A 23-year-old man complaining of right-sided chest pain was admitted to our hospital for further examination of an abnormal shadow on chest X-ray films. Initially, we suspected the abnormal shadow was that of a pleural effusion in the right thorax. Computed tomographic scans disclosed a tumor in the anterior mediastinum, adjacent to the abnormal shadow. After closer examination, particularly by fluoroscopy on catheterization, we recognized that the abnormal shadow was a giant cyst, and that it connected with a solid mass. There were no abnormal laboratory data on admission. An examination of f

Primary hydatid cyst of the mediastinum is rare. We report a patient with a primary mediastinal hydatid cyst which ruptured into the aorta, resulting in bilateral popliteal embolism. The contrast-enhanced computed tomographic features represent a pathognomonic sign of a communicating rupture of an echinococcal cyst into the aorta. Comparison is made with ultrasonography and aortography.

Bronchogenic Cysts (BCs) are benign congenital malformations commonly located in the mediastinum. In recent years the development of antenatal diagnosis has changed the BCs management allowing an earlier minimally invasive approach. We report a case of an asymptomatic 8-months-old girl with antenatal diagnosis of subcarinal posterior mediastinal BC. Thoracoscopic excision of the cyst was successfully performed. The management of antenatally diagnosed BCs is discussed. Thoracoscopic treatment of BCs is safe and effective with the advantage of a reduced morbidity compared to thoracotomy.

Here, we describe the case of a 56-year-old female patient who was diagnosed with an anterior mediastinal cyst measuring 26 × 16 mm in size. An endobronchial ultrasound-guided transbronchial needle aspiration was performed, and punctures occurred three times. The patient was then prescribed cefditoren pivoxil. Three days after the procedure, the patient developed infective mediastinitis. Panipenem/betamipron, clindamycin, and human immunoglobulin were administered, and her symptoms improved over 2 weeks. Five months after developing mediastinitis, surgical resection of the cyst was performed w…

Foregut duplication cysts are rare congenital anomalies of enteric origin. The diagnosis is usually made in infancy. We report the unusual case of a 71-year-old female presenting to the ENT department with shortness of breath and stridor due to an oesophageal reduplication cyst. The presentation, diagnosis and management of this potential pitfall for the unwary are outlined.

We report the case of a 70-year-old man with dense pleural adhesions who underwent an excision of a mediastinal bronchogenic cyst by video-assisted thoracoscopic surgery (VATS) through an extrapleural approach. The VATS approach for the diagnostic and therapeutic excision of a mediastinal tumor is easier and safer than an intrapleural approach if a patient has dense pleural adhesions.

Over the period 1983-1999 examination was done together with a surgical treatment of ten children with bronchogenic (BG) and enterogenic (EG) cysts of the mediastinum, their ages ranging between 2 months to 13 years, having been kept under medical surveillance in the clinic for thoracoabdominal surgery of developmental anomalies in children. In 80 percent of patients intrathoracic compression of the respiratory tracts caused by the above cysts was diagnosed. 87.5 percent of patients had inflammatory bronchopulmonary problems, 12.5 percent had secondary tracheomalacia as complications of BG and

We report a rare case with rupture of thymic cyst including mediastinal hemorrhage and hemothorax. A 68-year-old man was referred to our hospital for the treatment of right hemothorax. A chest roentgenogram following thoracentesis demonstrated a widened mediastinum. Chest computed tomograms revealed a large anterior mediastinal mass extending to the right pleural cavity and bilateral pleural effusion. Digital subtraction angiography showed a normal aorta and great vessels. A median stenotomy revealed a large encysted hematoma along the thymic cyst extending from the right anterior mediastinum

Current videoendoscopic technology and percutaneous techniques of exposure and dissection have been successfully applied to abdominal surgery with favorable results. Application of this technology to our practice of thoracic surgery is the basis of this report. Video-assisted thoracic surgery was performed in 36 patients for the following indications: Raynaud's syndrome, undefined pulmonary nodule, persisting spontaneous pneumothorax, T1 bronchial carcinoma, and mediastinal cyst. Videoendoscopic surgical procedures were accomplished using double-lumen endotracheal anaesthesia and a percutaneou…

BACKGROUND: A prenatal fetal mediastinal cyst is a benign disease. However, if a cyst enlargement grows, it may compress the adjacent organs and affect the fetal cardiopulmonary function. This study aimed to compare and analyze the prenatal ultrasound characteristics of different mediastinal cysts, and to evaluate the pregnancy outcome of the fetus and the factors affecting the prognostic of the fetus. To compare and analyze the prenatal ultrasound characteristics of different types of mediastinal cysts, and to evaluate the fetal pregnancy outcome and the influencing factors of fetal prognosis…

Mediastinal enteric cysts are very rare among in adults and usually asymptomatic. A 54-year-old male was referred to our hospital due to an abnormal shadow incidentally found on a chest X-ray at health check. Chest computed tomography scan revealed a cystic mass in the posterior and inferior mediastinum surrounded by diaphragm, inferior vena cava, and esophagus. Based on many reports of thoracoscopic esophagectomy in the prone position in recent years, we chose thoracoscopic resection of the mediastinal tumor in the prone position with artificial pneumothorax. The prone position with artificia

OBJECTIVE: The aim of this study was to evaluate the outcomes of video-assisted thoracic surgery (VATS) for mediastinal bronchogenic cyst (MBC) excision and investigate the surgical indication for MBC.
METHODS: We retrospectively reviewed all consecutive MBC patients who underwent surgical excision between April 2001 and June 2016. One hundred and nineteen patients were enrolled with a median age of 45.4 years and divided into two groups: anterior mediastinum group (n = 48), and middle and posterior mediastinum group (n = 71). VATS technique was initially performed for each patient. The cyst

We report the findings for 2 patients with a fluid-fluid level seen on endobronchial ultrasound (EBUS) images of bronchogenic cysts. The EBUS images demonstrated a well-circumscribed cyst with a fluid-fluid level showing an anechoic upper part and a relatively hyperechoic lower part. A fluid-fluid level on EBUS imaging of a bronchogenic cyst, which can help confirm the cystic nature of the lesion, has not been previously reported. EBUS-based confirmation of these cysts using fluid-fluid levels may help avoid unnecessary aspiration of the lesions.

Mediastinal enteric duplication cysts are a rare congenital malformation encountered mainly in neonates and infants. It is a distinct entity within the family of foregut duplication cysts. It can present with respiratory distress due to mass effect and hence surgical excision is the preferred treatment. Histologically, it is characterised by a double layered smooth muscle wall with intestinal lining epithelium. We report a case of mediastinal enteric duplication cyst with aberrant pancreatic tissue in a neonate due to its rarity and early presentation. A neonate presented with respiratory dist

Fibrosing mediastinitis is a chronic disease process with a spectrum of etiology. We report a 51-year-old female who underwent incision and drainage procedure in the neck for deep neck and mediastinal abscess. Five years later she developed fibrosing mediastinitis. This lesion infiltrated from neck base into the upper mediastinum with tracheal compression and vessel encasement. She had resection of the lesion which proved to be a ruptured bronchogenic cyst with chronic inflammation. This rare case illustrates the importance of including inflammatory bronchogenic cyst in the etiology of deep ne

OBJECTIVE: In spite of improvements in imaging techniques, surgical procedures are often needed to diagnose and definitively treat mediastinal masses. The range of application of video-assisted thoracic surgery (VATS) in this context is still poorly defined. The present study describes and analyzes the experience of the Cooperative Group for Video-assisted Thoracoscopic Surgery of the Spanish Society of Pneumology and Thoracic Surgery (GCCVT-SEPAR), with the aim of determining the usefulness of the technique for treating mediastinal cysts and tumors.

Five observations of mediastinal brochogenic cysts are used to illustrate the diagnostic and/or therapeutic difficulties encountered in cases with an ectopic topography or complications. In two cases, one of which was clearly located in the posterior mediastinum and associated with a false erosive appearance of the nearby vertebral body, it was possible to establish the degree of ectopic displacement of the bronchogenic cysts, and to comprehend the pre-operative diagnostic difficulties and nosological problems associated with these sequestrations. As far as complications are concerned, one obs
