Nuclear volume in type I gastric intestinal metaplasia.

OBJECTIVE: To retrospectively compare nuclear size of epithelial cells in type I intestinal metaplasia (IM) from various pathologic lesions of gastric mucosa. STUDY DESIGN: Endoscopic mucosal biopsies with type I IM from intestinal type gastric carcinoma (n = 25), chronic gastritis (n = 40) and benign ulcer (n = 32) cases were analyzed. After standard fixation, embedding, sectioning, routine hematoxylin and eosin and alcian blue--periodic acid--Schiff reaction (pH 1.0 and 2.5) staining were used for identification of IM. The mean point sampled nuclear intercept was estimated by the original t…

[Endocervical tubal metaplasia: morphological concepts and practical importance].

OBJECTIVE: Among uterine cervix tumor-like lesions, tubal metaplasia (TM) has been confused with endocervical in situ adenocarcinoma. TM is a benign lesion composed of three cellular types: ciliary, secretory and intercalary (or peg cell). Thus, the main purpose of this work is to localize and characterize tubal metaplasia and its relation to other morphological lesions in the cervix. METHODS: Eighteen cervical specimens from 8 cones and 10 hysterectomies with TM were reviewed in order to observe its relative frequency in different segments such as: superior, inferior, surface epithelium and …

Fibrous hematopoietic tumors arising in agnogenic myeloid metaplasia.

Tumors of extramedullary hematopoiesis occasionally occur in long-standing cases of agnogenic myeloid metaplasia (AMM) and resemble hypercellular marrow in a connective tissue stroma. Rarely, this type of tumor mass is markedly fibrous and may pose diagnostic difficulty, especially when there is only limited knowledge of a patient's clinical history. The authors reports two cases of AMM with such a fibrous hematopoietic tumor, occurring in one patient as a pelvic mass and in another as lymphadenopathy and skin nodules. A review of the literature revealed that fibrous hematopoietic tumors assoc…

Endoscopic diagnosis of gastric intestinal metaplasia in patients with autoimmune gastritis using narrow-band imaging: does pseudopyloric metaplasia muddy the waters?

Background and study aims  In autoimmune atrophic gastritis (AAG), associated with intestinal (IM) and/or pseudopyloric metaplasia (PPM), endoscopic surveillance is recommended for gastric cancer risk mainly linked to IM. Endoscopic Grading of Gastric Intestinal Metaplasia (EGGIM) reliably identifies IM, but has not been assessed in AAG. We aimed to assess the performance of EGGIM (index test) versus histology (reference test) of corpus IM in AAG. Patients and methods  This was a cross-sectional study of 210 AAG patients undergoing surveillance gastroscopy with narrow-band imaging (NBI): corpu…

The expression of the trefoil peptides pS2 and human spasmolytic polypeptide (hSP) in 'gastric metaplasia' of the proximal duodenum: implications for the nature of 'gastric metaplasia'.

Expression of pS2 protein (an oestrogen-induced gene discovered in the MCF-7 breast carcinoma cell line) and its homologue human spasmolytic polypeptide (hSP) was analysed, using immunohistochemistry and in situ hybridization to their mRNAs, in the proximal duodenum of 17 partial gastrectomy specimens removed from individuals with chronic peptic ulceration. Eight were found to have gastric-type metaplasia. In gastric metaplasia, mRNAs for pS2 and hSP, and pS2 peptide antibody were co-localized in the cells covering the duodenal villi. pS2 immunostaining was diffusely cytoplasmic in nature. A s…

Clinical significance of spasmolytic polypeptide-expressing metaplasia and intestinal metaplasia in Epstein-Barr virus-associated and Epstein-Barr virus-negative gastric cancer.

Spasmolytic polypeptide-expressing metaplasia (SPEM) and intestinal metaplasia (IM) have been recognized as neoplastic precursors in gastric carcinogenesis. We explored the relationship between SPEM and IM in Epstein-Barr virus-associated (EBVaGC) and Epstein-Barr virus-negative (EBVnGC) gastric cancer. Sixty-four EBVaGC and one hundred and fifty-four EBVnGC patients were included. EBV positivity was identified using Epstein-Barr virus-encoded RNA-1 in situ hybridization. SPEM was subclassified into absent, early, and advanced SPEM. Acute and chronic inflammation was graded as absent, mild, mo…

Therapeutic effect of metformin on reversing gastric intestinal metaplasia.

BACKGROUND: Currently, standard therapeutic agents are not available for treating gastric intestinal metaplasia (IM). However, emerging evidence suggests the potential efficacy of metformin in addressing this issue. Our aim was to assess the efficacy and safety of metformin in reversing IM. METHODS: We initially investigated the impact of metformin on reversing gastric IM in mice. An open-label, prospective, randomized controlled trial was subsequently conducted at a tertiary hospital in China from April 2022 to May 2023. A total of 140 nondiabetic patients with pathologically confirmed IM an…

Immunohistochemical characterization of peritoneal inclusion cysts with squamous metaplasia.

AIMS: Peritoneal inclusion cysts (PICs) are mesothelial-lined cysts that can uncommonly develop squamous metaplasia. Here, we describe the immunophenotype of PICs with squamous metaplasia using a comprehensive immunohistochemical (IHC) panel. METHODS AND RESULTS: We collected surgical excisions of PICs with squamous metaplasia from our institution and cases sent to our extramural consultation service. An IHC panel was used to characterize the immunophenotype, which included antibodies to claudin 4, WT-1, D2-40, calretinin, CK5/6, p40/p63, PAX8, BAP1, MTAP and merlin. 10 cases of PICs with squ…

Endometrial Osseous Metaplasia: A Retrospective Analysis and Systematic Review.

Endometrial osseous metaplasia is a rare condition defined as the presence of heterotopic bone in the endometrium. A retrospective, monocentric analysis was conducted, including patients with endometrial osseous metaplasia, and a systematic review of the literature was performed. The retrospective analysis included 10 cases. Most patients (n = 9) were of reproductive age and had a history of miscarriage. The main symptoms were infertility (n = 4) and menstrual cycle disorders (n = 4). The size and the uterine site of the metaplasia varied. It is crucial not to underestimate it, especially in i…

Disease-specific transcriptional programs govern airway goblet cell metaplasia.

Hypersecretion of airway mucus caused by goblet cell metaplasia is a characteristic of chronic pulmonary inflammatory diseases including asthma, cystic fibrosis (CF), and chronic obstructive pulmonary disease (COPD). Goblet cells originate from airway progenitor club cells. However, the molecular mechanisms and features of goblet cell metaplasia in lung disease are poorly understood. Herein, public single-cell RNA sequencing datasets of human lungs were reanalyzed to explore the transitional phase as club cells differentiate into goblet cells in asthma, CF, and COPD. We found that changes in c…

Gastric intestinal metaplasia and gastric cancer prevention: Watchful waiting.

Gastric intestinal metaplasia (GIM), a common histologic finding, is associated with increased risk of gastric cancer, and GIM associated with Helicobacter pylori infection is classified as an environmental metaplastic atrophic gastritis. Patients may be asymptomatic or present with various dyspeptic symptoms. Autoimmune metaplastic atrophic gastritis is a less common but important cause of chronic gastritis. The Correa cascade describes the evolution of precancerous mucosal changes that lead to development of GIM, with differentiation of 2 histologic types of GIM (complete and incomplete) and…

Soft tissue mass resembling myxochondroid metaplasia in Achilles tendon.

The Achilles tendon is the strongest and largest tendon in the human body, composed of the distal confluence of the gastrocnemius and soleus muscles and may also include the plantaris longus tendons. Despite its tremendous strength, it is frequently injured. Soft tissue tumors or tumor-like conditions of Achilles tendon are much less common in comparison to traumatic and infectious/inflammatory lesions. These include giant cell tumor of the tendon sheath, pigmented villonodular synovitis, xanthomas, hematomas, and tophaceous gout. We present a case of a myxochondroid mass arising from the Achi

Dynamic tuft cell expansion during gastric metaplasia and dysplasia.

Tuft cells are chemosensory cells associated with luminal homeostasis, immune response, and tumorigenesis in the gastrointestinal tract. We aimed to elucidate alterations in tuft cell populations during gastric atrophy and tumorigenesis in humans with correlative comparison to relevant mouse models. Tuft cell distribution was determined in human stomachs from organ donors and in gastric pathologies including Ménétrier's disease, Helicobacter pylori gastritis, intestinal metaplasia (IM), and gastric tumors. Tuft cell populations were examined in Lrig1-KrasG12D , Mist1-KrasG12D , and MT-TGFα mic…
