Blood pressure regulation in autonomic failure by dietary sodium, blood volume and posture.

In normal subjects natriuresis is tightly coupled to dietary salt ingestion to maintain sodium balance. Likewise, blood pressure remains unchanged over a wide range of salt intake because of pressure natriuresis, whereby an increase in blood pressure stimulates renal sodium excretion to restore homeostasis. These sodium handling mechanisms are impaired in autonomic failure. When exposed to salt restriction, autonomic failure patients are unable to reduce renal sodium excretion, and their orthostatic hypotension worsens. It follows that increased dietary salt would improve orthostatic tolerance…

Hyponatremia in Traumatic Brain Injury: A Practical Management Protocol.

BACKGROUND: Hyponatremia (defined as serum sodium <135 mEq/L) is the most common electrolyte abnormality in traumatic brain injury (TBI) and is also an independent predictor of poor neurologic outcome. The reported incidence of hyponatremia varies widely in literature reports, and there is continuing difficulty in clearly differentiating between the 2 common causes of hyponatremia with natriuresis: the syndrome of inappropriate antidiuretic hormone secretion (SIADH) and cerebral salt wasting (CSW). We encounter hyponatremia frequently in our practice, and we therefore decided to review data fr…

Low-dose atrial natriuretic factor and furosemide in experimental acute congestive heart failure.

This study was designed to address three objectives in an experimental model of acute congestive heart failure (CHF) in the dog produced by rapid ventricular pacing. The first objective was to characterize cardiorenal and humoral responses before and during 2 h of acute CHF. The second objective was to determine the modulating action of iv furosemide upon these biologic responses to acute CHF, testing the hypothesis that furosemide-mediated natriuresis is associated with activation of the renin-angiotensin-aldosterone system (RAAS) compared with the control group. The third objective was to de…

Hematocrit modulates response of ANP to volume expansion in immature rats.

Compared with the adult, the immature kidney responds to acute volume expansion (VE) with reduced diuresis and natriuresis. The underlying mechanisms are poorly understood. Diuresis and natriuresis are blunted despite a lower hematocrit (Hct) in the immature rat, which should enhance the response to VE. The present study was designed to evaluate the role of atrial natriuretic peptide (ANP), its second messenger, guanosine 3',5'-cyclic monophosphate (cGMP), and Hct in the regulation of the renal response to acute saline VE (3% body wt) during postnatal development in rats. Anesthetized Sprague-…

Diuretic effect of co-administration of furosemide and albumin in comparison to furosemide therapy alone: An updated systematic review and meta-analysis.

BACKGROUND: It has been a matter of much debate whether the co-administration of furosemide and albumin can achieve better diuresis and natriuresis than furosemide treatment alone. There is inconsistency in published trials regarding the effect of this combination therapy. We, therefore, conducted this meta-analysis to explore the efficacy of furosemide and albumin co-administration and the factors potentially influencing the diuretic effect of such co-administration. METHODS: In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we sea…

[Renovascular hypertension due to unilateral renal artery stenosis with hypokalemic alkalosis, the salt-losing syndrome and reversible hyperechogenicity of the contralateral kidney. A study of 2 infants].

This report describes two infants with severe arterial hypertension secondary to unilateral renal artery stenosis which was manifested by polyuria, polydipsia, hypokalemic alkalosis, hyponatremia, increased natriuresis and increased plasma values of rennin and aldosterone. On sonographic examination, the contralateral non-stenotic kidney of both patients appeared enlarged and hyperechogenic mimicking parenchymal lesion. When the patients became normotensive, their sodium and potassium balance became normal and their contralateral non-stenotic kidney also became normal in size and echogenicity.…

[Sulpiride (stereoisomers, racemic) and dopamine: actions and interactions on renal excretion of hydro-saline].

Renal effects of Dopamine (DA, subpressor dosage 0.1 microgram X kg -1 X min -1) during hypotonic polyuria in moderate hydro-saline retention are variously modified by either d- or l-Sulpiride isomers. In the presence of d-Sulpiride, DA effects, such as an increase in diuresis, free water clearance (CH20) and kaliuresis are suppressed, while increases of saluresis and natriuresis are significantly blunted. In the presence of l-Sulpiride no changes are observed in both saluresis and natriuresis, while decreases occur in diuresis, CH20 and kaliuresis. The inhibitory DA effects on isosmotic sodiu…

Interaction between cholinergic and osmolar stimulation of the lateral hypothalamic area (LHA) on sodium and potassium excretion.

Hypotonic and isotonic solutions into the LHA of unrestrained rats caused no alteration in renal water and electrolyte excretion. Similar results were obtained after injecting hypo, iso and hypertonic glucose solutions. On the other hand, a hypertonic NaCl solution produced an increase in natriuresis and kaliuresis immediately after being injected into the LHA. The carbachol elicited an increase in natriuresis and kaliuresis as well as a decrease in urine output in the first urine samples collected after the injection being injected into the LHA. This response was not modified when the same do…

Renal sodium avidity in heart failure: from pathophysiology to treatment strategies.

Increased neurohumoral stimulation resulting in excessive sodium avidity and extracellular volume overload are hallmark features of decompensated heart failure. Especially in case of concomitant renal dysfunction, the kidneys often fail to elicit effective natriuresis. While assessment of renal function is generally performed by measuring serum creatinine-a surrogate for glomerular filtration-, this only represents part of the nephron's function. Alterations in tubular sodium handling are at least equally important in the development of volume overload and congestion. Venous congestion and neu…

Effects of adrenalectomy and aldosterone on the action of centrally administered rat atrial natriuretic peptide-(99-126).

Intracerebroventricular (i.v.t.) administration of rat atrial natriuretic peptide-(99-126) (rANP) increases urinary volume and sodium excret … reated with aldosterone and given an oral water load followed by i.v.t. rANP or saline. In ADX rats natriuresis and diuresis after rANP were still evident. Our results indicate that the natriuretic effect of i.v.t. rANP is unrelated to plasma levels of mineralocorticoids. Likewise, diuresis and natriuresis can occur in the absence of the adrenal glands.

Effectiveness of high-dose spironolactone therapy in patients with chronic liver disease and relatively refractory ascites.

Ten patients with ascites due to chronic liver disease were treated with spironolactone as the sole diuretic in doses of 300 to 600 mg daily. Prior to spironolactone treatment ascites was relatively refractory as indicated by baseline 24-hr urine sodium values of less than 5 mEq and natriuresis of less than 30 mEq following 200 mg hydrochlorothiazide. With spironolactone, mean daily weight loss was 540 g and natriuresis 74 mEq; diuresis was considered satisfactory in nine of ten patients. Untoward effects were limited to a single episode of hepatic encephalopathy, a small fall in serum magnesi…

Intrarenal distribution of blood flow and cortico-medullary sodium gradient after unilateral splanchnicotomy in the dog.

1. Intrarenal distribution of blood flow and cortico-medullary sodium gradients have been studied in anaesthetized dogs, previously subjected to unilateral splanchnicotomy. Experiments were carried out on hydropoenic, normal, isotonic and hypotonic volume expanded animals.2. Polyuria and natriuresis were present under all experimental conditions after denervation.3. Denervation resulted in an increase in medullary blood flow, particularly in hydropoenic and normal dogs. In isotonic and hypotonic volume expansion, the increases in the medullary circulation were smaller despite a greater increas…

Spot urinary sodium in acute decompensation of advanced heart failure and dilutional hyponatremia: insights from DRAIN trial.

BACKGROUND: Diuretic resistance portends a poor prognosis in acute heart failure, especially in advanced stages. Early identification of a poor response to diuretics may help to improve treatment and outcomes. Spot natriuresis (UNa+) at 2 h from the start of intravenous furosemide has been proposed as an early indicator of diuretic response. Our paper aimed to determine the role of early natriuresis in patients hospitalized with advanced chronic heart failure (ACHF) and high risk of diuretic resistance. METHODS AND RESULTS: We performed a sub-analysis of the DRAIN trial, a randomized clinical…

Renal Medulla in Hypertension.

Studies have found that blood flow to the renal medulla is an important determinant of pressure-natriuresis and the long-term regulation of arterial pressure. First, a brief review of methods developed enabling the study of the medullary circulation is presented. Second, studies performed in rats are presented showing medullary blood flow plays a vital role in the pressure-natriuresis relationship and thereby in hypertension. Third, it is shown that chronic reduction of medullary blood flow results in hypertension and that enhancement of medullary blood flow reduces hypertension hereditary mod…
