Semiology and Neurophysiology of Clonic Seizures: A Report of 39 Patients.

BACKGROUND AND OBJECTIVES: Clonic seizures are currently defined as repetitive and rhythmic myoclonic contractions of a specific body part, producing twitching movements at a frequency of 0.2-5 Hz. There are few studies in the literature that have reported a detailed analysis of the semiology, neurophysiology, and lateralizing value of clonic seizures. In this article, we aim to report our findings from a retrospective review of 39 patients. METHODS: We identified 39 patients (48 seizures) from our center who had been admitted with clonic seizures between 2016 and 2022. We performed a retrosp

Multifaceted roles of DLG3/SAP102 in neurophysiology, neurological disorders and tumorigenesis.

DLG3, also known as Synapse-associated protein 102 (SAP102), is essential for the organization and plasticity of excitatory synapses within the central nervous system (CNS). It plays a critical role in clustering and moving key components necessary for learning and memory processes. Mutations in the DLG3 gene, which result in truncated SAP102 proteins, have been associated with a range of neurological disorders, including X-linked intellectual disability (XLID), autism spectrum disorders (ASD), and schizophrenia, all of which can disrupt synaptic structure and cognitive functions. Abnormal SAP

Cost-Reducing Traits for Agonistic Head Collisions: A Case for Neurophysiology.

Many animal species have evolved extreme behaviors requiring them to engage in repeated high-impact collisions. These behaviors include mating displays like headbutting in sheep and drumming in woodpeckers. To our knowledge, these taxa do not experience any notable acute head trauma, even though the deceleration forces would cause traumatic brain injury in most animals. Previous research has focused on skeletomuscular morphology, biomechanics, and material properties in an attempt to explain how animals moderate these high-impact forces. However, many of these behaviors are understudied, and m

Auditory neurophysiology reveals central nervous system dysfunction in HIV-infected individuals.

OBJECTIVE: To test the hypothesis that human immunodeficiency virus (HIV) affects auditory-neurophysiological functions. METHODS: A convenience sample of 68 HIV+ and 59 HIV- normal-hearing adults was selected from a study set in Dar es Salaam, Tanzania. The speech-evoked frequency-following response (FFR), an objective measure of auditory function, was collected. Outcome measures were FFRs to the fundamental frequency (F0) and to harmonics corresponding to the first formant (F1), two behaviorally relevant cues for understanding speech. RESULTS: The HIV+ group had weaker responses to the F1 t

What Is the Role for Pelvic Neurophysiology Testing in Neurourology Practice?

Pelvic neurophysiology testing uniquely evaluates the sacral S2-S4 segmental innervation, but most of these tests assess only somatic innervation and not the splanchnic nerves innervating the lower urinary tract, genitalia, and lower bowel. Nevertheless, there are specific situations in which these tests play an important role in evaluating patients with urogenital complaints.

Differential Diagnosis of Chronic Neuropathic Orofacial Pain: Role of Clinical Neurophysiology.

Orofacial pain syndromes encompass several clinically defined and classified entities. The focus here is on the role of clinical neurophysiologic and psychophysical tests in the diagnosis, differential diagnosis, and pathophysiological mechanisms of definite trigeminal neuropathic pain and other chronic orofacial pain conditions (excluding headache and temporomandibular disorders). The International Classification of Headache Disorders 2018 classifies these facial pain disorders under the heading Painful cranial neuropathies and other facial pains. In addition to unambiguous painful posttrauma

Ellen Grass Memorial Lecture: Clinical Neurophysiology in the Treatment of Disease.

Clinical neurophysiology has a long-standing history and value in the diagnosis of neurologic diseases. Because of their unique ability to assess physiology and function of the nervous system, electroencephalography (EEG), evoked potentials, electromyography (EMG), and polysomnography have long been used in the diagnostic evaluation of epilepsies, demyelinating disorders, neuromuscular disorders, sleep disorders, and other diseases affecting the central and peripheral nervous systems. The role of these tests and clinical neurophysiology in general has usually ended upon diagnosis. New applicat

Neurophysiology and new techniques to assess esophageal sensory function: an update.

This review aims to discuss the neurophysiology of the esophagus and new methods to assess esophageal nociception. Pain and other symptoms can be caused by diseases in the mucosa or muscular or sphincter dysfunction, together with abnormal pain processing, either in the peripheral or central nervous systems. Therefore, we present new techniques in the assessment of esophageal function and the potential role of the mucosal barrier in the generation and propagation of pain. We discuss the assessment and role of esophageal sphincters in nociception, as well as imaging and electrophysiological tec

Anatomy and neurophysiology of cough: CHEST Guideline and Expert Panel report.

Bronchopulmonary C-fibers and a subset of mechanically sensitive, acid-sensitive myelinated sensory nerves play essential roles in regulating cough. These vagal sensory nerves terminate primarily in the larynx, trachea, carina, and large intrapulmonary bronchi. Other bronchopulmonary sensory nerves, sensory nerves innervating other viscera, as well as somatosensory nerves innervating the chest wall, diaphragm, and abdominal musculature regulate cough patterning and cough sensitivity. The responsiveness and morphology of the airway vagal sensory nerve subtypes and the extrapulmonary sensory ner

Clinical neurophysiology of visual and auditory processing in dyslexia: a review.

Neurophysiological studies on children and adults with dyslexia provide a deeper understanding of how visual and auditory processing in dyslexia might relate to reading deficits. The goal of this review is to provide an overview of research findings in the last two decades on motion related and contrast sensitivity visual evoked potentials and on auditory event related potentials to basic tone and speech sound processing in dyslexia. These results are particularly relevant for three important theories about causality in dyslexia: the magnocellular deficit hypothesis, the temporal processing de

Johannes Muller and the beginnings of experimental neurophysiology: concepts and strategies.

Between 1828 and 1838 Johannes Muller (1801-1858) published his important neurophysiological and anatomical investigations (primary fibres, cranial nerves, reflex movement, experimental proof of Charles Bell's law). Muller's strategies and concepts in neurophysiology can be used as an example for his scientific approach. An analysis of the four editions of Muller's Handbook of Physiology shows that he specified his arguments continuously by introducing clear definitions of physiological terms (stimulus, excitability, cosensual movement, reflex), which were based on his own manifold experiments

Divergence of cortical neurophysiology across different neurodegenerative disorders compared to healthy ageing.

Neurodegenerative diseases involve disruption of healthy brain network communication occurring before the emergence of symptoms. Magnetoencephalography (MEG) is sensitive to the magnetic fields generated by cortical neuronal activity, and is the most spatio-temporally accurate method of directly assessing neuronal activity non-invasively. We used MEG to directly compare three neurodegenerative disorders with a large healthy cohort to characterise patterns of activity deviating from healthy ageing. Task-free MEG recordings were acquired from patients with Alzheimer's disease (AD, n = 29), Parki

Neurophysiology of Downhill Mountain Bike Athletes-Benchmark Assessments of Event-Related Potentials.

Background: Downhill mountain biking (MTB) is a high-velocity sport where riders are potentially exposed to impacts and concussion. Rapidly obtained Event-Related Potentials (ERPs) are a promising use of portable electroencephalography for clinical assessment of concussion-related neurological impairment at the point of care. However, the baseline neuroelectric profile of this specific sporting population is not fully understood. This study investigated the amplitude and latency of the N100, P300 and N400 ERPs at baseline among MTB athletes by age, sex and caffeine consumption. Methods: MTB at

Agonist-antagonist myoneural interface amputation preserves proprioceptive sensorimotor neurophysiology in lower limbs.

The brain undergoes marked changes in function and functional connectivity after limb amputation. The agonist-antagonist myoneural interface (AMI) amputation is a procedure that restores physiological agonist-antagonist muscle relationships responsible for proprioceptive sensory feedback to enable greater motor control. We compared results from the functional neuroimaging of individuals (n = 29) with AMI amputation, traditional amputation, and no amputation. Individuals with traditional amputation demonstrated a significant decrease in proprioceptive activity, measured by activation of Brodman
