Advanced Practice Registered Nurses. Final rule with comment period.

The Department of Veterans Affairs (VA) is amending its medical regulations to permit full practice authority of three roles of VA advanced practice registered nurses (APRN) when they are acting within the scope of their VA employment. Certified Registered Nurse Anesthetists (CRNA) will not be included in VA's full practice authority under this final rule, but comment is requested on whether there are access issues or other unconsidered circumstances that might warrant their inclusion in a future rulemaking. The final rulemaking establishes the professional qualifications an individual must po

Bringing critical realism to nursing practice: Roy Bhaskar's contribution.

In the context of modern nursing practice that is embedded within complex social situations, critical discussions about the contribution of major philosophers are relevant and important. Whilst nurse theorists have advanced and shaped nursing as a discipline, other major philosophers can offer much to advance nursing enquiry. In this paper, we focus on philosopher Roy Bhaskar who, amongst others, developed critical realism, a philosophy for social science which connects with how many of us think about the world. Bhaskar's work focuses our attention on the interplay between structure and agency

Legal, ethical and practical considerations in research involving nurses with dyslexia.

AIM: To discuss the legal, ethical and practical considerations in UK studies involving nurses with dyslexia and medication administration errors (MAEs). BACKGROUND: Nurses with dyslexia are a vulnerable population as they are susceptible to misrepresentation in research, especially that which involves a sensitive topic such as MAEs. DISCUSSION: Nurses with dyslexia may be particularly vulnerable to research that could exploit, implicate or attribute unsafe practice to them and their disability. Special consideration should be exercised when researching this population. CONCLUSION: Despite

A study of the practice of individual genetic counsellors and genetic nurses in Europe.

Advances in genetics have meant that the genetic services are now accessed by increasing numbers of patients. One way of dealing with the pressure on services without jeopardising patient care is the inclusion of nonmedical genetic counsellors and genetic nurses in the genetic services team. However, a cohesive approach to the profession has been lacking in Europe, and an educational programme and registration system for European practitioners is required. The aim of this study was to ascertain the type of work undertaken by genetic nurses and counsellors in Europe and the context in which the

Medication administration via enteral tubes: a survey of nurses' practices.

AIM: This article is a report of a study examining the practices of acute care nurses when administering medication via enteral tubes. BACKGROUND: Administering medication via enteral tubes is predominantly a nursing responsibility across countries. It is important to establish what nurses actually do when giving enteral medication to inform policy and continuing education development. METHOD: In 2007, a survey was conducted using a random sample of acute care nurses at two large metropolitan hospitals in Melbourne, Australia. There were 181 Registered Nurses who participated in the study; 9

Characteristics of risk in patients of nurse practitioner safety net practices.

PURPOSE: The purpose of this article is to describe the characteristics of vulnerability in patients at safety net practices where nurse practitioners (NPs) provide the majority of the health care and to discuss the implications regarding patient data collection for all NPs who provide safety net care. DATA SOURCES: Data were collected from patients at four safety net practices: a rural health clinic, a rural nurse-managed health clinic, an urban nurse-managed health clinic, and an urban Indian health clinic. CONCLUSIONS: The users of these practices reflected a significant degree of risk fo

Budd-Chiari syndrome from an advanced practice nurse perspective.

PURPOSE: To provide nurse practitioners (NPs) with a case study and literature review of Budd-Chiari syndrome (BCS). This rare liver disease has a multitude of clinical presentations that NPs may encounter in the primary care setting. DATA SOURCE: A literature search was conducted in Pub-Med and CINAHL using key search words. Information for the case study was obtained from a patient and his gastroenterology specialist, who is a foremost expert in this field. CONCLUSION: BCS is complex and may be difficult to diagnose because of its atypical clinical presentation. Delayed diagnosis can affec

Management of skin reactions during radiotherapy: a study of nursing practice.

The aim of this study was to investigate the consensus of skin care advice given by nurses during radiotherapy. Sixty-seven nurses, identified through nine Belgian radiotherapy departments, responded to a questionnaire survey consisting of 58 items regarding prevention and management of erythema, dry desquamation and moist desquamation. Consensus for a given advice was categorized as small if less than 50% of the nurses gave the same answer, as moderate if between 50% and 75% and as large when more than 75%. Overall, 33% of the items showed small consensus, 29% showed moderate consensus and 38

Advance-practice nursing in heart-failure management: an integrative review.

The number of patients with heart failure (HF) is predicted to escalate into the next decade, whereas the number of cardiac specialists who are skilled in evidence-based recommendations in HF practice will struggle to provide available, quality care. The advance-practice nurse, whose focus is HF management, may be an important key to improving access to this growing aggregate. This integrative review indicates the positive cost-to-benefit ratio for the advance-practice nurse's collaboration in HF management. Other measurable nursing outcomes have yet to scratch the surface, thereby inviting st

HIV-related lipodystrophy: a clinical syndrome with implications for nursing practice.

Since the clinical introduction of protease inhibitors for the treatment of HIV disease in 1996, optimism has grown for the long-term survival of persons living with HIV disease. With the addition of protease inhibitors to highly active antiretroviral therapy, the number of deaths from AIDS has decreased dramatically. However, along with reports of dramatic clinical improvement, many cases of a novel lipodystrophy syndrome associated with high triglyceride levels, diabetes, accumulations of fatty tissue, and alterations in body shape have been reported by researchers, clinicians, and persons l

Advanced practice nursing in Texas: the interface of accreditation, regulation, and certification.

The rapidity and degree of development of regulation by boards of nursing of advanced practice nurses (APNs) and programs for their preparation has varied from state to state over the past 20 years. In general, boards have authority only to regulate advanced practice through the recognition of APNs and the setting of standards and scope of their practice. In the early 1990s, lack of consistent APN educational program standards and experiences and criteria for recognition of APNs was problematic at the levels of accreditation, certification, and regulation. The interdependency of these concepts

Nursing research reframed by the inescapable reality of practice: a personal encounter.

This paper describes how an innocent venture outside the confines of academia to update my nursing skills completely changed the focus of my research. I was deeply involved in the theoretical development of my thesis, which I thought was a feminist exploration of the meaning of health for mid-life women. I was immersed in feminist theory and was exploring the work of the French Feminists. I had written comprehensive draft chapters about nursing, women's bodies and science. While I was absorbed in my theoretical exploration I decided to venture back into practice to improve my nursing skills. I

Biological correlates of panic disorder with agoraphobia: practice perspectives for nurses.

The literature on panic disorder is summarized with regard to epidemiological, environmental, cultural, psychological, family, and genetic factors. Laboratory provocation of anxiety is reviewed with emphasis on the noradrenergic model of panic from a biological perspective. Positron emission tomography results involving brain asymmetries in lactate-sensitive panic patients are discussed, as are the psychopharmacological agents currently effective in the treatment of panic disorder. It is suggested that psychiatric nurses base their clinical practice on a holistic understanding of treatment imp

Digitalization of Comprehensive Geriatric Assessments for Nursing Practice: A Feasibility and Proof-of-Concept Study Toward Nursing Home Implementation.

Background/Objectives: Comprehensive Geriatric Assessment (CGA) is essential for maintaining quality of life (QOL) and independence in older adults. Still, its implementation is labor-intensive and difficult to sustain in aging societies such as Japan. Digital technologies may enable continuous, scalable CGA in daily living environments. This study aimed to develop and preliminarily evaluate a digital CGA (D-CGA) framework by integrating data from multiple monitoring devices, as a preparatory step toward Artificial Intelligence (AI)-supported personalized care planning. Methods: Four devices (
