[Avascular necrosis of the femoral head].

Avascular necrosis of the femoral head (AVN) describes an ischemia-induced necrosis of the subchondral bone of the femoral head. Early diagnosis is crucial to initiate appropriate treatment and achieve the longest possible preservation of the native joint. Pathophysiologically, AVN results from impaired blood supply to the subchondral bone, promoted by various risk factors or underlying pathologies. The most relevant include prolonged or high-dose corticosteroid therapy and chronic alcohol abuse. Magnetic resonance imaging (MRI) is regarded as the key modality for early detection, enabling ide…

The Many Faces of Marrow Necrosis.

Depending on the age and location within the skeleton, bone marrow can be mostly fatty or hematopoietic, and both types can be affected by marrow necrosis. This review article highlights the magnetic resonance imaging findings of disorders in which marrow necrosis is the dominant feature.Fatty marrow necrosis is detected on T1-weighted images that show an early and specific finding: the reactive interface. Collapse is a frequent complication of epiphyseal necrosis and detected on fat-suppressed fluid-sensitive sequences or using conventional radiographs. Nonfatty marrow necrosis is less freque…

Avascular Necrosis of the Femoral Head.

About 250000 patients are operated on annually for hip arthroplasty in the USA alone. Of this number, about 10% the cause of their osteoarthritis is the avascular necrosis of the femoral head. But the causes of necrosis are multiple and many of them are still unknown. That is why small advances in etiopathogenesis, diagnosis and treatment of different causes are very important in order to reduce the number of affected patients. It may seem that small changes, studies focused on the details of different diseases do not bring great changes to science, but it is these small changes that end up ad…

[Avascular necrosis of the femoral head].

Avascular necrosis of the femoral head in adults is not common, but not too rare diseases. In orthopedic practice, it is one of the diseases that are causing implantation of hip replacement at a relatively early age. In the early detection and initiation of therapy can delay the implantation of prosthesis for several years, which is certainly more convenient for the patient and beneficial. This article is intended to acquaint the reader with the basic diagnostic procedures and therapy.

Subcutaneous fat necrosis of the newborn.

Subcutaneous fat necrosis (SCFN) is a rare fat tissue inflammation of the newborn. Risk factors include cord prolapse, perinatal asphyxia, therapeutic hypothermia, meconium aspiration, and sepsis. When present, hypercalcemia comes with lethargy, hypotonia, irritability, vomiting, polyuria, polydipsia, constipation, and dehydration. Kidney injury must be avoided. SCFN is often completely autoresolutive.

Bilateral avascular necrosis of the trapezoid.

We present a 5-year follow-up of a patient with bilateral necrosis of the trapezoid that improved clinically and radiographically with nonoperative treatment.

Posttraumatic avascular necrosis of the talus.

Avascular necrosis of the talus is one the most challenging problems encountered in posttraumatic reconstruction of the hindfoot. Since the first description of the talus injury in 1608 by Fabricius of Hilden, our knowledge of the talar anatomy, injuries, sequelae, and management has increased significantly. Adequate knowledge of the etiology, the extent of the disease, and the degree of patient symptoms are required to determine optimal treatment.

[Necrosis of epiploic appendages of the colon].

Fifty-two patients with necrosis of epiploic appendages (EA) of the colon were treated. Necrosis of EA of the appendix was diagnosed in 4 (7,7%) patients, caecum - in 7 (13,5%), ascending colon - in 5 (9,6%), descending colon - in 2 (3,8%), and sigmoid colon - in 34 (65,4%) cases. Multiple necrosis was revealed in 3 (5,8%) patients. Surgical treatment of these patients is radical and justified. It prevents progress of destruction and development of complications. Long-term results are evaluated for the first time.

[Subcutaneous fat necrosis of the newborn].

Subcutaneous fat necrosis presents as red-to-bluish, indolent, well-circumscribed, subcutaneous nodules and plaques, unusually in healthy infants. Histologically, a lobular panniculitis with a mixed inflammatory infiltrate is seen. Characteristic crystalloid structures can be found within some of the lipocytes and, most prominently, within macrophages. This rarely diagnosed skin disease is harmless and self-limited, yet the associated hypercalcaemia has a high complication rate.

Borderline necrosis of the femoral head.

To determine whether the histologic lesions classified by the system of Arlet et al as Type 2 (granular necrosis of fatty marrow) and Type 3 (complete medullary and trabecular necrosis) always progress to Type 4 (complete necrosis with marginal medullary fibrosis and appositional new bone formation), 10 femoral heads (nine patients) were monitored for 4 years using serial magnetic resonance images. These femoral heads had been diagnosed histologically as having either Type 2 (seven hips) or Type 3 (three hips) necrosis on initial core biopsies. On the initial magnetic resonance image, none of …

Avascular necrosis of the trapezoid bone.

A case of idiopathic avascular necrosis of the right trapezoid is presented. The aetiology was not clear. Treatment consisted of bone curettage, autologous bone graft and revascularization with a dorsal metacarpal artery.

Warfarin-induced necrosis of the skin.

Warfarin (Coumadin)-induced necrosis of the skin is an unusual condition associated with the administration of warfarin. The necrotic lesions appear suddenly and are extremely painful. Single or multiple lesions develop in a distinct pattern. A hypercoagulable state created early in the course of warfarin administration is most likely responsible for warfarin-associated necrosis. Functional and inherited low levels of proteins C and S are found in some patients. The differential diagnosis of warfarin necrosis is of interest to the plastic surgeon. The condition may mimic purpura fulminans, nec…

Idiopathic avascular necrosis of the scaphoid.

Idiopathic avascular necrosis of the scaphoid is a rare condition. A review of the literature shows a variety of conditions labelled as spontaneous avascular necrosis or Preiser's disease. In this paper we report on a study of eight patients with idiopathic avascular necrosis affecting only the proximal pole of the scaphoid. Seven of these patients had positive ulnar variance. The possible aetiology is discussed and the natural history has been studied. A staging system is proposed, as this helps to determine the prognosis and appropriate management. Two of our patients were managed conservati…
