PURPOSE: Deep phenotyping of genetic retinal disease using multimodal adaptive optics ophthalmoscopy and protein structure variant analysis.
OBSERVATIONS: In a patient with extensive atrophy of the retinal pigment epithelium and yellow deposits in the retina, genetic testing identified two CYP4V2 variants: c.802-8_810delinsGC and c.1169G > A, p.Arg390His. AI-generated protein structures indicated loss of CYP4V2 function. Reflectance confocal and multiple-scattering Adaptive Optics Scanning Light Ophthalmoscopy (AOSLO) captured crystalline deposits throughout the retina as well as previously u…

A 76-year-old woman with a 1-month history of headache, jaw claudication, scalp tenderness, and blurred vision was admitted to our hospital. Erythrocyte sedimentation rate was highly elevated. Brain MRI showed marked perineural optic nerve enhancement and superficial temporal artery enhancement bilaterally. Neuro-ophthalmic examination detected left dominant decline in critical fusion frequency whereas visual acuity, visual fields, and ophthalmoscopy were normal. Intravenous pulse methylprednisolone was administered for 3 days to treat suspected giant cell arteritis (GCA); however, visual acui…

Direct ophthalmoscopy continues to be the "gold standard" method for retinal and optic nerve evaluation despite known challenges for learners to acquire this clinical skill and minimal opportunities for patient-facing practice. Here we review current literature regarding smartphone fundoscopy (SF) as an alternative learning tool that remains underutilized in Canadian medical schools. SF has significant advantages over direct ophthalmoscopy, such as enhanced learner visualization of ocular structure and pathologies, greater learner usability in comparison, and improved fundus assessment during …

Lumpfish use their vision to hunt prey or, in the case of aquaculture, to see and eat pelleted diets. A common anterior ocular opacity abnormality designated in the literature as "cataract" described in both farmed and wild lumpfish has not yet been characterized in detail at the pathobiological level. We describe here lens tissue changes associated with cataract in cultured and domesticated lumpfish. Methodology included gross observations, ophthalmoscopy and histology. Young adult cultured animals approaching 400 days post-hatch presented a range of anterior segment opacities associated with…

BACKGROUND: Sequential visual field testing is an extremely helpful adjunct to ophthalmoscopy and fundus photography in the management of cytomegalovirus (CMV) retinitis with the antiviral agents ganciclovir or foscarnet in patients with the acquired immune deficiency syndrome (AIDS). The authors studied the visual field defects found in a series of 110 patients with AIDS and CMV retinitis.
METHODS: Ophthalmoscopy and fundus photography were performed on all patients. Visual field analysis was performed with either tangent screen, Goldmann kinetic, or Humphrey automated static perimetry.

Isolated ocular toxoplasmosis developed in two patients with Hodgkin's disease. The diagnosis was made by ophthalmoscopy of the fundus, serologic tests, and evolution. This raises the possibility that Toxoplasma infection and the localization of retinochoroiditic lesions on the macula could be the results of the immunosuppression associated with Hodgkin's disease.

PURPOSE: An annular dark shadow (ADS) reflex has been observed while performing direct ophthalmoscopy on subjects with keratoconus. This study describes a method that may serve as a diagnostic technique for early keratoconus and may be used as a quantitative measure of severity.
METHODS: Healthy keratoconic subjects and keratoconus suspects underwent corneal tomography and a full ocular examination. Keratoconus severity was graded based on Belin ABCD criteria. An iPhone camera was connected to a direct ophthalmoscope to take a picture of the eye. The height of the ASD was measured using the A…
