Open Reduction and Internal Fixation of Femoral Head Fracture After Firearm Injury: A Case Report.

CASE: A 21-year-old man presented with a femoral head (FH) fracture from a firearm injury. The FH had a comminuted, minimally displaced fracture with intertrochanteric extension and a retained bullet. This patient was treated with surgical hip dislocation (SHD) and open reduction internal fixation (ORIF). This case describes the operative management of a fracture pattern of the FH not covered within the Pipkin classification. The patient was noted to be progressing well at 1-year follow-up. CONCLUSION: FH fractures from firearm injuries may present with unique fracture patterns. SHD and ORIF

The Influence of Opioid Use Disorder on Open Reduction and Internal Fixation Following Ankle Fracture.

BACKGROUND: Ankle fractures are common and may require open reduction and internal fixation (ORIF). Literature is scarce evaluating the associations of opioid use disorder (OUD) with ORIF postoperative outcomes. This study investigates whether OUD patients have increased (1) costs of care, (2) emergency room visits, and (3) readmission rates. METHODS: ORIF patients with a 90-day history of OUD were identified using an administrative claims database. OUD patients were matched (1:4) to controls by age, sex, and medical comorbidities. The Welch t-test determined the significance of cost of care.

Arthroscopically Assisted Versus Standard Open Reduction and Internal Fixation Techniques for the Acute Ankle Fracture.

BACKGROUND: Ankle fractures represent one of the most common orthopaedic injuries requiring operative treatment. Although open reduction and internal fixation (ORIF) of ankle fractures leads to good results in most patients, poor functional outcomes continue to be reported in some patients for whom anatomic reduction was achieved. It has been theorized that these lesser outcomes may in part be due to a component of missed intra-articular injury that reportedly ranges between 20% and 79%, although to date the true explanation for this subset of lower functional outcomes remains unknown. Such co

The treatment of malocclusion after open reduction of maxillofacial fracture: a report of three cases.

The posttraumatic complications of jaw fractures related to jaw function and facial deformity include nonunion, malunion, malocclusion, temporomandibular joint dysfunction and facial asymmetry. This report presents cases referred to our department for revision of malunion and malocclusion following inadequate reduction of jaw fractures. Three patients with posttraumatic malocclusions caused by malunion were treated with a LeFort I osteotomy in one case and re-fracture in two cases. All of the patients exhibited stable results without further complications (e.g., malunion or malocclusion). Accu

Open reduction and internal fixation of an apparent capitellar fracture using an extended lateral exposure.

Apparent fractures of the capitellum are often more complex involving the trochlea and the posterior aspect of the lateral column. Many of these fractures can be addressed through an extended lateral exposure, but fixation techniques specific to small, nearly entirely articular fragments will be necessary. When the fracture fragments do not fit, there is usually posterior impaction of the lateral column and sometimes the posterior trochlea.

Intra-articular neuropathic fracture of the calcaneal body treated by open reduction and subtalar arthrodesis.

A novel pattern of neuropathic intra-articular calcaneal fracture in a diabetic patient is described. This fracture combined proximal retraction of the tuberosity and body along with hindfoot collapse and plantar ulceration. Following control of the acute Charcot process with total contact casting, surgical reduction and subtalar arthrodesis was performed to stabilize the hindfoot and decrease the risk of recurrent ulceration. After healing, the patient successfully resumed ambulation and presently uses extra-depth shoes for daily activities. Despite the risks of surgical treatment, this diffi

Advances in Open Reduction and Internal Fixation of Multiple Mandibular Fractures with Condylar Involvement.

Multiple mandibular fractures with condylar involvement are challenging to treat. This article presents a series of steps to simplify the treatment and improve the outcomes in open reduction and internal fixation of multiple mandibular fractures with condylar involvement. Four new elements are identified: three-dimensional printing, model surgery, Kirschner wires to manipulate the segments in condylar fractures, and a top-to-bottom sequence. This algorithm has contributed to streamlining the procedure and improving the outcomes of these challenging fractures. For the same operator, reduction a
