The human antibody response to the surface of Mycobacterium tuberculosis.

BACKGROUND: Vaccine-induced human antibodies to surface components of Haemophilus influenzae and Streptococcus pneumonia are correlated with protection. Monoclonal antibodies to surface components of Mycobacterium tuberculosis are also protective in animal models. We have characterized human antibodies that bind to the surface of live M. tuberculosis. METHODS: Plasma from humans with latent tuberculosis (TB) infection (n = 23), active TB disease (n = 40), and uninfected controls (n = 9) were assayed by ELISA for reactivity to the live M. tuberculosis surface and to inactivated M. tuberculosis…

Fluoroquinolone resistance in renal isolates of Mycobacterium tuberculosis.

SETTING: Alberta, Canada, 1990-2003. OBJECTIVE: Monotherapy of active tuberculosis (TB) promotes drug resistance. Given the common practice of empiric fluoroquinolone (FQ) therapy for urinary tract infections (UTI) and frequent delayed diagnosis of renal TB, we assessed urine Mycobacterium tuberculosis isolates for FQ resistance. DESIGN: Retrospective study. Urine M. tuberculosis isolates underwent FQ susceptibility testing. Records were reviewed for evidence of FQ exposure and diagnostic delay. RESULTS: Among 78 culture-positive renal TB patients between 1990 and 2003, initial isolates of …

Novel agents in the management of Mycobacterium tuberculosis disease.

The goals of tuberculosis control are to cure active disease, prevent relapse, reduce transmission and avert the emergence of drug resistance. However, since the 1960s, there have been few developments in available therapies. Currently available agents are complicated by numerous side-effects, drug interactions and the need for a long duration of therapy. Rifampicin-containing regimes lead to hepatic enzyme induction which can complicate or preclude the use of protease inhibitors and non-nucleoside reverse transcriptase inhibitors in patients infected with the human immunodeficiency virus. [Can optic perineuritis be triggered by Mycobacterium tuberculosis?].

A 38-year-old woman presented with a swollen left optic disc while being treated for miliary tuberculosis. The diagnosis of optic perineuritis was made, antibiotics were adapted, and the patient slowly recovered. We discuss the elements required to confirm optic perineuritis and the differential diagnosis in cases of papilledema with no visual impairment, as well as the hypotheses on the possible etiologies: dysimmune neuropathy as opposed to infection or toxicity.

Nosocomial transmission of Mycobacterium tuberculosis in a children's hospital.

An infant was admitted for evaluation of respiratory distress and tracheomalacia. After several weeks of hospitalization, both the infant and her mother were diagnosed with culture-positive pulmonary tuberculosis (TB). Sixteen pediatric patients and 293 health care workers were evaluated for exposure to Mycobacterium tuberculosis. Tuberculin skin test conversions occurred in 6.7% of patients and 1.9% of pediatric health care workers. While nosocomial transmission of TB is relatively rare in children's hospitals, this report highlights the differences between TB control efforts in pediatric vs.…

Transmission of Mycobacterium tuberculosis from human to cattle.

We describe the first transmission of Mycobacterium tuberculosis from human to cattle confirmed by molecular typing of isolates involved in the transmission. IS6110-based restriction fragment length polymorphism analysis showed that the isolates from the cattle and farm worker who suffered from pulmonary tuberculosis 1 year prior to this case were the same strains.

[Exposure to Mycobacterium tuberculosis during a bus travel].

BACKGROUND: In an outbreak of tuberculosis that occurred in a school, we evaluated the possible transmission during a trip in a school bus as the cause of infection. SUBJECTS, PATIENTS AND METHODS: Contact tracing of the driver, 3 teachers and 49 students from different grades who were travelling from Málaga to Sierra Nevada (Spain) in March 1998. All the students from the ten grades where any case of tuberculosis appeared from April to December 1998 and all the teachers of the school were included. RESULTS: At the school, 232 students were studied. The prevalence of the infection was 35%. A

[The antilysozyme activity of Mycobacterium tuberculosis L forms].

The method for the detection of antilysozyme activity (ALA) in M. tuberculosis L forms was developed. The level of ALA in M. tuberculosis L forms isolated from patients with different clinical forms of the disease varied within 1-5 micrograms. M. tuberculosis L forms with the ALA level > 4 micrograms were isolated from patients with the progressing course of the disease. The method for the prognostication of the course of the tuberculous process in the lungs by the results of the antilysozyme test was proposed.

Correlates of protective immunity to Mycobacterium tuberculosis in humans.

Correlates of protective immunity to Mycobacterium tuberculosis in humans are desirable for identifying protective antigens, demonstrating the immunogenicity of a vaccine candidate and its potential efficacy, and permitting optimization of the dose, vehicle, adjuvant, and schedule of immunization. Potential correlates can be proposed on the basis of animal models and ex vivo/in vitro studies in humans. Most critical is their validation; ultimate validation will require correlation with protection in a phase III efficacy trial of an effective vaccine. Other approaches, however, can allow select…

Extensive transmission of Mycobacterium tuberculosis from a child.

BACKGROUND AND METHODS: Young children rarely transmit tuberculosis. In July 1998, infectious tuberculosis was identified in a nine-year-old … microscopical examination. CONCLUSIONS: This investigation showed that a young child can transmit Mycobacterium tuberculosis to a large number of contacts. Children with tuberculosis, especially cavitary or laryngeal tuberculosis, should be considered potentially infectious, and screening of their contacts for infection with M. tuberculosis or active tuberculosis may be r …

Mycobacterium tuberculosis miniepidemic in a church gospel choir.

BACKGROUND: Conventional contact investigation and molecular fingerprinting of Mycobacterium tuberculosis isolates in tuberculosis (TB) outbreaks have facilitated recognition as well as application of public health control activities. Singing in a choir as an activity that promotes TB transmission has been occasionally recognized. Such avocational transmission in a middle class community can occur with attendant difficulties encountered in contact investigation. METHODS: Five cases of TB (one index case; three secondary cases; one unassociated case) were identified among members of a famous c…

Immunotherapy of tuberculosis with Mycobacterium vaccae NCTC 11659.

The history of immunotherapy for tuberculosis is briefly reviewed, and the early appreciation of the importance of secreted antigens, common mycobacterial antigens and stress proteins is noted. The methods by which Mycobacterium vaccae strain NCTC 11659 was selected for special attention, and results of some of the pilot studies of its use as an immunotherapeutic for tuberculosis are reviewed. The results suggested that immunotherapy with M. vaccae may be an important step forward in the treatment and eventual control of tuberculosis. Used in combination with modern short course chemotherapy,

The role of Mycobacterium tuberculosis in experimental allergic encephalomyelitis.

Various detergents used in preparative membrane protein chemistry were added to a complete Freund's adjuvant/water emulsion in order to increase the solubility and/or immunologic availability of the Mycobacterium tuberculosis membrane and to explain its role in blood-brain barrier (BBB) permeability. Magnetic resonance imaging was used for in vivo determination of the BBB breakdown and cerebral edema. The results showed that with 1% 10 tridecyl ether, which increases emulsion stability, abundant BBB breakdown and cerebral edema were observed, similar to those encountered in experimental allerg…

[Sepsis caused by Mycobacterium tuberculosis in patients with AIDS].

BACKGROUND: Techniques for detecting mycobacteria in blood can be a good alternative for diagnosing tuberculosis in acquired-immunodeficiency syndrome patients (AIDS). PATIENTS AND METHODS: Eleven patients with AIDS and pulmonary tuberculosis, and four controls with AIDS and without tuberculosis were studied. The diagnosis of mycobacteremia was made by the identification of the microorganisms in blood. The method used was a lysis-centrifugation with subsequent stain or culture identification in different media (agar Löwenstein-Jensen, M7H9, M7H11, brain-heart infusion and Sauton broth). RESU
