A case is reported of an 84-year-old white man in whom a palisaded encapsulated neuroma (PEN) had developed in his glans penis over the course of 50 years. It represents a distinctive cutaneous nerve sheath tumor that consists of an overgrowth of both axons and Schwann cells and is surrounded by a complete or incomplete capsule of perineural cells. This entity warrants wider recognition by urologists and uropathologists. To our knowledge this is the first description of this kind of tumor in the glans penis.

The purpose of this investigation is to review the outcome of 83 consecutive acoustic neuroma patients, managed solely or in part by the Department of Otolaryngology at University Hospital, London, Ontario, over a 6-year period (1987 to 1993). The majority of patients underwent operative intervention, utilizing the middle cranial fossa, suboccipital, or most commonly, the translabyrinthine approach depending on the tumour size and level of residual hearing. Demographic, preoperative, intraoperative, and postoperative data are analyzed including complications of surgery. As well, we report on o…

Up to 15% of patients with acoustic neuroma may have a history of sudden hearing loss. Recovery from sudden hearing loss with acoustic neuroma is rare. In this paper, a case of bilateral acoustic neuromas presented as repeated sudden hearing loss with recovery on the left side after removal of right-sided tumor were reported. She experienced three episodes of sudden hearing loss, showing various types of audiograms and ABR on the left side. There was no relationship between the type of audiograms and ABR findings. Factor influencing ABR might be different from the cause of hearing impairment. …

Cerebrospinal fluid (CSF) leak has been a constant and unresolved complication of acoustic tumor surgery. This study retrospectively reviews 381 primary acoustic tumor surgeries performed by a single, senior, neurotologist and neurosurgeon team from 1979 through 1991. There were 68 cerebrospinal fluid leaks in 66 patients (66/381; 17%). There was no significant difference in the incidence of CSF leak between the translabyrinthine group (21%) and the retrosigmoid transmeatal group (16%). Translabyrinthine leaks were evenly divided between rhinorrhea and the postauricular wound while retrosigmoi

The results and complications of translabyrinthine and transotic surgery for petrous apex lesions between 1980 and 1992 are presented. An acoustic neuroma was found in 52 patients. In 1988, the translabyrinthine approach was modified into the transotic approach and replaced the former technique. There was no mortality in this series, but two patients had mild brainstem infarcts and there was post-operative bleeding into the cerebellopontine angle in one. Cerebrospinal fluid (CSF) leakage was seen in six patients and meningitis in two. Three suffered deep vein thrombosis in their legs. There wa…

A case of huge facial nerve neuroma was reported. The patient was a 59-year-old woman who had 5 years' history of right facial paralysis. A CT study with contrast medium showed extensive destruction of the right temporal bone including the semicircular canals, and with protrusion of the tumor into the cerebello-pontine angle. CT findings with contrast enhancement and MRI suggested a neuroma and its origin. The operation confirmed that the tumor arose from the vertical portion of the facial nerve. Previous papers dealing with this kind of tumor were reviewed to delineate clinical characteristic…

Patients consider facial paralysis the most concerning sequelae following acoustic tumor resection. Surgical and anesthetic refinements have lowered operative mortality to allow the surgeon to focus on preserving facial nerve function. Tumor size, microsurgical technique, and intraoperative monitoring are the most important factors that define the risk of postoperative facial paralysis. A protocol for uniform surgical reporting is proposed.

During a 14-year period from 1976 to 1989, the authors experienced 160 cases of acoustic neuroma (AN). The clinical features were investigated in 132 of these cases (excluding those cases of recurrence and neurofibromatosis 2). The cases were divided into three periods for study. There were no significant differences in the patients' age, sex, or affected side among the three periods. The number of intracanalicular tumors have increased in recent years. The proportion of medium-sized and large tumors, however, has not changed, comprising about 70% of the total number of cases. Tumors of 20 mm …

A case is presented in which, following the failure of several surgical and non-surgical procedures to control the increasingly severe pain in an amputation thumb stump, a toe transfer was carried out, successfully relieving the symptoms without transfer of the pain to the donor site. The patient has remained pain-free for 3 years.

The clinical features, diagnosis, and management of a series of 11 cases of trigeminal neuroma (four men, seven women; mean age 40 years) are summarized. Symptoms at presentation were generally those of cerebellopontine angle and petrous apex space occupying lesions. Computed tomography scanning and magnetic resonance imaging were found to provide the best preoperative localization of these lesions, allowing better planning of the surgical approach and thereby maximizing tumor exposure, an essential for any attempt at total excision, considering the large size (greater than 3 cm) of these tumo…

A series of 76 consecutive patients undergoing unilateral acoustic neuroma excision is reported. Preoperative facial weaknesses and a tumour larger than 2.5 cm were predictive factors of poor facial nerve recovery, re-emphasising the need to detect and remove these tumours at an early stage. Electroneuronography performed around one week after surgery appears to be a good indicator of the likelihood of recovery in facial nerves which are anatomically intact, but which show no function at that stage. Multiple rehabilitative procedures are often required when inadequate function or poor cosmetic…

A case of amputation neuroma, presenting incidious proptosis, was reported in a 44-year-old man. To the best of our knowledge the case seems to be the first report of amputation neuroma caused by trauma in the orbita.

A number of new concepts in the pathology, diagnosis, and management of Morton's metatarsalgia have been presented in the recent literature. Probably as many questions as answers have been generated by these publications. The taut transverse metatarsal ligament appears to play a critical role compressing the interdigital nerve but the exact pathomechanics producing the neuroma and the role of the intermetatarsal bursa remain unclear. Electrodiagnostic techniques for this condition are in the early development stage and may be clinically applicable in the near future. Support for and recommenda…

Successful clinical management of symptomatic neuromas continues to present a challenge to the responsible surgeon. It is unexplained why some patients with neuromas are completely asymptomatic while others exhibit debilitating symptoms. Prevention of neuromas is paramount with precise attention to severed nerves following amputations and other surgical procedures. Once established, treatment of neuromas consists of careful patient counseling, local massage, and desensitization procedures. Sympathetic blockade with guanethidine may be beneficial in some patients. When necessary, surgical excis…
