Intraoperative Electromyographic Monitoring in Children Less Than 1 Year of Age.

BACKGROUND: Despite the potential to improve patient outcomes, quantitative train-of-four (TOF) monitoring has seen limited use in infants and children primarily due to the lack of effective equipment. The current study investigates the feasibility of using an electromyography-based TOF monitor (TetraGraph) in patients < 1 year of age. METHODS: Following informed consent, patients < 1 year of age presenting for elective surgery that required use of a non-depolarizing neuromuscular blocking agent were enrolled. The TetraGraph electrodes were positioned on the volar aspect of the forearm over t

Chronic glucocorticoid management in neuromuscular disease: A survey of neuromuscular neurologists.

INTRODUCTION/AIMS: Glucocorticoids (GC) are first-line therapy for many neuromuscular diseases. There is a lack of guidelines regarding the prevention and management of GC complications in the context of neuromuscular disease, introducing the potential for practice variation, that may compromise quality of care. Our aim was to evaluate the practice patterns among Canadian adult neuromuscular neurologists on the screening, management, and treatment of GC-related complications and to identify variances in practice. METHODS: A web-based anonymous questionnaire was disseminated to 99 Canadian adu

Neuromuscular disorders in the omics era.

Neuromuscular disorders encompass a spectrum of conditions characterized by primary lesions within the peripheral nervous system, which include the anterior horn cell, peripheral nerve, neuromuscular junction, and muscle. In pediatrics, most of these disorders are linked to genetic causes. Despite the considerable progress, the diagnosis of these disorders remains a challenging due to wide clinical presentation, disease heterogeneity and rarity. It is noteworthy that certain neuromuscular disorders, once deemed untreatable, can now be effectively managed through novel therapies. Biomarkers eme

From load monitoring to training decisions: a practical approach using drop jump metrics in semi-professional soccer.

BACKGROUND: Soccer performance involves frequent high-intensity efforts and often insufficient recovery, making neuromuscular fatigue a critical factor in player monitoring. The drop jump (DJ) test, particularly the reactive strength index (RSI), has gained attention as a practical tool for assessing neuromuscular readiness. This study examined the relationship between accumulated external training load and neuromuscular status, assessed via DJ-derived metrics, in semi-professional soccer players. METHODS: Nineteen semi-professional outfield players (20.7 ± 1.4 years) were monitored over a 30

Sensitivity to vecuronium after botulinum toxin administration.

When used to treat focal dystonias, botulinum toxin may cause a transient impairment of neuromuscular transmission in muscles distant from those injected. These systemic effects are not clinically evident, but should not be ignored when patients are exposed to other drugs or conditions that also impair neuromuscular transmission. A patient is described who underwent general anesthesia twice during treatment with botulinum toxin for a long history of blepharospasm. On both occasions, the neuromuscular block produced by vecuronium (0.05 mg kg-1) was monitored in the abductor digiti minimi muscle

Transcutaneous respiratory electromyographic monitoring.

The integrated diaphragm electromyogram (EMG) signal reflects function from the inspiratory centers to the neuromuscular junction. The feasibility and potential value of transcutaneous diaphragm electromyography (tcEMG) was confirmed in a group of infants using two prototype respiratory EMG monitors. Infants were monitored continuously for periods ranging from hours to days. One hundred were monitored for clinical reasons, looking for disordered respiratory behavior, while 47 were studied for technical/experimental reasons. Reliable measurements of diaphragm EMG activity were obtained, provide

A single-centre, randomised comparison of the performance and safety of a novel mechanomyography sensor with electromyography.

BACKGROUND: Quantitative neuromuscular monitoring is crucial to ensure patient safety when using neuromuscular blocking agents. Although historically considered as the research standard for neuromuscular monitoring, currently, no certified mechanomyography device is available for routine clinical use. This study aimed to investigate noninferiority in precision of a newly developed mechanomyography (TOF3D mechanosensor; MIPM, Mammendorf, Germany) compared with electromyography. METHODS: We conducted a prospective, interventional, single-centre agreement study comparing electromyography and mec…

Practical Guide to Management of Long-Term Noninvasive Ventilation for Adults With Chronic Neuromuscular Disease.

Recent technological advances in respiratory support and monitoring have dramatically enhanced the utility of long-term noninvasive ventilation (NIV). Improved quality of life and prolonged survival have been demonstrated for several common chronic neuromuscular diseases. Many adults with progressive neuromuscular respiratory disease can now comfortably maintain normal ventilation at home to near total respiratory muscle paralysis without needing a tracheostomy. However, current practice in many communities falls short of that potential. Mastery of the new technology calls for detailed awarene

Maternal respiratory distress and successful reversal with sugammadex during intrauterine transfusion with fetal paralysis.

A 70 kg, 34-year-old woman at 29 weeks-of-gestation required intrauterine transfusion for Rh (D) alloimmunization. In the ambulatory treatment clinic, 19 mg of rocuronium was administered intramuscularly in split doses into the fetal buttock. The fetus moved and inadvertent maternal neuromuscular blockade occurred, leading to respiratory distress. The patient was transferred to the operating room where she had poor muscle tone, dyspnea and dysphonia. Sugammadex 100 mg was administered intravenously and complete resolution of neuromuscular blockade was demonstrated using a Neuromuscular Transmi

ClC-1 Inhibition as a Mechanism for Accelerating Skeletal Muscle Recovery After Neuromuscular Block in Rats.

Neuromuscular blocking agents are used commonly to induce skeletal muscle relaxation during surgery. While muscle relaxation facilitates surgical procedures and tracheal intubation, adequate recovery of muscle function after surgery is required to support pulmonary function, and even mild residual neuromuscular block increases the risk of severe postoperative pulmonary complications. While recovery of muscle function after surgery involving neuromuscular blocking agents can be monitored and, in addition, be accelerated by use of current antagonists (reversal agents), there is a clear clinical

A case where rocuronium was unable to achieve neuromuscular block immediately after sugammadex administration.

We present a case where immediate muscle relaxation was needed following sugammadex administration. A 72 year-old female underwent surgery for a cerebral artery aneurysm. Upon conclusion of the operation sugammadex (9.3 mg/kg) was administered and the patient was noted to have left hemiplegia. Rocuronium (1.2 mg/kg × 2 doses) was given in order to gain neuromuscular block approximately 25 minutes after sugammadex had been injected. Although TOF monitoring was not utilized in this case and assessing residual muscular block was difficult, spontaneous respirations continued and breathing had to b

[Anesthesia for a patient with xeroderma pigmentosum].

A 23-year-old man with xeroderma pigmentosum underwent laparoscopic cholecystectomy. He experienced transient worsening of the neurological symptom after anesthesia with volatile agents in the previous surgery. Because volatile anesthetics potentially cause genotoxic effects in patients with xeroderma pigmentosum, this time we chose total ir.travenous anesthesia (TIVA). The intraoperative management and the post-operative course were uneventful this time. From these two anesthesia experiences in one patient, we suggest that TIVA is more appropriate than anesthesia with volatile agents as a met

An observational study comparing the performance of TOF-Cuff with TOF-Scan monitoring during anaesthetic induction in clinical routine.

INTRODUCTION: Neuromuscular monitoring by acceleromyography assesses the effects of non-depolarising neuromuscular blocking agents used during anaesthesia induction to optimise intubation conditions. A new type of neuromuscular monitor, TOF-Cuff, integrates electrode stimulation into a blood pressure monitoring cuff. Comparisons of this device with TOF-Scan, considered a clinical standard acceleromyography device, have not been published. MATERIAL AND METHODS: This prospective, observational study was approved by the Ethics Committee East Switzerland (BASEC-nr. 2016-02044), and patients' cons…
