A case of invasive partial hydatidiform mole requiring chemotherapy and hysterectomy in a 30-year-old white woman is presented. This is the first histologically and cytogenetically documented partial mole with persistent elevation of human chorionic gonadotropin (hCG) level and invasion of myometrium. There was no evidence of distant spread.

One hundred and six patients with complete mole and 35 patients with partial mole were analysed. The clinical behaviour of complete mole was more florid. The clinical diagnosis of partial mole was more difficult. 20.7% of the complete mole patients and 11.4% of the partial mole patients required chemotherapy because of persistent or metastatic trophoblastic disease. The response to chemotherapy was more favourable in the partial moles. All patients with either partial or complete mole needed hCG assay monitor because of the risk of malignant sequelae.

Ultrastructural analysis of the surface of the syncytial absorptive epithelium of the aberrant form of human placenta known as hydatidiform mole reveals modifications of the microvillous apical surface. We have described these features as microgibbosities. They are apparently groups or rings of microvilli greater than the usual length in the surrounding cytoplasm. The existence of such microvilli and the probable consequences for the control of the dynamics of microvillar cytoskeletal elements should be accommodated by future theories of microvillar biogenesis.

A five year retrospective analysis of 198 cases of hydatidiform mole recorded in three maternity hospitals in Singapore is presented. The incidence was 1 in 868 deliveries. The number of molar pregnancies in the first half of the year was higher than in the subsequent six months. The incidence of hydatidiform mole and its correlation with possible environmental factors is discussed.

In 14 women with hydatidiform mole, 9 were hyperthyroid. Serum thyroxine (T4) levels varied between 18 and 34 mug/100 ml, and serum triiodothyronine (T3) levels between 300 and 800 ng/100 ml in the hyperthyroid patients. Bioassayable thyroid-stimulating hormone (molar TSH) was found in high concentrations in the serum of 13 patients in whom preoperative serum was available.
