Nurse-Led Interventions to Mitigate High-Alert Medication Errors in Critical Care: A Systematic Review of Effectiveness and Implementation Strategies.

BACKGROUND: Intensive care units (ICUs) are high-risk environments where nurses administer the majority of high-alert medications, making them the final checkpoint in the medication-use process. However, evidence on nurse-led interventions specifically targeting high-alert medication errors has not been systematically synthesized across all ICU populations. AIM: This systematic review evaluated the effectiveness of nurse-led interventions in reducing high-alert medication errors in neonatal, paediatric and adult intensive care units and examined implementation strategies that enhance patient

Electronic prescribing and patient safety: a rigour-stratified systematic review of medication error, adverse drug events, workflow, and communication.

BACKGROUND: Electronic prescribing is promoted to improve medication safety. Whether its benefit differs between process failure and patient harm is rarely asked. OBJECTIVE: To synthesise the evidence on electronic prescribing and medication error, adverse drug events, workflow and communication against the designs able to establish each. METHODS: We searched PubMed, Scopus, Web of Science, CINAHL, Embase and ProQuest (1994 to 2024, English), reporting to PRISMA 2020 and PRISMA-S. Risk of bias was assessed at domain level with design-matched tools. Non-poolable metrics precluded meta-analysi

Interference of Residual 99m Tc-MIBI Activity in Syringe Needle on 99m Tc-DMSA Scan Interpretation; Medical Error.

A 4-month-old infant with a small right kidney underwent a 99m Tc-DMSA scan showing unjustifiable findings including global cortical loss in the left kidney, increased liver and bowel uptake. Retrospective review revealed reuse of a sterile needle contaminated with residual 99m Tc-MIBI, compromising scan quality and falsely elevating right kidney function. A repeat scan showed normal left kidney uptake and significantly reduced right kidney function. This case highlights the importance of employing safe techniques with disposable equipment, thorough workflow review, and transparent error repor

Two decades of medication administration errors related to pain management: Hospital admissions for nonopioid analgesics, antipyretics, and antirheumatics in Australia.

The rate of medication poisoning is constantly increasing globally, increasing the number of hospital admissions and the burden on the health system. In Australia, nonopioid analgesics, antipyretics, and antirheumatics medications cause a high rate of accidental poisonings. This study aims to study hospital admissions trends related to medication administration errors (MAEs; poisoning by, adverse effect of and underdosing of) of nonopioid analgesics, antipyretics, and antirheumatics-related in Australia from 1998 to 2022. This ecological study used hospital admissions data from the National Ho

Methotrexate-induced pancytopenia: clinical characteristics, medication errors, and outcomes in a tertiary care centre: a retrospective single-centre study.

Methotrexate (MTX) is a widely prescribed disease-modifying antirheumatic drug (DMARD) used at ≤ 25 mg/week in inflammatory conditions. Although effective, MTX-induced Pancytopenia remains a serious adverse event, often resulting from medication errors, idiosyncratic reactions, comorbidities, or polypharmacy. To comprehensively characterize methotrexate-induced Pancytopenia's clinical profile, risk factors, and outcomes. A retrospective analysis was conducted on patients diagnosed with MTX-induced Pancytopenia between 2015 and 2024. Pancytopenia was defined as WBC < 3,500/mm³, Hb < 11 g/dL, an

Incidence and root causes of medication errors by anesthetists: a multicenter web-based survey from 8 teaching hospitals in Ethiopia.

BAKGROUND: The operating room is a demanding and time-constrained setting, in comparison to primary care settings, where perioperative medication administration is more complicated and there is a high risk that the patient will experience a medication error. Without consulting the pharmacist or seeking assistance from other staff members, anesthesia clinicians prepare, deliver, and monitor strong anesthetic drugs. The purpose of this study was to determine the Incidence and root causes of medication errors by anesthetists in Amhara region, Ethiopia. METHODS: A multi-center cross sectional web

Patient-reported outcomes and medication errors during oral antitumour therapy in paediatrics: Results from the youngAMBORA care programme.

AIM: Increasing use of oral antitumour therapeutics (OAT) in paediatrics is associated with numerous advantages, but also with error-prone aspects. Based on the randomized AMBORA trial including adults treated with various OAT, we developed a tailored pharmacological/pharmaceutical care programme for children and their caregivers during OAT treatment (youngAMBORA). We now aimed to analyse the impact of the youngAMBORA care programme on medication safety by assessing (1) patient-reported outcomes (PROs) and (2) medication errors. METHODS: Depending on the intended OAT treatment duration, the y

Medication Preparation Errors Involving Very Small and Difficult-to-measure Volumes in a Pediatric Hospital: A 14-year Descriptive Study.

PURPOSE: Medication preparation errors are a major source of preventable harm in pediatric hospitals. Oral liquid medications, especially those involving difficult-to-measure or very small volumes, may increase the risk of such errors, yet supporting evidence is limited. This study aims to determine whether difficult-to-measure and very small oral liquid medication volumes are associated with higher rates of preparation errors in a pediatric hospital pharmacy. METHODS: We conducted a retrospective cohort study of all oral liquid medication preparations produced between January 2010 and Decemb

Medication improves velocity, reaction time, and movement time but not amplitude or error during memory-guided reaching in Parkinson's disease.

The motor impairments experienced by people with Parkinson's disease (PD) are exacerbated during memory-guided movements. Despite this, the effect of antiparkinson medication on memory-guided movements has not been elucidated. We evaluated the effect of antiparkinson medication on motor control during a memory-guided reaching task with short and long retention delays in participants with PD and compared performance to age-matched healthy control (HC) participants. Thirty-two participants with PD completed the motor section of the Movement Disorder Society Unified Parkinson's Disease Rating Sca

A systematic review of whether the number of linguistic errors in medical interpretation is associated with the use of professional vs ad hoc interpreters.

Medical consultations depend on a shared linguistic understanding between the patient and physician. When language concordance is not possible, interpretation is required. Prior studies have revealed that professional in-person interpretation (PIPI) results in patients reporting higher satisfaction and a better understanding of things the physician explained. Despite this, language-discordance often results in using family and/or friends for ad hoc interpretation. This systematic review examines the linguistic aspect of medical interpretation by assessing the number of linguistic errors made a

Prevalence and Risk Factors of Medication Errors in Enteral Tube Feeding Among ICU Patients: A Comprehensive Cross-Sectional Analysis.

OBJECTIVE: Most of the patients who are admitted to the intensive care unit (ICU) are forced to feed and use nutrition and medicine through an implanted tube. When administering medication through enteral feeding tubes, it is essential to be cautious, as some drugs may not be suitable due to interactions with feeding formulas or adverse effects when crushed. Some errors during drug gavage can lead to feeding tube blockage, reduced drug effectiveness, or drug toxicity. This study aimed to assess medication errors (MEs) in ICU patients using enteral feeding tubes and identify factors that affect

unDerstandIng the cauSes of mediCation errOrs and adVerse drug evEnts for patients with mental illness in community caRe (DISCOVER): a qualitative study.

BACKGROUND: It is estimated that 237 million medication errors occur in England each year with a significant number occurring in the community. Our understanding of the causes of preventable medication errors and adverse drug events (ADE) affecting patients with mental illness is limited in this setting. Better understanding of the factors that contribute to errors can support the development of theory-driven improvement interventions. METHODS: Remote qualitative semi-structured interviews with 26 community-based healthcare professionals in England and Wales were undertaken between June-Novem

Ipratropium Bromide/Salbutamol-Induced Acute Urinary Retention as a Result of Medication Error: A Case Report and Review of Cases in the Literature.

Medication errors have the potential to cause serious toxicity and hospitalization. This case report describes a 25-year woman who suffered serious side effects and was ‎hospitalized after receiving intravenous ipratropium bromide/salbutamol. This was due to a medication error in its preparation and administration. The caregiver diluted an intravenous antibiotic with the incorrect diluent (nebulizer solution), which led to serious toxicity, including acute urine retention and sinus tachycardia, and then resulted in patient hospitalization. A literature review of case reports was conducted to c

A study of medication errors during the prescription stage in the pediatric critical care services of a secondary-tertiary level public hospital.

BACKGROUND: Medication Errors (MEs) are considered the most common type of error in pediatric critical care services. Moreover, the ME rate in pediatric patients is up to three times higher than the rate for adults. Nevertheless, information in pediatric population is still limited, particularly in emergency/critical care practice. The purpose of this study was to describe and analyze MEs in the pediatric critical care services during the prescription stage in a Mexican secondary-tertiary level public hospital. METHODS: A cross-sectional study to detect MEs was performed in all pediatric crit
