Out-of-Pocket Prescription Drug Savings for Medicare Beneficiaries with Asthma and COPD Under the Inflation Reduction Act.

BACKGROUND: High and rising prescription drug costs for asthma and chronic obstructive pulmonary disease (COPD) contribute to medication nonadherence and poor clinical outcomes. The recently enacted Inflation Reduction Act includes provisions that will cap out-of-pocket prescription drug spending at $2,000 per year and expand low-income subsidies. However, little is known about how these provisions will impact out-of-pocket drug spending for Medicare beneficiaries with asthma and COPD. OBJECTIVE: To estimate the impact of the Inflation Reduction Act's out-of-pocket spending cap and expansion

Identifying potential targets for prevention and treatment of amyotrophic lateral sclerosis based on a screen of medicare prescription drugs.

Background: Few well-established factors are associated with risk of amyotrophic lateral sclerosis (ALS). We comprehensively evaluate prescription drugs use in administrative health claims from U.S. Medicare beneficiaries in relation to ALS risk to generate hypotheses for further research. Methods: This is a population-based case-control study of 10,450 U.S. Medicare participants (ages 66-89 years) diagnosed with ALS, based on Medicare Parts A and B fee-for-service claims, between 1 January 2008, and 31 December 2014, and 104,500 controls (1:10 ratio) frequency-matched on age, sex, and selecti

Differences in the Measurement of Comorbidities Based on ICD-10-CM Coding Definitions Using Medicare Advantage Encounter Data and Fee-For-Service Claims.

BACKGROUND: Medicare Advantage (MA) encounter data became available for research in 2019; data quality concerns remain. OBJECTIVES: We evaluated the consistency of ICD-10-CM comorbidity coding between MA and Fee-For-Service (FFS) data. METHODS: We used round 7 (2017) of the National Health and Aging Trends Study (NHATS) linked to Medicare enrollment, MA encounter, and FFS claims (2016-2017). We included participants continuously enrolled in MA or FFS for 1 year before round 7. Comorbidities were identified using ICD-10-CM codes from the Gagne combined comorbidity index. Demographic, socioeco

Cost-Related Medication Nonadherence After the Inflation Reduction Act.

IMPORTANCE: High prescription drug costs are a pressing national concern and contribute to medication nonadherence and poor health outcomes. The Inflation Reduction Act (IRA) introduced sweeping reforms to improve medication affordability, but their potential impact on medication adherence is unknown. OBJECTIVE: To evaluate the association of the IRA's 2024 prescription drug provisions with cost-related medication nonadherence as well as health care-related financial strain. DESIGN, SETTING, AND PARTICIPANTS: This quasi-experimental difference-in-differences analysis used data from the 2021-

End-of-Life Care for Older Adults With Blood Cancers With Medicare Advantage Versus Medicare Fee-For-Service Insurance.

PURPOSE: Adults with hematologic malignancies (HMs) often experience suboptimal end-of-life (EOL) care, with patients from minoritized racial/ethnic groups at even greater risk. It is unclear whether these disparities are partly driven by modifiable factors such as insurance. METHODS: Using the Centers for Medicare and Medicaid Services database, we compared the quality of EOL care as defined by hospice use, high-intensity health care utilization, and advance care planning between Medicare advantage (MA) and Medicare fee-for service (FFS) insurance among patients with HM 66 years and older wh

Association Between a Bundled Payment Program for Lower Extremity Joint Replacement and Patient Outcomes Among Medicare Advantage Beneficiaries.

IMPORTANCE: Much of the evidence for bundled payments has been drawn from models in the traditional Medicare program. Although private insurers are increasingly offering bundled payment programs, it is not known whether they are associated with changes in episode spending and quality. OBJECTIVE: To evaluate whether a voluntary bundled payment program offered by a national Medicare Advantage insurer was associated with changes in episode spending or quality of care for beneficiaries receiving lower extremity joint replacement (LEJR) surgery. DESIGN, SETTING, AND PARTICIPANTS: Cross-sectional

Associations Between Industry Payments to Physicians for Antiplatelet Drugs and Utilization of Cardiac Procedures and Stents.

BACKGROUND: A study has shown that industry payments to physicians for drugs are associated not only with higher drug prescriptions but also with higher non-drug costs due to additional utilization of healthcare services. However, the association between industry payments to cardiologists for antiplatelet drugs and the costs and number of percutaneous coronary interventions they perform has not been investigated. OBJECTIVE: To examine the association between industry payments to cardiologists for antiplatelet drugs and the costs and number of percutaneous coronary interventions they perform.

Treatment of toxic epidermal necrolysis by burn units: another market or another threat?

A survey of burn care facilities in the United States has provided information regarding the treatment of toxic epidermal necrolysis and related diseases in burn units. The survey suggests that a disproportionate share (12% to 15%) of the projected number of cases of toxic epidermal necrolysis that occur annually are being transferred to the 2% of United States hospitals that have burn units. Because of the potential for a complex hospital course and extended length of stay, treatment of these patients in a prospective payment system poses a significant fiscal threat to hospitals with burn uni

Rising Revision Burden in Total Wrist Arthroplasty: National Medicare Trends and Projections to 2060.

BACKGROUND: Total wrist arthroplasty (TWA) is an alternative to arthrodesis for end-stage wrist arthritis, offering preserved wrist motion and functional improvement. Historically limited by high complication and revision rates, TWA has seen renewed interest with advances in implant design and expanding indications beyond rheumatoid arthritis. The purpose of this study was to analyze national trends in TWA utilization and project future procedure volumes for both primary and revision TWA through 2060 using Medicare data. METHODS: Procedure volumes for Medicare Part B beneficiaries undergoing

Organizational Perspectives on Access to Care Among Persons With End-Stage Kidney Disease After Enrollment in Medicare Advantage.

OBJECTIVE: To examine Medicare Advantage (MA) plan, dialysis provider organization, and kidney care management company perspectives on how MA enrollment impacts access to care among persons with end-stage kidney disease (ESKD). The 21st Century Cures Act enabled persons with ESKD to enroll in MA, resulting in over 70% growth in enrollment among this group. However, the impact of MA enrollment on access to healthcare and services for people with ESKD remains unknown. STUDY SETTING AND DESIGN: This was a qualitative study that included 48 semistructured interviews with 56 participants represent

Trends in Selection Into Medicare Advantage.

Medicare Advantage (MA) enrollment more than doubled from 2013 to 2023, raising concerns about risk selection, spending, and the continued use of traditional Medicare (TM) spending as a benchmark for MA payment. This study examines trends in selection into MA from 2009 to 2020 using administrative and survey data from the Medicare Current Beneficiary Survey. For each survey year, we estimate a regression model of Part A and B spending among TM enrollees based on demographic characteristics, self-reported health status, limitations in activities of daily living, and enrollee group type (e.g., d

Outpatient chemotherapy drug costs and expensive chemotherapy drug use in 340B and Non-340B hospitals: an observational study.

BACKGROUND: The 340B Drug Pricing Program has been controversial since its inception in 1992, a major criticism being that 340B hospitals use more outpatient drugs, and more expensive drugs, because of financial incentives to "make money" through the program. The goal of this study was to determine whether characteristics of patients treated at 340B hospitals, and affiliation of hospitals with NCI-designated cancer centers, would explain higher Part B drug costs and use of more expensive chemotherapy drugs. METHODS: This is an observational study using data from SEER-Medicare and 340B entity

The effects of medication therapy management on medication utilization among Medicare beneficiaries with HIV/AIDS.

BACKGROUND: Medicare Part D plans are required to provide medication therapy management (MTM) programs to eligible beneficiaries to optimize medication utilization. MTM programs' effects on medication utilization among older persons living with HIV/AIDS (PLWHs) remain unclear. OBJECTIVE: This study examined the effects of the Medicare MTM programs on medication utilization among PLWHs. METHODS: This study analyzed 2017 Medicare databases linked to the Area Health Resources Files. Recipients and nonrecipients of the MTM services were compared on their medication utilization: adherence to anti

Trends in the Incidence and Treatment of Distal Radius Fractures in the United States in Privately Insured and Medicare Advantage Enrollees.

Background: This study aims to determine the trends in the treatment of distal radius fractures (DRFs) in patients aged 18 years and older. Methods: An administrative claims database of more than 100 million patients was used to identify patients aged 18 years and older with a DRF between 2005 and 2014. A total of 137 130 DRFs were identified in 135 128 patients. Results: The proportion and rate of fractures were more predominant in those aged 55 years and older compared with a decreasing incidence in patients younger than 55 years. Age-adjusted rates of surgical treatment have significantly i

Declining trend in osteoporosis management and screening following vertebral compression fractures - a national analysis of commercial insurance and medicare advantage beneficiaries.

BACKGROUND CONTEXT: Despite the increasing national incidence, osteoporosis and its associated comanagement, often remain an overlooked issue in the orthopedic world. Screening and associated management of osteoporosis is often only considered by providers when patients present with multiple fragility fractures. Current evidence with regard to the trends in screening and medical comanagement/antiosteoporotic therapy of osteoporotic vertebral compression fractures (VCFs) remains limited. PURPOSE: To understand trends, costs, and clinical impact associated the utilization of antiosteoporotic me
