Differences in Cholecystectomy Outcomes and Operating Time Between Male and Female Surgeons in Sweden.

IMPORTANCE: Female surgeons are still in the minority worldwide, and highlighting gender differences in surgery is important in understanding and reducing inequities within the surgical specialty. Studies on different surgical procedures indicate equal results, or safer outcomes, for female surgeons, but it is still unclear whether surgical outcomes of gallstone surgery differ between female and male surgeons. OBJECTIVE: To examine the association of the surgeon's gender with surgical outcomes and operating time in elective and acute care cholecystectomies. DESIGN, SETTING, AND PARTICIPANTS:

It's All About the Timing: When Do I Take This Injury to the Operating Room?

Over time, what was considered urgent or emergent in orthopaedic trauma has been revisited, and as awareness of factors associated with outcomes has increased, priorities have changed. There are multiple procedures performed urgently in the belief that early intervention allowed for better outcomes for the injury and the patient. Classic examples of conditions for which urgent intervention has been implemented include open fractures, femoral neck fractures in the young adult, talus fractures, and compartment syndrome. All of these conditions are considered nonurgent except for compartment synd

Results of spinal cord decompression and thoracolumbar pedicle stabilisation in relation to the time of operation.

Short-segment pedicle stabilisation, transpedicular disc resection and autologous cancellous bone grafting is performed in trauma patients being transferred to our trauma centre with thoracolumbar fracture/dislocations. A follow-up examination of 88 patients, who were operated during the years of 1985-1992, took place after an average time of 5.6 years. Our results suggest that the earlier operative decompression and spine stabilisation takes place, the better is the recovery rate in patients with neurologic deficits. The highest neurologic recovery rates were found in patients operated within

[Ultrasonographic rating score in evaluation of pathological severity and timing of operation for acute cholecystitis].

A rating score of ultrasonography to evaluate the pathological severity and timing of operative intervention of acute cholecystitis was designed. The ultrasonic rating score consisted of 7 items with were sensitive and specific in diagnosis of acute cholecystitis, accorded with the findings during operation. Emergency cholecystomies performed on patients with score < or = 10, 11-18 and > 18 points were 1/30, 4/42 and 7/13 respectively, followed by definite procedures. The results suggest that the ultrasonographic rating score is an useful approach in ultrasound diagnoses and timing of operatio

[Clinical study on developmental hydrocephalus and its operative timing in lumbo-sacral meningomyelocele].

Nine infants with lumbo-sacral meningomyelocele were evaluated in the diagnosis and the operative timing of hydrocephalus. Seven cases received early operation for meningomyelocele within 36 hours after birth. Two cases with closed meningomyelocele were operated on 8 days and 32 days after birth. All of them were not infected in the central nervous system before and after its surgery. Five out of nine cases had rapidly developed hydrocephalus within a few weeks, and received ventriculo-peritoneal shunt (Shunt group: Case1-5). Two cases with gradual enlargement of the lateral ventricles and two

Bacteraemia during prostatectomy and other transurethral operations: influence of timing of antibiotic administration.

The relation between the timing of prophylactic antibiotic administration and the occurrence of bacteraemia during transurethral operations was studied in 112 patients whose urine was infected before operation. Blood cultures taken during operation were positive in 15 (60%) of 25 patients who did not receive appropriate antibiotics, in 13 (21%) of 63 patients who were given appropriate antibiotics less than 24 h before operation, and in none of 24 patients in whom antibiotic "cover" was started more than 24 h before operation. In all cases the bacteraemia was transient. No patient developed se

Does When We Operate Matter? Revisiting Surgical Timing in Pancreatic Trauma.

PURPOSE: Pancreatic injuries are rare but are associated with high morbidity and mortality, particularly when diagnosis or treatment is delayed. However, the optimal timing for surgical intervention remains unclear. This study aimed to assess the impact of surgical timing on outcomes of hemodynamically stable patients with blunt pancreatic trauma using data from the Trauma Quality Improvement Program (TQIP). METHODS: This retrospective study analyzed TQIP data (2017-2019) for hemodynamically stable patients with blunt pancreatic injuries who underwent pancreatic surgery. Outcomes including mo

Lightweight real-time detectors of apple-leaf diseases operating on embedded devices.

Agricultural leaf disease detection is crucial for early intervention and yield protection in precision agriculture. Among representative economic crops, such as apples, leaf lesions are typically small and appear in complex backgrounds, making accurate detection performed on resource-constrained embedded devices challenging. To address this, we propose a lightweight small-object detection models, namely the dynamic Differential Compensation Lightweight-YOLO (DCL-YOLO) model and its pruned version (DCL-YOLO-P), based on YOLO11n. A novel Dual-Aspect Feature Complementary Mapping (DAFCM) module

Multimodal neural operators for real-time biomechanical modelling of traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) remains a major public health concern, with over 69 million cases annually worldwide. Accurate patient-specific biomechanical modeling is critical for injury risk assessment, but it requires integrating heterogeneous data sources such as volumetric neuroimaging, scalar demographic parameters, and acquisition metadata. Conventional finite element solvers can perform this modeling, yet they remain too computationally expensive for time-sensitive clinical settings. Neural operators have emerged as a promising alternative by learning resolution-invariant ma

Laparoscopic Heller Myotomy with Dor Fundoplication: An Operation that has Withstood the Test of Time.

BACKGROUND: Achalasia is a primary esophageal motility disorder characterized by aperistalsis and defective relaxation of the lower esophageal sphincter in response to swallowing. Patients' symptoms include dysphagia, regurgitation, weight loss, chest pain and aspiration. The disease is idiopathic, and the goal of treatment is to eliminate the resistance determined by the abnormal lower esophageal sphincter, therefore allowing passage of the ingested food from the esophagus into the stomach. Three effective treatment modalities are available today-pneumatic dilatation, peroral endoscopic myoto

Endoscopic Endonasal Excision of a Complex Craniopharyngioma Operated Multiple Times by Transcranial Route.

BACKGROUND: Complex craniopharyngiomas pose a significant surgical challenge owing to its proximity to critical structures and its intrinsic nature to resist radical excision. OBJECTIVE: To show that endoscopic endonasal approach (EEA) is potentially a better alternative to transcranial approach in tumors that have been operated multiple times by transcranial route for achieving radical excision with minimal morbidity. MATERIALS AND METHODS: A 32-year-old male previously operated twice through interhemispheric approach for craniopharyngioma presented with blurring of vision accompanied by he

The Manchester operation - is it time for it to return to our surgical armamentarium in the twenty-first century?

We conducted a study to assess the efficacy and outcome results of Manchester operation for women with symptomatic elongation of uterine cervix between 2010 and 2020. Forty-five women were enrolled. Mean age was 54.2 ± 10.5 years, 57.5% were premenopausal. Mean follow up was 4.53 years. Most patients were pleased from the surgery: 29 (76.3%) very pleased and three (7.9%) quite pleased. There were four cases of late postoperative complications: hematometra, pyometra, vesicovaginal fistula and small bowel evisceration through the posterior vaginal fornix. All four cases required surgical interve

Impact of Operative Timing in Infective Endocarditis with Cerebral Embolism-The Risk of Intermediate Deterioration.

Cerebral embolism due to infective endocarditis (IE) is associated with significant morbidity and mortality. The optimal time-interval between symptomatic stroke and cardiac surgery remains unclear. This study aimed to analyze the patients' outcomes and define the potential risk factors with regard to surgical timing for IE patients with preoperative symptomatic cerebral embolism (CE). A total of 119 IE patients with CE were identified and analyzed with regard to operative timing: early (1-7 days), intermediate (8-21 days), and late (>22 days). The preoperative patient data, comorbidities and
