This longitudinal study aimed to assess the performance of the Facility-Equator system as mandibular overdenture (MO) retainers from a prosthetic perspective during 2 years of loading and to investigate the oral health-related quality of life (OHRQoL) and whether prosthetic events can affect the OHRQoL. Twenty-four patients (68.1 ± 7.51 years) reported their OHRQoL through the Geriatric Oral Health Assessment Index (GOHAI) and Oral Health Impact Profile (OHIP-EDENT) questionnaires before MO loading and after 1 and 2 years of usage. Prosthetic occurrences were recorded during this period. Data

Rehabilitation of maxillary atrophy with dental implants is challenging to the clinician despite the wide variety of surgical techniques available. Finding the right indication for a procedure is highly important for the long-term stability of dental implants. With the introduction of the concept of "teeth-in-a-day," clinicians have explored innovative techniques to attain the goal of immediate implant-supported provisional prosthesis. However, costs and comorbidities are limitations to advancing these techniques. This article focuses on algorithms to rehabilitate the atrophic maxilla with the

Patients who have unusually small mouths may have difficulty in obtaining dental care and maintaining good oral hygiene. The fabrication of conventional complete removable dental prostheses for an edentulous patient with microstomia is challenging because of the limited access to the oral cavity. Sectional collapsible complete removable dental prostheses were designed as hinged maxillary and mandibular complete dentures that can be folded for denture delivery. This design also prevented denture deflection during function by using the upper part of the prosthesis, minimizing the possibility of

This report describes the fabrication of a prosthesis to prevent obstructive sleep apnoea in edentulous patients. The objective of treatment in a 62-year-old man was to establish a comfortable protrusive and vertical position of the mandible that minimised hypopharyngeal obstruction nocturnally. An appliance was designed incorporating two concepts in the elimination of obstructive sleep apnoea: mandibular advancement, which maintains hypopharyngeal width nocturnally; and advancement of the tongue with the aid of a device holding the tongue in a protrusive position by vacuum pressure. This comb

Most edentulous patients prefer fixed restoration or detachable prostheses over conventional complete dentures. The case report aims to address and discuss the clinical and laboratory steps of implant-supported, bar-retained maxillary overdenture and mandibular implant-supported, metal-acrylic resin, screw-retained, fixed complete denture. Impression techniques and laboratory steps are discussed and demonstrated to minimize the postfabrication or insertion complication. It lets you achieve the passive fit of the prosthesis that maintains the osseointegration by reducing stress on implants and

Huntington's disease is a hereditary, progressive, neuro-degenerative disorder characterised by increasingly severe motor impairment, cognitive decline and behavioural manifestations leading to functional disability. Dyskinesia and hyperkinesia of the tongue and the peri-oral musculature make it impossible for the patient to wear a conventional complete denture, despite an adequate alveolar ridge. The present paper reports on a patient with Huntington's disease who was rehabilitated with a mandibular overdenture supported by two endosteal implants. One year follow-up examination showed that th

PATIENT: A fifty-year-old man received embolization for mandibular hemangioma two years ago, and the surgeon had placed four implants in his mandible and made a removable denture upon the implants. His denture however fractured repeatedly in the past years. After examination and communication with the patient, an implant-assisted overdenture incorporating bar attachment combined ERA attachment and Locator abutment was re-fabricated. A sixteen months follow-up showed acceptable outcome.
DISCUSSION: There are some guidelines on design of implant over-denture, the site of implant will affect the

The main aim of this study was to perform a complex biomechanical analysis for a custom-designed temporomandibular joint (TMJ) prosthesis in combination with a fibular free flap in a pediatric case. Numerical simulations in seven variants of loads were carried out on 3D models obtained based on CT images of a 15-year-old patient in whom it was necessary to reconstruct the temporal-mandibular joints with the use of a fibula autograft. The implant model was designed based on the patient's geometry. Experimental tests on a manufactured personalized implant were carried out on the MTS Insight test

To tackle a large midline diastema and generalized spacing existing before extraction often poses a challenge to the treating prosthodontist. The situation becomes even more complicated if the patient is a teenager, with multiple missing teeth, associated deep bite and where the jaw bone growth has not yet been completed. Possible treatment options would include a removable prosthesis, a fixed partial denture or an implant supported prosthesis. Treating such cases with a simple removable prosthesis cannot be justified if a deep bite existed which would result in posterior disocclusion. Also a

This case report presents inferior alveolar nerve (IAN) repositioning as a viable approach for implant placement in the mandibular molar region, where challenges of severe alveolar bone width and height deficiencies can exist. Two patients requiring implant placement in the right mandibular molar region underwent nerve transposition and lateralization. In both cases, inadequate alveolar bone height above the IAN precluded the use of short implants. The first patient exhibited an overall low alveolar ridge from the anterior to posterior regions, with a complex relationship with adjacent implant

OBJECTIVE: A 3D finite element model of the mandibular second molar and its periodontal ligament (PDL) was designed to study the impact of different occlusal contact points on tooth mobility and to identify more stable occlusal contact patterns for precise clinical adjustments and prosthesis design. METHODS: A 300 N static load was applied vertically to the tooth’s occlusal surface. Two contact types were evaluated: cusp tip-fossa contact (Model 1) and cusp-fossa tripod contact (Model 2). The study focused on tooth mobility in 94 occlusal contact patterns. RESULTS: Increased contact points red

BACKGROUND: Segmental bone defects of the mandible result in the complete loss of the affected region. We had incorporated the pressure-reducing device (PRD) designs into the customized mandible prostheses (CMP) and conducted a clinical trial to evaluate this approach.
METHODS: Seven patients were enrolled in this study. We examined the association among the history of radiotherapy, the number of CMP regions, the number of chin regions involved, and CMP exposure.
RESULTS: We included five men and two women with an average age of 55 years. We excised tumors with an average weight of 147.8 g a

A comprehensive rejuvenation of the neck depends on accurate analysis of the lower face and neck with attention to the contours and deep-lying structures. Although many surgeons address the well-recognized changes in skin and soft tissue that occur with aging, we believe bone resorption is also an important component. Loss of bone volume leads to loss of support for the soft tissues of the face. The result is soft tissue ptosis and loss of angularity between the various planes of the face. Initially, there is loss of the submental shadow and loss of height of the mandibular ramus. The gonial a

INTRODUCTION: The use of vascularised composite free flaps (VCFF) has become a widely accepted method for primary reconstruction of mandibular defects. Adjuvant or neo-adjuvant radiotherapy (RTH) increases susceptibility to trauma and infections. The aim of this study is to compare the incidence of local complications after mandibular reconstruction with vascularised composite free flaps, related to pre- or postoperative radiotherapy. The effects of these complications on functional rehabilitation with a dental prosthesis fixed on bone implants are also studied.
METHODS: Between 1990 and 1999

Prosthetic rehabilitation of mandibular defects is challenging for prosthodontists. Prosthetic treatment strategies require a detailed clinical evaluation of the patient. The first consideration is the mandibular continuity as the residual mandible deviates toward the site of the defect in segmental mandibulectomy without rigid reconstruction. In this condition, the initial prosthetic intervention often focuses on mandibular guidance therapy using interim appliances to re-educate muscles and re-establish acceptable occlusal relationships. Definitive restorations include conventional removable

A significantly successful procedure for tongue stabilization, used in six cases of severe war injuries to the mandible, which prevents the serious complications of tongue prolapse, is here presented. The procedure consists of a the insertion of horseshoe-shaped Kirschner wire, the ends of which are fixed to the posterior mandibular segments, thus acting as a scaffolding for the lacerated soft tissues of the anterior two-thirds of the tongue, floor of the mouth and the submandibular region. This technique provides an acceptable appearance and assists feeding, reduces dribbling and dryness of t

Total joint replacement (TJR) with a prosthesis can be indicated for patients with severe temporomandibular joint (TMJ) dysfunction. Surgical accuracy is necessary for correct translation of the preoperatively predicted functional outcome, wear, and biomechanical behaviour of the patient-specific TMJ-TJR prosthesis. This study describes the first clinical applications of the patient-specific TMJ-TJR prosthesis according to the Groningen principles (G-TMJ-TJR), which was developed and validated in a prior human cadaver test study. The aim of this study was to validate the accuracy of placement
