Surgical therapy of chronic mastoiditis with cholesteatoma.

One hundred fifty-seven ears treated surgically for chronic mastoiditis with cholesteatoma are reviewed. The selection of an operative procedure ("canal wall up" or "canal wall down") was dictated by the extent of pathologic indications in relation to the size of the patient's mastoid. The incidence of recurrent or residual cholesteatoma, the hearing results, and the frequency of visits required for mastoid cavity care are reported.

Bacteroides meningitis complicating chronic mastoiditis.

A 70-year-old man with a history of chronic middle and external ear suppuration presenting with acute meningitis due to Bacteroides uniformis is described. Anaerobic organisms including Bacteroides species were cultured from the affected external auditory meatus. Administration of metronidazole led to complete clinical recovery and a cholesteatoma was subsequently surgically evacuated. Chronic suppuration in the middle ear and paranasal sinuses is the commonest primary cause of cerebral abscess and in this setting meningitis is associated with a particularly high mortality. The major role of a

Infectious mononucleosis, mastoiditis, and facial paralysis.

To our knowledge this is the first report of a patient with infectious mononucleosis associated with a mononuclear granulomatous mastoiditis and facial paralysis. Pathologic examination of tissue from this patient, removed at the time of radical mastoidectomy, provides a basis for further speculation about the relationship between facial paralysis and infectious mononucleosis.

Treatment of chronic mastoiditis by grafting of mastoid cavities with autologous epithelial layers generated by in vitro culture of buccal epithelium.

Autologous cultured epithelial layers were established from biopsies from the mucosa of the cheek, a non-keratinizing region of the oral cavity. These were grafted to the unepithelialized mastoid cavities of nine patients with chronic mastoiditis and severe otorrhoea varying from two to 30 years' duration. All procedures were performed on an out-patient basis, with no anaesthesia except for topical anaesthesia for the mucosal biopsy. In seven of the patients the grafts took well, with complete resolution of the otorrhoea for a minimum follow-up period of eight months. In one patient there was …

Quality of life after revision mastoidectomy with mastoid obliteration.

AIMS: To evaluate the postoperative quality of life (QoL) after revision canal wall down mastoidectomy with mastoid obliteration (rCWD). MATERIAL AND METHODS: A retrospective analysis was conducted on patients treated by rCWD for cholesteatoma between 2016 and 2019. A control group including all patients treated by primary canal wall down with mastoid obliteration (pCWD) for cholesteatoma between 2009 and 2014 was used for the comparison of the postoperative QoL, assessed by the COMQ-12. RESULTS: The rCWD and pCWD groups respectively counted 38 and 78 patients with an average follow-up of 30…

Facial nerve palsy following mild mastoid trauma on trampoline.

We present a case report of a 14-year-old boy who was jumping on a trampoline when he struck his right mastoid on a support pole. The following day, he developed a right-sided facial droop and inability to close his right eye. He presented to the emergency department, where CT of his temporal bone was negative and he was started on prednisone. Over the next month, he had spontaneous recovery of his facial nerve (FN) function. In cases of traumatic FN palsy, urgent referral to otolaryngology is needed, even without a fracture of the temporal bone, as edema within the facial nerve could require …

Mastoid obliteration with S53P4 bioactive glass in cholesteatoma surgery.

CONCLUSION: Evaluation of the follow-up of 67 patients shows that S53P4 bioactive glass (BAG) granules are safe and effective as obliteration material in cholesteatoma surgery. OBJECTIVES: To investigate the safety and efficacy of mastoid obliteration using S53P4 BAG in cholesteatoma surgery. Clinical outcomes were infection control (Merchant's grading), cholesteatoma recidivism, and audiometric performance. METHODS: Retrospective follow-up study at the Diakonessenhuis, Utrecht, the Netherlands. Eighteen young (age <17 years) and 49 adult (age ≥17 years) patients treated for cholesteatoma un…

Mastoid cavity obliteration with combined palva flapand bone pâté.

INTRODUCTION: This study was designed to evaluate the usefulness of mastoid cavity obliteration with combined bone pâté and Palva flap in the prevention of problematic mastoid cavities after canal wall down mastoidectomy. MATERIALS AND METHODS: In a prospective longitudinal study with a mean follow-up of 28 months conducted between 2008-2012, a series of 56 ears in 48 patients with chronic otitis media due to a cholesteatoma underwent canal wall down mastoidectomy that their mastoid cavity obliterated with combined bone pâté and Palva flap. Seventeen (30%) ears were managed via revision surge…

Mastoid infection caused by entomophthorales: a rare fungal disease.

OBJECTIVE: To report an unusual case of fungal mastoiditis caused by entomophthorales in an immunocompetent patient, and its management. METHOD: Case report with a review of the literature. CASE REPORT: A 13-year-old girl presented to us with a mastoid abscess. Entomophthoromycotic infection of the mastoid was diagnosed on histopathological examination, and subsequently treated with surgical debridement and amphotericin B injection. CONCLUSION: This is the first reported case of mastoid abscess secondary to entomophthoromycosis. Early detection and treatment contributed to this patient's go…
