Stewart Treves Syndrome in a Woman with Mastectomy.

Stewart Treves Syndrome is an angiosarcoma generally seen long time after radical mastectomy in breast carcinoma patients in chronic lymphoedema area. It's an angiosarcoma developed on a long standing lymphoedema, following a radical mastectomy. An 86-year-old woman was referred to our Dermatology outpatient clinic which developed a giant erythematous, oedematous, purplish lobulated plaque on her right forearm anteromedially with a few satellite erythematous papulonodules on her arm. The pathology revealed spindle-shaped tumour cells invading dermis with vascular differentiation into the subcu

Synchronous, bilateral mastectomy.

Sixty-four patients (mean age, 51 years) had mastectomies which were synchronous and bilateral. Sixty-one premastectomy biopsies (bilateral, 34 and unilateral, 27) demonstrated the following: invasive carcinoma, 17; noninvasive carcinoma, 24; combination of above, 10; and benign disease, 10. Twenty-two patients had bilateral mastectomy because of bilateral positive biopsy. Twenty-nine patients with unilateral carcinoma on biopsy had bilateral mastectomy. Thirteen patients had bilateral mastectomy despite benign disease only on biopsy (10) or no biopsy (3). Ten unexpected carcinomas (34%) were

Therapy of primary breast cancer.

Most breast cancers are multicentric in origin. They drain into two primary lymphatic depots-the axilla and internal mammary chain of nodes. The incidence of metastasis to the internal mammary nodes rises as the location of the primary tumor approaches to the sternal margin of the breast. One hundred and thirty-seven patients primarily with in situ and non-infiltrating intraductal carcinoma were treated adequately by simple mastectomy and axillary dissection with preservation of the pectoral muscles. All have remained free of disease. Infiltrating cancers arising in the lateral portion of the

Stewart-Treves syndrome: lymphangiosarcoma following mastectomy.

Lymphangiosarcoma (LAS) is an aggressive, malignant vascular tumor following long-lasting chronic lymphedema. Patients with LAS demonstrate a history of breast cancer treated by radical mastectomy in the majority of patients. In the 1960s the incidence of LAS in patients with a 5-year survival after radical mastectomy varied from 0.07 to 0.45%. Today, due to changes in the operative techniques of breast cancer, less chronic lymphedema is seen with only a scant number of LAS patients. The etiology of this enigmatic tumor is not yet completely understood. Histologically, LAS arises from vascular

Pathobiology of breast cancer--treatment implications.

In most patients (approximately 85%), breast cancer at the time of diagnosis is already a systemic disease. Multicentricity (20-40%) and synchronous (5-10%) and metachronous (15-30%) bilaterality are indicative of etiologically similar noxae. Ductal and lobular carcinoma in situ become invasive in approximately 50% of patients. Whereas ductal carcinoma in situ is mainly diagnosed clinically (lumpiness, tissue irregularity), lobular carcinoma in situ, a small, nonpalpable lesion, is usually discovered accidentally following biopsy for fibrocystic disease and/or suspicious mammography. Treatment

[Surgical treatment of the mammary carcinoma according to stage (author's transl)].

Improvement of the results of treatment was achieved only when an increased number of patients were diagnosed in stage I. The prerequisite for limitation of radical surgery is histomorphologic classification of the stages. The central axillary group of lymph nodes appears to be representative of lymph node involvement. Differentiated methods of operations range from local excision to mastectomy in case of precancer. In histologic stage I, modified radical mastectomy; is the treatment of choices; operations with further limitations are recommended only within the scope of controlled studies. In

[Conservative treatment of breast cancer. A therapeutic trial in progress at the Cancer Institute of Milan].

The report describes a clinical trial on conservative surgery in progress at the National Cancer Institute of Milan, Italy. The randomized clinical trial compares radical mastectomy with a more conservative procedure consisting of mammary resection plus axillary dissection plus radiotherapy. The resection comprises an entire quadrant of the breast together with the overlying skin. After surgery the patients receive 6 000 rads to the residual breast tissue over five to six weeks, starting 15 days after operation. The cosmetic results are satisfactory in the majority of cases. The trial is limit

Paget's disease of the male breast.

Histologically proved Paget's disease of the breast with negative hormonal receptor assay in a 73-year-old man induced a review of the 27 valuable cases of the literature. Our patient is disease-free, eight years after treatment by modified radical mastectomy and adjuvant irradiation. Peak incidence of the disease is between 50 and 70 years of age. The most frequent presenting signs reported are ulcerations and eczematous lesions of the nipple (71.4%) with discharge and bleeding in 39.3% of the cases. Other findings are a palpable mass (42.8%) and involved axillary lymph nodes (53.5%). The tre

Axillary metastases from an unknown primary source.

The appropriate therapeutic approach is considered in patients who present with carcinomatous metastases in the axillary lymph nodes and in whom the site of the primary tumor remains obscure. Eight patients with axillary lymph node metastases are reported, in five of whom the location of the primary tumor could not be established. In three patients the primary growth was ultimately found to be in the adjacent breast. Early death occurred in only one patient, and in this instance the primary site proved to be in the breast. The remaining seven patients have all shown a relatively good prognosis

Lymphangiosarcoma in chronic lymphoedema. Stewart-Treves syndrome.

A case of post-mastectomy lymphangiosarcoma is reported. Lymphangiosarcoma is an extremely rare but highly lethal complication of chronic lymphoedema. Our patient was treated by amputation and died 6 months later. Treatment of post-mastectomy lymphangiosarcoma is still unsatisfactory. Early recognition and radical ablative surgery seem to provide best chance for survival.

Radiation therapy following mastectomy for axillary node-positive breast cancer: indication of chest wall irradiation.

This retrospective study was conducted to determine the indication of chest wall irradiation following mastectomy in axillary node-positive breast cancer patients. Between 1982 and 1993, 103 women with axillary node-positive breast cancer received postoperative radiation therapy following mastectomy using the hockey-stick field, which included the ipsilateral supraclavicular fossa and internal mammary nodes, without the chest wall. Ages ranged from 33 to 73 years (median: 47). Thirty-five patients underwent modified radical mastectomy, 48 radical mastectomy, and 20 extended radical mastectomy.

Results of the National Cancer Institute early breast cancer trial.

In 1979, a prospective, randomized trial was begun at the National Cancer Institute comparing lumpectomy, axillary dissection, and radiation therapy (XRT) to modified radical mastectomy for the treatment of invasive breast cancer, clinical stage T1-2, N0-1, M0. Treatment in the radiation arm consisted of full axillary dissection and gross removal of the tumor, followed by 4500 to 5000 cGy to the whole breast and a boost to the tumor bed of 1500 to 2000 cGy. Patients with positive axillary dissections received radiation to the supraclavicular nodes, in addition to cyclophosphamide and doxorubic

Fatty changes and metastases in axillary lymph nodes.

A variety of pathological changes with clinical and prognostic significance is described in axillary lymph nodes removed at mastectomy. We studied 572 lymph nodes removed from 43 women and 1 man during a radical or modified radical mastectomy performed for carcinoma of the breast. We found that in about one-third of the specimens some degree of fat infiltration, or changes, was present; metastases were found in either fatty or nonfatty lymph nodes. We conclude that the presence of fat in axillary lymph nodes does not influence implantation of tumor cells from a primary carcinoma of the breast

Influence of the timing of physiotherapy upon the lymphatic complications of axillary dissection for breast cancer.

A prospective randomized study was carried out to discover the influence of the timing of shoulder physiotherapy after-axillary dissection for breast cancer upon the incidence and duration of lymphatic fluid production and seroma after these operations. Sixty-eight patients underwent a modified radical mastectomy, 31 were submitted to early physiotherapy and 37 to delayed physiotherapy after removal of the suction drainage. In 32 patients this surgery was conservative of the breast; in 16 the physiotherapy was early and in 16 delayed. The shoulder was left free when the physiotherapy was delay

Immediate Breast Reconstruction with Latissimus Dorsi Flap and Implant: The Innovation of Flap Design and Transfer.

BACKGROUND: Modified radical mastectomy is the mainstream surgical treatment for breast cancer in which nipple-areolar complex and fusiform skin may be excised due to cancer invasion. Latissimus Dorsi (LD) flap combined with prosthesis implantation is a common method of immediate breast reconstruction after modified radical mastectomy. In this study, we introduced the design and transfer of flap based on the mastectomy incision for better practicality and appearance. METHODS: From January 2016 to December 2022, a retrospective analysis was performed in patients who received immediate breast r

Solitary adrenal metastasis from invasive ductal breast cancer: an uncommon finding.

BACKGROUND: Invasive ductal carcinoma (IDC) of the breast usually metastasizes to the lungs, liver, bones and brain. Solitary adrenal metastasis is extremely rare. Due to the rarity of this condition, the optimal treatment is unclear. We report the first case of IDC of the breast metastasizing solely to the adrenal gland after a modified radical mastectomy but having a long-term disease-free survival while treated merely by a left adrenalectomy. CASE PRESENTATION: A 64-year-old woman was found a left adrenal mass on a follow-up visit two years after taking a right modified radical mastectomy

Intraductal "noninfiltrating" carcinoma of the breast.

We are reviewing 40 patients with noninvasive intraductal carcients; 10% developed oppostie breast cancer of the invasive variety. Twenty-one patients underwent radical mastectomy in which one patient was found to have a positive axillary node. The 19 remaining patients had total mastectomy alone. No recurrence of death attributed to intraductal carcinoma was found in 39 patients available for follow-up.
