OBJECTIVES: We evaluated whether early nephrological referral of patients with chronic renal failure (CRF) resulted in improved condition of patients at initiation of maintenance dialysis and in better outcome on dialysis.

Among 94 women with diagnosed "primary" gestosis during pregnancy, 67 patients demonstrated (3-6 months after delivery) chronic glomerulonephritis (25 women) or chronic pyelonephritis (28 women) or hypertension caused by others than nephrologic reasons. "Primary" gestosis was diagnosed correctly only in 29% cases. The most often reason of "secondary" gestosis was undiagnosed chronic nephropathy before and during pregnancy. Obtained results confirm other data informing that "primary" gestosis is a rare phenomenon.

A study was undertaken over a period of one year to assess the place of percutaneous renal biopsy in clinical nephrology. During this time 83 patients were biopsied. In 76 patients (92 percent) renal tissue was obtained at the first attempt. In the other seven patients renal tissue was obtained on a repeat biopsy after ultrasonic localisation of the kidney. BACKGROUND: We have noted a large number of referrals for abnormal kidney imaging and laboratory tests and postulated that such referrals have increased significantly over time. Renal osteodystrophy (brown tumors) represents a complication of chronic renal failure, especially in patients receiving hemodialysis treatment. It is determined by the secondary hyperparathyroidism. The incidence ranges between 1.5 and 13%. These lesions occur most often in the pelvis and ribs; both of them are present in our patient. Thoracic involvement brings the patient to the specialist in lung diseases. The differential diagnosis is instructive.
