Accuracy of family history of cancer: clinical genetic implications.

Family medical history is the cornerstone of clinical genetic diagnosis and management in cases of familial cancer. The soundness of medical decisions can be compromised if reports by the family on affected relatives are inaccurate. Although very time consuming, family medical histories are therefore routinely verified. To investigate whether such verification is clinically justified, we retrospectively analysed the accuracy of a consecutive series of 383 tumour reports from counsellees on 120 families in our clinic. We evaluated these families for the impact of verification on clinical geneti

Interpretable Machine Learning Prediction of Drug-Induced QT Prolongation: Electronic Health Record Analysis.

BACKGROUND: Drug-induced long-QT syndrome (diLQTS) is a major concern among patients who are hospitalized, for whom prediction models capable of identifying individualized risk could be useful to guide monitoring. We have previously demonstrated the feasibility of machine learning to predict the risk of diLQTS, in which deep learning models provided superior accuracy for risk prediction, although these models were limited by a lack of interpretability. OBJECTIVE: In this investigation, we sought to examine the potential trade-off between interpretability and predictive accuracy with the use o

Generative Artificial Intelligence in Prostate Cancer Imaging.

Prostate cancer (PCa) is the second most common cancer in men and has a significant health and social burden, necessitating advances in early detection, prognosis, and treatment strategies. Improvement in medical imaging has significantly impacted early PCa detection, characterization, and treatment planning. However, with an increasing number of patients with PCa and comparatively fewer PCa imaging experts, interpreting large numbers of imaging data is burdensome, time-consuming, and prone to variability among experts. With the revolutionary advances of artificial intelligence (AI) in medical

Measuring the effectiveness of a radiology-based lecture series on both confidence and career prospects of UK-based undergraduate medical students: a survey-based quantitative study.

AIMS: Studies investigating Radiology Teaching within the undergraduate population suggest that medical students do not feel adequately trained in radiological scan interpretation. This study aims to address this shortfall by designing and evaluating a longitudinal teaching series based upon the nationally recognised RCR Curriculum. By doing this, a framework can be provided for other centres to reproduce and develop their own teaching programme. MATERIALS AND METHODS: Participants were pre-clinical and clinical medical students across the United Kingdom. The course consisted of 2 sessions pe

Consequences of the lack of clinical forensic medicine in emergency departments.

Most victims of physical violence sooner or later will access a hospital or medical cabinet because of that violence, and in particular emergency departments (EDs). This paper aims to analyze the performance of emergency ward clinicians in the forensic management of such victims by examining the activities carried out and the data reported. A total of 991 medical records were extrapolated from the database of the ED of the Policlinico of Milan in an average pre-pandemic 1-year activity. For each medical record, 16 parameters were analyzed in-depth including epidemiological data, information on

Pocapavir for neonatal enteroviral sepsis: limitations of rapid viral sequencing on treatment decisions.

BACKGROUND: Neonatal enterovirus infection can be severe and life-threatening, and there is ongoing interest in the development of antiviral compounds; however, their clinical efficacy has not been definitively proven. CASE SUMMARY: We describe a case of neonatal enterovirus infection treated with the investigational therapeutic pocapavir, a viral capsid inhibitor. We describe the medical decision-making regarding the choice to offer pocapavir therapy. Because of clinical uncertainty about whether pocapavir contributed to clinical improvement, we performed sequencing of the viral genome and m

Klotho-FGF23, Cardiovascular Disease, and Vascular Calcification: Black or White?

BACKGROUND: Patients affected by Chronic Kidney Disease and Mineral Bone Disorder (CKD-MBD) have a high risk of cardiovascular (CV) mortality that is poorly explained by traditional risk factors. The newest medical treatments for CKD-MBD have been associated with encouraging, but still inconsistent, improvement in CV disease complications and patient survival. A better understanding of the biomarkers and mechanisms of left ventricular hypertrophy (LVH), atherosclerosis, and vascular calcification (VC) may help with diagnosis and treatment of the organ damage that occurs secondary to CKD-MBD, t

Incidental finding - the discovery of a bronchopulmonary foregut malformation through investigations for Crohn's disease.

Pulmonary sequestration (PMS) is a rare bronchopulmonary malformation. It has an incidence of between 0.15% to 1.7%. Likewise, cystic adenomatoid malformation (CCAM) is another relatively rare category of a bronchopulmonary malformation with a reported incidence of between 1 in 25,000 to 1 in 35,000. Moreover, a bronchopulmonary malformation with features allied to both of these forms is considered an even rarer entity. In general, bronchopulmonary malformations present with a range of non-specific symptoms. Radiological features can be non-specific yet distinctive when related to clinical fea

Multimodal Deep Learning Model Based on Ultrasound and Cytological Images Predicts Risk Stratification of cN0 Papillary Thyroid Carcinoma.

BACKGROUND: Accurately assessing the risk stratification of cN0 papillary thyroid carcinoma (PTC) preoperatively aids in making treatment decisions. We integrated preoperative ultrasound and cytological images of patients to develop and validate a multimodal deep learning (DL) model for non-invasive assessment of N0 PTC risk stratification before surgery. METHODS: In this retrospective multicenter group study, we developed a comprehensive DL model based on ultrasound and cytological images. The model was trained and validated on 890 PTC patients undergoing thyroidectomy and lymph node dissect

AI-based automated breast cancer segmentation in ultrasound imaging based on Attention Gated Multi ResU-Net.

Breast cancer is a leading cause of death among women worldwide, making early detection and diagnosis critical for effective treatment and improved patient outcomes. Ultrasound imaging is a common diagnostic tool for breast cancer, but interpreting ultrasound images can be challenging due to the complexity of breast tissue and the variability of image quality. This study proposed an Attention Gated Multi ResU-Net model for medical image segmentation tasks, that has shown promising results for breast cancer ultrasound image segmentation. The model's multi-scale feature extraction and attention-

Federated deep learning for detecting COVID-19 lung abnormalities in CT: a privacy-preserving multinational validation study.

Data privacy mechanisms are essential for rapidly scaling medical training databases to capture the heterogeneity of patient data distributions toward robust and generalizable machine learning systems. In the current COVID-19 pandemic, a major focus of artificial intelligence (AI) is interpreting chest CT, which can be readily used in the assessment and management of the disease. This paper demonstrates the feasibility of a federated learning method for detecting COVID-19 related CT abnormalities with external validation on patients from a multinational study. We recruited 132 patients from se

Data-Driven Study on Why Wrist Pulse Can Assess Health Conditions.

Pulse diagnosis (PD) is a traditional diagnostic technique in which physicians palpate the wrist pulse at the radial artery to evaluate an individual's health status. However, the scientific validity of PD has been questioned due to the lack of quantitative and objective evidence. In particular, variations in wrist pulse waves across different health conditions remain underexplored, and the correlations between pulse wave characteristics and physiological states are not yet well understood. To fill these gaps, we design a data-driven analysis architecture that integrates a quantitative analysi

[A case study of cognitive behavioral therapy for illness anxiety and depression caused COVID-19 in a breast cancer].

Cancer and the various medical treatments and tests are a major physical and psychological challenge for patients, their relatives and for the medical staff as well. Adherence and compliance become critical factors during prolonged oncological therapies. The mental health of people with malignant cancer, untreated psychological symptoms can affect survival by increasing distress and suffering, weakening quality of life and reducing adherence. Furthermore, they have a particularly high comorbidity with psychiatric disorders. The aim of this study is to introduce the tools of cognitive behaviour

Confrontation and politeness strategies in physician-patient interactions.

The therapeutic success of physician-patient interactions depends in large part on how physicians interpret and respond to patients' implicit and explicit messages. Using a hypothetical vignette, in which a patient refuses to comply with a recommended therapeutic regimen, we found that first-year medical students with no classroom training in medical interviewing implicitly recognized that the situation called for face preserving or polite linguistic behavior. Ninety percent of them used culturally sanctioned politeness forms to repair the conversational breakdown depicted in the vignette. The

The Role of Interpreters in Supporting Resilience and Implications for Artificial Intelligence.

BACKGROUND: Interpreters help millions of patients with a non-English language preference (NELP) navigate the healthcare system. Artificial intelligence (AI) tools are rapidly improving and undergoing testing and implementation for interpretation purposes. However, little is understood about interpreters' roles beyond direct interpretation, such as how they may promote resilience, which supports patient safety. Understanding these additional roles is critical as AI tools expand in interpretation. OBJECTIVE: To understand how medical interpreters enhance resilience in the care of patients with
