Understanding Problems in Communication and Cognition Experienced by Uninsured Individuals Following Brain and Spinal Cord Injuries.

PURPOSE: Uninsured individuals who experience brain injury and spinal cord injury (SCI) may not have access to health care services following their injury. We describe the demographic characteristics and reported problems in communication and cognition of individuals attending the Georgia Rehabilitation Services Volunteer Partnership (RSVP) Clinic, a free clinic that provides rehabilitative care to uninsured and underinsured individuals who experience these injuries. METHOD: Descriptive statistics examined at admission include injury type, age, race/ethnicity, highest level of education compl

Reducing the Emergency Department Revolving Door Syndrome for the Poor, Uninsured, and Chronically Ill Patient in Los Angeles: Process Improvement Recommendations from a County Health Program Evaluation.

My Health LA (MHLA), a Los Angeles countywide health program, connects eligible uninsured poor residents with partner primary care clinics and a range of free health care services. Nevertheless, primary care nonengagement and inadequate emergency department (ED) visits remained significant in this population in fiscal year 2015-2016. This evaluation examined the structure, process and outcomes of MHLA at select participating agencies to delineate pilot improvement models. Five sites were selected for the program evaluation, including a large urban ED, its affiliated urgent care clinic, and 3 p

Vaccination coverage among U.S. children aged 19-35 months entitled by the Vaccines for Children program, 2009.

OBJECTIVES: Following the measles outbreaks of the late 1980s and early 1990s, vaccination coverage was found to be low nationally, and there were pockets of underimmunized children primarily in inner cities. We described the percentage and demographics of children who were entitled to the Vaccines for Children (VFC) program in 2009 and evaluated whether Healthy People 2010 (HP 2010) vaccination coverage objectives of 90% were achieved among these children. METHODS: We analyzed data from 16,967 children aged 19-35 months sampled by the National Immunization Survey in 2009. VFC-entitled childr

Pilot of interactive texting for diabetes self-management education and support.

BACKGROUND: Controlling type 2 diabetes requires medication adherence, monitoring blood sugar, weight management, and dietary and physical activity behaviors. Estimates link a 1% reduction in hemoglobin A1c (HbA1c) to up to a 40% decrease in complications. Accessible supports for self-management behaviors can help increase adherence and lower HbA1c. However, uninsured and low-income populations may experience unique self-management challenges and have limited access to self-management supports. OBJECTIVES: To assess the feasibility, acceptability, and preliminary effectiveness of a pharmacist

Determinants of delay and association with outcome in head and neck cancer: A systematic review.

INTRODUCTION: Head and neck cancers (HNC) are relatively fast-growing tumours, and delay in treatment initiation is associated with tumour progression and adverse outcome. An overview of factors contributing to delay can provide critical insights on necessary adjustments to optimize care pathways. This systematic review aims to identify factors associated with delay and summarize the effect of delay on oncological outcome measures. METHODS: A search strategy was conducted according to PRISMA guidelines to search electronic databases for studies assessing the carepathway interval (days between

Addressing Binocular Diplopia in a Diabetic Patient: Efficacy of Corticosteroids.

This report outlines an unusual instance involving a 65-year-old man who has a medical history of beta thalassemia minor, diabetes mellitus complicated by retinopathy, and iron deficiency anemia. He presented with binocular diplopia, a condition that frequently presents treatment difficulties. Despite inconclusive magnetic resonance imaging (MRI) findings, the patient's symptoms improved significantly with prednisone treatment. The etiology likely includes underlying retinopathy and cranial nerve palsy associated with diabetic complications. This case highlights utilizing corticosteroids, a la

Estimates of the Percentage of US Adults With Diabetes Who Could Be Screened for Diabetic Retinopathy in Primary Care Settings.

IMPORTANCE: Prior studies found that screening for diabetic retinopathy (DR) in primary care settings using telemedicine increased screening rates among individuals with diabetes. This finding has led to interest in expanding the use of primary care-based screening for DR. OBJECTIVE: To estimate the percentages of US adults with diabetes and high-risk US adults with diabetes who have regular contact with primary care physicians and therefore could potentially receive timely screening for DR in primary care settings. DESIGN, SETTING, AND PARTICIPANTS: The empirical analyses used data from the

Vaginitis testing outcomes by documented symptom status in a student-run free clinic: a retrospective study.

BACKGROUND: Vaginitis is a common gynaecologic condition in patients assigned female at birth in the United States, most often caused by bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), or trichomoniasis vaginalis (TV). Symptoms such as discharge, irritation, and burning often overlap. Symptom-based diagnosis is frequently inaccurate, with correct identification in only about half of cases. Empiric treatment without confirmatory testing remains common and can drive inappropriate therapy and antimicrobial resistance. Uninsured populations face higher infection burdens and barriers to c

Disparities in the management of ectopic pregnancy.

BACKGROUND: Ectopic pregnancy is common among young women. Treatment can consist of either surgery with salpingectomy or salpingostomy or medical management with methotrexate. In addition to acute complications, treatment of ectopic pregnancy can result in long-term sequelae that include decreased fertility. Little is known about the patterns of care and predictors of treatment in women with ectopic pregnancy. Similarly, data on outcomes for various treatments are limited. OBJECTIVE: We examined the patterns of care and outcomes for women with ectopic pregnancy. Specifically, we examined pred

Perceptions and Experiences of Uninsured Latinx Adult Participants in a Community Health Worker-Delivered Intervention to Address Depression.

Latinx immigrants have an increased risk of depression and other negative mental health problems. Community health workers (CHWs) have a history of bridging the divide between marginalized communities and health care systems. Since 2019, we have been implementing and evaluating a ten 1-hour-sessions intervention delivered by a CHW and adapted from cognitive behavioral therapy, mindfulness exercises, and behavioral activation to treat mild to severe depressive symptoms and anxiety. There has been little research on the subjective experiences of the participants receiving mental health services

Adaptations of an online cognitive-behavioral therapy intervention for binge type eating disorders in publicly-insured and uninsured adults: a pilot study.

BACKGROUND: Publicly-insured and uninsured individuals-many of whom are marginalized because of race/ethnicity, ability status, and/or other social identities-experience barriers to accessing evidence-based interventions (EBIs) for eating disorders (EDs). Additionally, EBIs have not been developed with or for diverse populations, exacerbating poor treatment uptake. Mobile technology is well-positioned to bridge this gap and increase access to low-cost, culturally-sensitive EBIs. METHODS: This study leverages a user-centered design approach to adapt an existing coached cognitive-behavioral the

Ocular Findings and Referral Rates From a Community-Based Eye Clinic in New York City: The Kress Vision Program for Uninsured New Yorkers.

Vision loss in high-risk populations may be prevented in many cases with the early detection and management of eye diseases. The Kress Vision Program (KVP) is a free-of-charge community-based vision screening, referral, and treatment program for uninsured New Yorkers. This retrospective analysis describes the prevalence of ocular diseases among participants seen at the KVP, as well as the referral rates, treatment, and follow-up at an academic ophthalmology department. From October 2020-January 2023, 26 community-based organizations (CBOs) referred 887 participants, and 618 (69.7%) were screen

Effects of the Affordable Care Act on Young Women With Gynecologic Cancers.

OBJECTIVE: To evaluate the effects of the dependent coverage mandate of the 2010 Affordable Care Act (ACA) on insurance status, stage at diagnosis, and receipt of fertility-sparing treatment among young women with gynecologic cancer. METHODS: We used a difference-in-differences design to assess insurance status, stage at diagnosis (stage I-II vs III-IV), and receipt of fertility-spearing treatment before and after the 2010 ACA among young women aged 21-26 years vs women aged 27-35 years. We used the National Cancer Database with the 2004-2009 surveys as the pre-ACA years and the 2011-2014 sur

Rehospitalization during the first year of life by insurance status.

OBJECTIVE: To assess the association of insurance status on infant rehospitalization in a population-based setting. METHODS: In this longitudinal retrospective study, hospitalizations were tracked for 1 year after birth discharge for 203 031 infants born in hospitals during 2008 using data from the New York State Inpatient Database. Relative risk was estimated using multivariable negative binomial regression models. RESULTS: Rehospitalization occurred in 9010 infants (4.4%). Medicaid coverage and being uninsured were strong predictors of rehospitalizations after adjustment for birth weight a

Mapping Geospatial Gaps in Early Identification of Children With Autism Spectrum Disorder.

Despite the known developmental benefits of early intervention for autism spectrum disorder (ASD), diagnosis before age 5 years is often not achieved. Research suggests that lack of health insurance and living in rural areas and areas of severe provider shortages contribute significantly to these delays. The purpose of this project was to conduct a geospatial evaluation of potential gaps in early ASD diagnosis of uninsured children in Virginia. A secondary purpose was to show the use of geospatial analysis by pediatric nurse practitioners for policy advocacy. We mapped data from a statewide pr

Utilization of physician offices by adolescents in the United States.

UNLABELLED: Recent guidelines for adolescent primary care call for the specification of clinical services by three adolescent age subgroups. Yet analyses of office visits have either merged adolescence into one stage or divided it at age 15 years. OBJECTIVE: To explore the utilization of physician offices in the United States by early (11-14 years), middle (15-17 years), and late (18-21 years) adolescents. DESIGN: Secondary analysis of the 1994 National Ambulatory Medical Care Survey, focusing on visits made by the three adolescent age groups. SETTING: Nationally representative sample of 24

Leveraging Machine Learning Models to Explore Disparities in Prostate Cancer Diagnosis, Treatment, and Survival.

IntroductionProstate cancer (CaP) is the second most common cancer in men globally and a leading cause of cancer-related mortality, particularly among older men. In the United States, disparities in incidence, stage at diagnosis, and outcomes persist across racial, socioeconomic, and geographic lines. Men in medically underserved regions like Appalachia experience higher mortality due to limited access to timely screening and treatment. Given the complex interplay of clinical, behavioral, and sociodemographic factors, machine learning (ML) offers promise in identifying survival predictors that
