A brief history of neurosurgery in Bosnia and Herzegovina: historical vignette.

The modern period of neurosurgery in Bosnia and Herzegovina began with the first neurosurgical procedure performed by Dr. Karl Bayer in 1891 on 3 patients with depressed skull fractures and epilepsy. In 1956 the Department of Surgery in Sarajevo designated several beds specifically for a neurosurgical unit. A significant milestone in the history of neurosurgery in Bosnia and Herzegovina was the establishment of the Division of Neurosurgery at the Clinical Center University of Sarajevo in 1970. The first neurosurgeon to complete his training in Bosnia and Herzegovina was Dr. Faruk Konjhodžić. T

Robot-assisted procedures in pediatric neurosurgery.

OBJECTIVE During the last 3 decades, robotic technology has rapidly spread across several surgical fields due to the continuous evolution of its versatility, stability, dexterity, and haptic properties. Neurosurgery pioneered the development of robotics, with the aim of improving the quality of several procedures requiring a high degree of accuracy and safety. Moreover, robot-guided approaches are of special interest in pediatric patients, who often have altered anatomy and challenging relationships between the diseased and eloquent structures. Nevertheless, the use of robots has been rarely r

Anaesthetic management of intracranial hypertension and pregnancy. A case report.

Hydrocephalus is an active distension of the ventricular system of the brain. The improved survival rates of patients with neurosurgical pathology is accompanied by a greater number of non-neurosurgical procedures in patients who have therapeutic neurosurgical devices. The real incidence of pregnancy in patients with obstructive hydrocephalus controlled with ventriculoperitoneal shunt (VPS) is unclear. We present a case of a pregnant 34-year-old female with a VPS for obstructive hydrocephalus. Due to VPS obstruction secondary to uterus volume, she presented several episodes of neurological imp…

Endoscopic Approaches to Intraventricular Lesions.

BACKGROUND: Endoscopy is becoming increasingly popular for the neurosurgical management of intraventricular lesions and has recently been accepted as an effective alternative approach to open surgery. The deep location of intraventricular lesions makes the microsurgical approach difficult. Moreover, many intraventricular tumors do not require aggressive neurosurgical treatment. Some of these lesions are even associated with hydrocephalus or an enlarged ventricular system. METHODS: We collected the data of 32 patients affected by purely intraventricular lesions in the lateral or third ventricl

Identifying Gaps in Bolivian Neurosurgery: A Systematic Review and Proposed Framework for Resource Implementation.

Neurosurgical systems in low- and middle-income countries (LMICs) often face challenges in infrastructure, technology, workforce, and continuity of care. Bolivia, an LMIC in South America, remains underrepresented in global neurosurgical research. Correspondingly, this study aimed to systematically review the neurosurgical literature of Bolivia to identify existing strengths, key gaps, and opportunities for targeted investment and technological implementation. A systematic review was conducted using PubMed, Scopus, and Embase to identify studies reporting original data on neurosurgical care in

[Renal transplantation: surgery in the patient with a functioning kidney].

One hundred and seven surgical procedures performed on 98 patients with stable renal allograft function are analyzed. They represent 35.5% of 276 patients who underwent renal transplants. The surgical procedures were not related to the graft or its arteries and vein. The procedures were gynecologic 8, urologic 24, thoracic 6, ophthalmologic 8, neurosurgical 2, general surgery 37, orthopedic 17 and plastic surgery 5. Thirty-five (32.7%) were emergency and 72 (67.3%) elective procedures. Twenty-six patients were operated on for infections. The overall operative mortality rate was 15.3%. It was s

Three-dimensional computer assisted magnetic resonance imaging for neurosurgical planning in parasagittal and parafalcine central region tumors.

Usually, conventional magnetic resonance spin echo images (MRI) are sufficient to establish the diagnosis of intracranial pathology. Planning and executing a neurosurgical procedure requires the ability of the neurosurgeon to transform these two-dimensional MRI into a three-dimensional (3-D) virtual image of the pathology and the surrounding neuronal anatomy. Such mentally performed transformations after sequential observation of the individual two-dimensional slices (i.e., MRI and angiography) may be virtual tasks that are very difficult or sometimes impossible to achieve. Using 3-D MRI data

Intraoperative Challenge: Managing Venous Air Embolism During Sitting Craniotomy.

Venous air embolism (VAE) represents a rare yet potentially life-threatening complication encountered during neurosurgical procedures, particularly craniotomy. Here, we present a case of a 30-year-old male undergoing excision of a cerebellar abscess who developed VAE midway through the procedure. Immediate recognition and intervention were paramount in managing the embolism effectively, ensuring a favorable surgical outcome. Vigilant monitoring, prompt cessation of the procedure, and implementation of preventive measures such as oxygen therapy and venous air aspiration were pivotal in mitigati…

Deep brain stimulation for obsessive-compulsive disorder.

Neurosurgical interventions have been used for decades to treat severe, refractory obsessive-compulsive disorder (OCD). Deep brain stimulation (DBS) is a neurosurgical procedure that is used routinely to treat movement disorders such as Parkinson's disease and essential tremor. Over the past two decades, DBS has been applied to OCD, building on earlier experience with lesional procedures. Promising results led to Humanitarian Device Exemption (HDE) approval of the therapy from the United States Food and Drug Administration in 2009. In this review, the authors describe the development of DBS fo

Cushing's experience with the surgical treatment of spinal dysraphism.

Although Harvey Cushing is best known for his role in developing surgical treatments for tumors of the central nervous system, he performed diverse neurosurgical procedures throughout his career, both at The Johns Hopkins Hospital (1886--1912) and at the Peter Bent Brigham Hospital (1912--1932). His unique and innovative approach to the treatment of myelomeningoceles associated with hydrocephalus, displayed early in his career, is characteristic of his attempts to circumvent the technical limitations of his time in the management of neurosurgical problems. In this report, the authors discuss t…

Delayed tension pneumocephalus following decompressive craniectomy.

Pneumocephalus is defined as a collection of air in the cranial cavity and is a common finding following intracranial neurosurgical procedures or following skull base fractures. The term tension pneumocephalus describes the accumulation of air under pressure such that it exerts mass effect and leads to a neurological deterioration. Whilst this is a rare event it can be life threatening and requires prompt intervention. We describe a case that occurred in a delayed fashion following a decompressive craniectomy for a severe traumatic brain injury. It was initially treated by urgent aspiration at…
