Lichen Simplex Chronicus (LSC) is chronic dermatitis caused by repetitive scratching or rubbing of the skin. It presents as hyperpigmentation and thickening of the skin with variable scaling. Because LSC is a secondary lesion with a wide variety of causes, optimal management is contingent on identifying and managing its exact aetiology. We report an intriguing case of LSC in an elderly patient with longstanding scabies and sensory neuropathy.

In a busy dermatologic practice, it may be frustrating to encounter a vulvar disease patient, who must fully disrobe for examination. Further, the wet mounts, cultures, biopsies, and patient education required for treatment of vulvar disorders take up considerable time. However, most patients will respond to appropriate therapy, and can be very gratifying to treat.

Cutaneous tuberculosis is not an uncommon entity but it often remains undiagnosed and is inappropriately treated due to its varied presentation. We present a 42-year-old male with lupus vulgaris in the perianal area, who was treated as lichen simplex chronicus with steroids for two years. With four drug antitubercular treatment, the patient had a dramatic response. The case is being reported because in this HIV era with the resurgence of tuberculosis, not only will there be an increase in incidence but also more involvement of atypical sites.
