A classification and construction of congenital lateral facial clefts.

BACKGROUND: The repair of the lateral or transverse facial cleft is a surgical challenge on the account of the abnormal positioning and appearance of the cleft. MATERIALS AND METHODS: Over a twenty-seven year period, 22 lateral facial cleft cases were evaluated at a cleft lip (CL) and palate clinic and seven children underwent reconstruction of the lateral CL. RESULTS: Twenty-two of 3187 (0.69%) cases presented with a lateral CL. Five of these 22 cases (23%) had a bilateral, eight (36%) had a right-sided and nine (41%) had a left-sided cleft. The evaluation of these cases resulted in a new c

Heterotopic polyodontia in a cynomolgus monkey.

A case of heterotopic polyodontia was detected in a 6-year-old male laboratory cynomolgus monkey. A heterotopic tooth had erupted through the right maxillofacial area at the side of the nose, with a crustated surface. Macroscopically, the tooth was not connected with the right parotid. Radiography and histopathology of the transverse section of the lesion showed almost normal propriodentium containing alveolar bone and partial continuity with the maxillary bone. We believe this is the first case description of heterotopic polyodontia in a monkey.

Retinal arteriovenous communication.

BACKGROUND: Disturbances in development of the embryonic vascular system, though uncommon, may cause formation of retinal arteriovenous communications (RAC). Such anomalies may also involve the intracranial, orbital, and maxillofacial blood vessels. Field loss, neovascular glaucoma, optic atrophy, Parinaud syndrome, hemiparesis, hemiplegia, and proptosis may be associated with RAC. CASE REPORT: A 27-year-old woman was referred for photography of a vascular anomaly in her left eye. I noted a large arteriovenous malformation joining major temporal branches of the left central retinal artery and

Radiographic features of plasma cell leukemia in the maxilla: A case report.

Plasma cell leukemia (PCL) is an aggressive form of multiple myeloma where there is hematogenous spread of abnormal plasma cells into the periphery. This is opposed to multiple myeloma, where the abnormal plasma cells stay in the bone marrow. PCL is more common in males than females, and is also more common in African-Americans than Caucasians. Signs and symptoms of PCL include, but are not limited to, renal insufficiency, hypercalcemia, anemia, lytic bone lesions, thrombocytopenia, hepatomegaly, and splenomegaly. Here, we discussed a case of a 71-year-old Caucasian female recently diagnosed w

Subcutaneous Emphysema and Pneumomediastinum Triggered by Nasotracheal Intubation: A Case Report.

Nasotracheal intubation (NTI) is frequently chosen in oral and maxillofacial surgery to provide unobstructive access to the surgical field. Despite being rare, subcutaneous emphysema (SE) and pneumomediastinum (PM) caused by nasopharyngeal wall injury are serious complications. In this report, we present the case of a 74-year-old woman who developed right cervical SE and PM during anesthesia induction for oral surgery under general anesthesia associated with nasopharyngeal wall injury due to NTI. She had no remarkable medical history or abnormal preoperative findings. During intubation, swelli

Management of dystonia of the lateral pterygoid muscle with botulinum toxin A.

Oromandibular dystonia (OMD) is a rare neurological condition infrequently seen by oral and maxillofacial surgeons which may result in inappropriate deviation of the mandible, subluxation and intraoral soft tissue trauma. A case is presented of a patient suffering from spasmodic torticollis and lateral pterygoid dystonia and currently under treatment with botulinum A toxin (Botox) injections. Botox has emerged as the most effective form of symptomatic treatment for abnormabilities in muscle movement, namely dystonia with oromandibular symptoms.

Deglutition Syncope: A Case Report and Review of the Literature.

Deglutition syncope (DS) is a rare, neurally-mediated syncopal syndrome arising from an aberrant vagotonic reflex during swallow-associated esophageal dilation. Its association with gastroesophageal disorders often prompts gastroenterology consultation. An 89-year-old man with recent dysphagia and otalgia was admitted after a syncopal episode occurred while eating. Esophageal imaging and endoscopy demonstrated no causative abnormalities. Maxillofacial imaging revealed chronic sinusitis and mastoiditis. Telemetry monitoring demonstrated high-grade atrioventricular block and pause associated wit

Nasopharyngeal carcinoma. Computed tomographic imaging of four cases.

Nasopharyngeal carcinoma occurs with great frequency in Asia. It is an infiltrative tumor that often produces no symptoms early in its course. However, the proximity of the nasopharynx to the nasal cavity, eustachian tubes, masticatory muscles, and cranial nerves can allow extension of the lesion into these structures, resulting in numerous maxillofacial abnormalities. Computed tomography is effective in imaging the development of nasopharyngeal carcinoma in the nasopharynx as well as the subsequent spread of the neoplasm to adjacent areas. Four cases of nasopharyngeal carcinoma are presented …

Computed tomography of orbital medial wall fractures.

Increasing experience with computed tomographic evaluation of head trauma reveals this radiologic modality to be superior to conventional polytomography in the assessment of certain areas of maxillofacial injury. A comparison of hypocycloidal polytomography and computed tomography in nine patients with fractures of the medial wall of the orbit reveals CT to be superior in localization of the fracture site, delineation of displaced bone fragments, and recognition of herniation of intraorbital contents into the ethmoid labyrinth. Information regarding damage to the nasolacrimal duct and associat

Craniofacial abnormalities in obstructive sleep apnoea: implications for treatment.

Obstructive sleep apnoea (OSA) is a common disorder, and is characterized by repetitive closure of the upper airway during sleep. Upper airway narrowing and sleep-induced loss of muscle tone are important factors in the development of OSA. Over the last decade there has been a growing recognition that craniofacial abnormalities occur commonly in OSA patients. The more commonly identified abnormalities include mandibular deficiency, an inferiorly placed hyoid bone relative to the mandibular plane, a narrowed posterior air space, a greater flexion of the cranial base, and elongation of the soft

Brown tumor of mandible in association with primary hyperparathyroidism: a case report.

Brown tumors are giant cell focal lesion that arises as a result of abnormal bone metabolism in patients with hyperparathyroidism (HPT). The lesions localize in areas of extensive bone resorption, which is replaced by fibrovascular tissue and giant cells with abundant deposits hemorrhage and hemosiderin. A rare case of brown tumor of mandible in a 22-year-old woman is reported here. This case emphasizes the importance of a detailed systemic investigation for all lesions in the maxillofacial region and also discusses the diverse presentations associated with primary HPT.

Most frequent tumors in maxillofacial area rehabilitated through surgical reconstruction and prostheses.

Cancer is regarded as the abnormal cellular multiplication; it is not controlled by the organism; and its cells present a differentiated DNA. Initially, the disease does not show clinical signs, but it can be diagnosed by laboratorial examinations. When tumors are present in the maxillofacial area, they can entail the loss of these area organs, which become responsible for the carrier's social environment exclusion. This paper aimed at showing, through a literature review, the cancers that more commonly happen in the face and the possibilities of regenerating in the patient mutilated through s

[Research progress in diagnosis and treatment of non-tumorous salivary gland diseases].

Salivary gland diseases are common disorders in the oral and maxillofacial region, mainly classified into two categories: Tumorous and non-tumorous. Non-tumorous salivary gland diseases include various types such as salivary gland inflammation, Sjögren syndrome, granulomatous diseases, and developmental abnormalities. Some of these diseases are local lesions, while others are closely associated with systemic diseases, often accompanied by impaired salivary secretion function, leading to xerostomia and secondary lesions. Over the past more than 20 years, the Salivary Gland Disease Research Cent

Multiple endocrine neoplasia-2B presenting with orthodontic relapse.

The multiple endocrine neoplasia (MEN) syndromes are a relatively uncommon group of genetic disorders characterized by the development of tumors in various endocrine organs. MEN type 2B is of particular interest to the dental profession because of its oral manifestations, which are often some of the earliest clinically detectable signs of the disorder. Early identification of this syndrome is critical because affected patients often develop a characteristic malignancy, medullary carcinoma of the thyroid, at a very early age. We describe a 17-year-old male whose initial diagnosis of MEN-2B was
