INTRODUCTION: Bicruciate bony avulsion fracture with osseous avulsions of dual root LM in combination with posterior horn LM meniscocapsular injury causing floating meniscus in lateral tibial plateau depressed fracture is a very rare complex injury which was managed carefully with a proper work up and surgical planning to get excellent functional outcome. Few times innocuous looking tibial plateau fractures can reveal severe hidden injuries which have potentials of disastrous complications later in the life and therefore high degree of suspicion is required for accurate diagnosis.

BACKGROUND: A medial meniscus posterior root tear (MMPRT) is defined as an injury to the posterior meniscal insertion on the tibia. In MMPRT, the medial meniscus (MM) hoop function is damaged, and the MM undergoes a medial extrusion into the interior from the superior articular surface of the tibia. However, the details of MM position and movement during knee joint movement are unclear in MMPRT cases. The present study aims to evaluate MM position and movement via magnetic resonance imaging (MRI) examination of the MM posterior extrusion (MMPE) at knee flexion angles of 10° and 90°. We hypothe…

BACKGROUND: Lateral meniscus (LM) posterior root tear (PRT) is often associated with anterior cruciate ligament (ACL) injury and can result in rotational instability, joint overloading, and degenerative changes in the knee. Improved rotational stability and kinematics have been reported after LMPRT repair. However, it is unclear what repair technique can achieve the greatest reduction in LM extrusion (LME).
HYPOTHESIS: We hypothesized that transtibial pullout repair would decrease LME to a greater extent than other repair techniques.
PATIENTS AND METHODS: Seventeen patients with ACL injury a…

Background and Objectives: Meniscal pathologies are common abnormalities of the knee joint and a frequent cause of knee pain. Prompt and accurate diagnosis is essential to ensure appropriate treatment. Ultrasonography is increasingly used due to its accessibility, cost- and time-efficiency, and capacity for dynamic assessment. This study aimed to evaluate the usefulness of ultrasonography in identifying specific types of meniscal tears and to assess their frequency of occurrence. Materials and Methods: A retrospective study was conducted to assess the frequency and sonographic appearance of va

BACKGROUND: Advancements in technology and techniques have shaped meniscus repair outcomes, but long term results remain unclear. This review aims to comprehensively evaluate studies with a mean follow-up of greater than 10 years following inside-out, outside-in, all-inside, open, or pull-out repair techniques. In addition, this article will review the impact of concomitant anterior cruciate ligament (ACL) reconstruction on long-term meniscus repair outcomes and compare osteoarthritis risk of patients treated partial meniscectomy compared with meniscus repair. We hypothesize there is increased…

Meniscectomy is known to alter the mechanics, stability, and kinematics of the tibiofemoral joint, leading to early knee osteoarthritis (KOA). While several meniscal substitutions exist, such as meniscus allograft transplantation, collagen meniscus implants, and artificial substitutes, they often come with technical challenges, high costs, and risks, including allograft failure, infections, and disease transmission. Tendon autografts emerge as a promising option, offering safety, availability, biocompatibility, and a reduced risk of pathophoresis. This review delves into basic, in vivo, in vit…

Cellular senescence, traditionally associated with aging and chronic diseases, has recently been identified as a potential facilitator of tissue regeneration via a senescence-associated secretory phenotype (SASP). In rodents, the meniscus is known to regenerate spontaneously from the surrounding synovium, but the mechanism, and especially its relationship to cellular senescence, remains unclear. This study investigated the contribution of cellular senescence to spontaneous repair of the rat meniscus. We created a rat partial medial meniscectomy (pMx) model to evaluate time-course changes in re…

A discoid meniscus is a congenital abnormality that usually affects the lateral meniscus, leading to instability and increased risk of tearing. A discoid medial meniscus is an extremely rare pathology that is seldom described in literature. In this report, we present the technique of operative treatment of a symptomatic, torn discoid medial meniscus. The meniscus is saucerized to 6-8 mm of stable rim, and the inside-out technique is used as the modality of meniscal fixation. Although a discoid medial meniscus is an uncommon finding, all treating surgeons should be aware of the possibility duri…

The knee joint is one of the most important joints in the human body. The meniscus is a half-moon cartilage which locates between the medial and lateral condyle of the thighbone and the medial and lateral condyle of the shin in the knee joint, and it has multiple functions such as bearing load, cushioning and stabilising joints. It is easy to be injured because of its complex structure and functions. In this study, the physical fitness of patients with meniscus injury was tested using kinematic measurement, and the data were collected for biomechanical analysis on the parameters such as the an…

The present study reports our experience of treating four cases of symptomatic discoid medial meniscus, three of which were bilateral. We performed partial meniscectomy with a four-portal technique using a knife leaving a 6 mm peripheral margin after confirmation of magnetic resonance imaging findings. Clinical results were assessed at the end of 2-year follow-up using the Knee Injury and Osteoarthritis Outcome Score and a visual analogue scale. We obtained satisfactory clinical results without recurrence of the symptoms in all cases.

The knee is one of the most commonly injured joints in the body. Its superficial anatomy enables diagnosis of the injury through a thorough history and physical examination. Examination techniques for the knee described decades ago are still useful, as are more recently developed tests. Proper use of these techniques requires understanding of the anatomy and biomechanical principles of the knee as well as the pathophysiology of the injuries, including tears to the menisci and extensor mechanism, patellofemoral conditions, and osteochondritis dissecans. Nevertheless, the clinical validity and a

OBJECTIVE: Parameniscal cysts have a very high association with meniscal tears in all locations except the anterior horn lateral meniscus (AHLM). The common insertion of the anterior cruciate ligament (ACL) and the AHLM root may provide a pathway for disease. The purpose of our study was to determine if cysts of the ACL are the origin of cysts adjacent to the AHLM.
MATERIALS AND METHODS: Radiology reports of all magnetic resonance (MR) examinations of the knee over a 7-year period were searched for "cyst", "ganglion", and "ganglia". Two fellowship-trained musculoskeletal radiologists independ…

Knee arthroscopy to address meniscus tears is among the most common orthopedic procedures performed, and technical advances in the treatment of meniscus tears have affected the treatment options available to orthopedic surgeons. The purpose of this study was to perform a large cross-sectional analysis of orthopedic patients to investigate trends in arthroscopic meniscectomy and meniscus repair in the United States. Patients who underwent arthroscopic meniscectomy (Current Procedural Terminology codes 29881 and 29880) and arthroscopic meniscus repair (Current Procedural Terminology codes 29882 …

The incidence of traumatic meniscal tears in children is on the rise, likely because of increased sports participation and more accurate diagnostic modalities. The increased vascularity of the developing meniscus is believed to enable greater healing potential. Meniscal tears in children are often amenable to repair, and excellent clinical results have been reported. Knee size must be considered when determining the optimal method of repair. Discoid menisci represent a spectrum of morphologic abnormalities and instability of the lateral meniscus. Highly unstable variants often present with the…

Current therapies for meniscal injury seek to preserve and repair damaged tissue since loss of meniscal tissue is associated with degenerative changes in the joint, ultimately leading to osteoarthritis (OA). After a meniscal tear, the difficulty of integrating juxtaposed meniscal surfaces continues to be an obstacle. In order to determine the local factors that are necessary for successful tissue repair, previous studies have developed in vitro model systems that allow both biological and quantitative biomechanical measures of meniscus repair. Many studies have shown the importance of individu…
