in 62-year-old woman. The patient suffered for 5 months with V, VII and VIII nerves paresis and with cerebellar ataxia. CT scan demonstrated large hyperdensive mass in cerebellopontine angle translocating cerebellar hemisphere and cerebral trunk. The patient was subjected to surgery and the tumour was removed totally by suboccipital retromastoidal right craniectomy approach. Histopathological examination revealed schwannoma infiltrated with high grade B-cell lymphoma. The patient did well follow

B-cell acute lymphoblastic leukemia as a secondary malignancy following diffuse large B-cell lymphoma.

Secondary acute lymphoblastic leukemia (ALL) is a rare disease that has not been well characterized compared with secondary myelodysplastic syndrome or secondary acute myeloid leukemia. We present a report of two patients who developed ALL following complete remission of diffuse large B-cell lymphoma (DLBCL). The first case is more consistent with a therapy- related ALL as a PCR analysis of bone marrow aspirate revealed a distinct clone and the mixed-lineage leukemia gene rearrangement, commonly associated with exposure to topoisomerase II inhibitors. The second case is more consistent with cl

Histological Transformation and Clonal Relationship of Subcutaneous Marginal Zone B-Cell Lymphoma and Diffuse Large B-Cell Lymphoma.

Histological transformation (HT) is an exceptionally uncommon and poorly understood event where a low-grade or indolent B-cell lymphoma transforms into a more aggressive entity, typically diffuse large B-cell lymphoma (DLBCL). The pathogenesis is unclear; however, HT is associated with a worse prognosis. This article reports a unique case of marginal zone lymphoma (MZL) limited to skin/subcutis (confirmed with PET-CT) that subsequently developed DLBCL, followed by nodal MZL. We explored phenotypic, molecular genetic, and cytogenetic findings in subcutaneous MZL with HT to DLBCL and subsequent

B-cell lymphoma, unclassifiable, with features intermediate between diffuse large B-cell lymphoma and burkitt lymphoma: study of 39 cases.

B-cell lymphoma, unclassifiable (B-UCL), with features intermediate between diffuse large B-cell lymphoma and Burkitt lymphoma, is a poorly characterized entity. Therefore, we investigated cases of B-UCL treated by the Nebraska Lymphoma Study Group (NLSG). We searched the NLSG registry for years 1985-2010 for cases of B-UCL. Immunohistochemical stains and fluorescence in situ hybridization studies for MYC, BCL2 and BCL6 gene rearrangements were performed. Among the 39 cases studied, 54% were male and 46% were female, with a median age of 69 years. The majority of patients presented with advanc

Metabolic vulnerabilities and therapeutic opportunities in diffuse large B-cell lymphoma.

Diffuse large B cell lymphoma (DLBCL), the most common type of non-Hodgkin lymphoma (NHL), exhibits considerable biological heterogeneity. While its classification has traditionally relied on genetic and transcriptomic features, emerging evidence points to distinct metabolic subtypes that may represent novel therapeutic vulnerabilities. Intriguingly, current chemoimmunotherapy regimens exert profound but non-specific effects on tumour metabolism, inadvertently exploiting metabolic dependencies yet without precision. Novel inhibitors targeting glucose, amino acid, lipid, and mitochondrial metab

CSNK1E sustains stemlike drug persistence in diffuse large B-cell lymphoma.

Relapsed or refractory (R/R) disease occurs in up to 40% of patients with diffuse large B-cell lymphoma (DLBCL) following first-line immunochemotherapy. However, the molecular mechanisms underlying drug persistence remain incompletely defined. In this study, we performed single-cell RNA and B-cell receptor sequencing on paired diagnostic and R/R samples from 8 patients who were either treatment-refractory or relapsed after remission, and validated our findings in 3 independent patient cohorts. We found that drug-persistent cells exhibited a transcriptional profile indicative of a less-differen

Prognostic impact of TP53 mutations in diffuse large B-cell lymphoma.

To evaluate the prognostic value of TP53 mutations in patients with diffuse large B-cell lymphoma (DLBCL). We retrospectively analyzed the clinical data and gene sequencing results of 253 newly diagnosed DLBCL. Survival and correlation analyses were performed. We further revealed significant prognostic heterogeneity among different TP53 hotspot mutations, with mutations at codons G245, R175, R273, and R282 indicating a poorer prognosis. Within the DBD, mutations in exons 5, 7, and 8 were associated with poorer PFS, while mutations in exons 5, 6, and 8 were linked to poorer OS. Additionally, mu

Diffuse Large B-cell Lymphoma Associated With Exposure to Agent Orange.

Agent Orange (AO) is a toxic herbicide carcinogen mixture of 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) that was used extensively during the Vietnam conflict of the 1960s and 1970s. We present a case of an elderly veteran who presented with a progressively enlarging right neck mass. During active combat in Vietnam, he experienced extensive exposure to AO. He was eventually diagnosed with stage I diffuse large B-cell lymphoma (DLBCL) with locally advanced/bulky disease. The patient was deemed higher risk for systemic disease given the skin breakthrough at presentation. Additional labs revealed

Primary uveal diffuse large B-cell lymphoma: A population-based study.

PURPOSE: Primary involvement of the uveal tract by diffuse large B-cell lymphoma is exceedingly rare and poorly characterized. Most reported ocular lymphomas in this location are indolent mucosa-associated lymphoid tissue lymphomas with favourable outcomes. We aimed to evaluate the epidemiologic and clinical features of primary uveal diffuse large B-cell lymphoma using a large population-based dataset. MATERIAL AND METHODS: We performed a retrospective analysis of cases recorded between 2000 and 2022 in the Surveillance, Epidemiology, and End Results (SEER) database. Eligible patients had his

Adult ileocolic intussusception due to primary diffuse large B-cell lymphoma.

Intestinal intussusception is an uncommon condition in adults. Unlike the idiopathic spontaneous occurrence in children, about half of the cases in this population are associated with malignant neoplasms. This report describes a 44-year-old male patient presenting with abdominal pain and signs of intestinal obstruction. Through a CT examination, the patient was diagnosed with ileocecal intussusception. A 3 cm lesion was found in the terminal ileum, and a segmental enterectomy was performed. An immunohistochemical analysis identified a large B-cell lymphoma. Due to the high association with tum

Diffuse Large B-Cell Lymphoma with Cardiac Metastasis: A Case Report.

BACKGROUND Diffuse large B-cell lymphoma (DLBCL) is an aggressive tumor derived from mature B cells and is the most common type of non-Hodgkin's lymphoma (NHL). Cardiac invasion is rare and heart rupture or cardiac arrest can cause increased risk. Patients with DLBCL progress rapidly and are prone to recurrence, although its 5-year survival rate is high. The clinical manifestations of these patients lack specificity and this can delay diagnosis. Herein, we present a rare case of DLBCL with cardiac and multiorgan metastases and discuss the diagnostic and therapeutic challenges. CASE REPORT A 63

Genetic prognostic markers in LymphGen-unclassifiable diffuse large B cell lymphoma.

Diffuse large B cell lymphoma (DLBCL), the most common subtype of non-Hodgkin lymphoma, has variable treatment responses and distinct molecular subtypes. Despite therapeutic advances, a significant number of patients experience treatment failure or relapse. Recent genetic subtyping methods, such as the LymphGen algorithm, classify DLBCL into molecular subtypes. However, a subset of cases remains categorized as "LymphGen-unclassifiable" ("Other" group in the LymphGen classification). These cases lack distinctive genetic features and present challenges for risk assessment. In this study, we aime

Primary gastric diffuse large B-cell lymphoma: A multicentre retrospective study.

Primary gastric diffuse large B-cell lymphoma (PG-DLBCL) accounts for the majority of extra-nodal DLBCL. Even so, literature is lacking on early, localised presentations. We studied a cohort of patients with stage I disease, diagnosed between 2006 and 2018, from six centres between Australia, Canada and Denmark. Our goal was to characterise outcomes, review treatment and investigate the role of interim positron emission tomography (iPET). Thirty-seven eligible patients were identified. The median duration of follow-up was 42.2 months. All received chemoimmunotherapy with 91.9% (n = 34) given r

Hypomethylating agent decitabine sensitizes diffuse large B-cell lymphoma to venetoclax.

Despite recent advances in the therapy of diffuse large B-cell lymphoma (DLBCL), many patients are still not cured. Therefore, new therapeutic strategies are needed. The anti-apoptotic B-cell lymphoma 2 (BCL2) gene is commonly dysregulated in DLBCL due to various mechanisms such as chromosomal translocation t(14;18)(q32;q21) and copy number alterations; however, targeting BCL-2 with the selective inhibitor, venetoclax, led to response in only a minority of patients. Thus, we sought to identify a rational combination partner of venetoclax to improve its activity against DLBCL cells. Utilizing a

Plasmablastic myeloma with a history of diffuse large B-cell lymphoma.

Plasmablastic myeloma is a rare subtype of multiple myeloma, accounting for 5-10% of all cases, associated with poorer survival rates than typical multiple myeloma. This case report presents a man with plasmablastic myeloma with a previous history of diffuse large B-cell lymphoma which was in remission. He presented with paraplegia due to spinal cord compression and severe weakness. Investigations revealed abnormal immunoglobulin levels, hypercalcaemia and 80-90% bone marrow infiltration by plasmablasts. MRI revealed multiple spinal metastases and an incidental 6.1 cm abdominal aortic aneurysm
