Surgical-orthodontic management of dental impaction.

UNLABELLED: The aim of this study was to assess the management of impacted teeth using surgical-orthodontic techniques at a dental treatment centre in Casablanca and to suggest strategies designed to enhance treatment of dental impaction. MATERIALS AND METHODS: The sample comprised 30 patients represented by their clinical files (mean age: 17 years; standard deviation: 8.141). RESULTS: Impaction most often involved a single tooth (56.7%), generally the upper canine (66%). The location was buccal in 43.3% of cases and generally high (63.3%). Presurgical orthodontic treatment was required in 8

Total impaction of deciduous maxillary molars: two case reports.

AIM: The purpose of this report is to present two cases of totally impacted maxillary deciduous molars, considered a rarity in dental practice. BACKGROUND: Primary tooth impaction is quite rare during the development of primary dentition. Various factors contribute to the impaction of a deciduous tooth, including anklyosis, congenitally missing permanent teeth, defects in the periodontal membrane, trauma, injury of the periodontal ligament, precocious eruption of the first permanent molar, defective eruptive force, or a combination of these factors. REPORTS: Case #1: An 18-year-old male pres

Orthodontic movement into bone defects augmented with bovine bone mineral and fibrin sealer: a reentry case report.

Periodontal disease can lead to migration of anterior teeth with the presence of infrabony defects. This creates the opportunity for treating such patients with a combined orthodontic-periodontic treatment. In the presented clinical case, an adult periodontal patient with extrusion of the maxillary central incisors and an infrabony defect on their lingual aspects was treated. During the surgical procedure, the bone defects were augmented with a combination of porous bovine bone mineral (Bio-Oss) and a fibrin-fibronectin sealing system (Tissucol). Ten days after surgery, the active orthodontic

[Possibilities and limitations of treatment with clear aligners. An orientation].

The aim of this research was to evaluate the efficiency and effectiveness of an orthodontic treatment with clear-aligner systems, specifically with Invisalign®. In addition to a review of the literature, 4 orthodontists and 9 patients treated with fixed orthodontic therapy and Invisalign® were interviewed. The literature showed that it is difficult to correct an extrusion, rotation or overjet with clear aligners. Often 'refinement' (additional intervention during treatment) is necessary to achieve the best possible end result. The patients interviewed experienced few limitations in their daily

Management of Cervically Fractured Central Incisors: A Multidisciplinary Approach.

INTRODUCTION: Fracture of tooth structure at or below the gingival margin compromises rehabilitation and hampers esthetics and function. MANAGEMENT: Management of such cases by a post-core and crown restoration, or periodontal surgery or orthodontic extrusion alone may not always suffice in attaining a good result. CASE REPORT: A multi-disciplinary approach which includes all of the above mentioned procedures helps in long term success. CONCLUSION: Careful case evaluation, treatment planning and meticulous attention to detail are the keys to the best treatment outcome.

Angle Class II, subdivision, with agenesis of mandibular second molars and extrusion of maxillary second molars.

This clinical case reports the treatment of an Angle Class II malocclusion in a young woman with a balanced face affected by agenesis of second and third mandibular molars and subsequent extrusion of second maxillary molars. The atypical and peculiar occlusal anomaly led to individualized treatment proposed in order to normalize dental malpositions, with subsequent rehabilitation of edentulous areas by means of a multidisciplinary approach. This case was presented to the Brazilian Board of Orthodontics and Dentofacial Orthopedics (BBO) in partial fulfillment of the requirements for obtaining t

Crown lengthening revisited.

Over the last 37 years, crown-lengthening procedures have been used predictably to restore teeth broken down from caries, trauma, and extensive wear. With crown lengthening, the dentogingival junction is "re-created" at a more apical level on the root to accommodate the junctional epithelium and the connective tissue attachment. Forced eruption can be used in addition, or as an alternative, to tooth lengthening. The authors discuss the indications for tooth lengthening, forced eruption, and orthodontic extrusion, as well as the treatment planning for these procedures.

Nonsurgical treatment and stability of an adult with a severe anterior open-bite malocclusion.

OBJECTIVES: A skeletal anterior open-bite is a challenging malocclusion for the orthodontist due to the difficulty and instability of correction. Treatment options for the adult patient include extractions, anterior extrusion with intermaxillary elastics, posterior intrusion using skeletal anchorage, occlusal adjustment, and orthognathic surgery. Patient compliance plays a key role in posttreatment stability. The present case report demonstrates the orthodontic treatment of an adult patient who presented with a complex open-bite malocclusion. MATERIALS AND METHODS: Treatment involved the plac

Coupled FE Modeling of Superelastic NiTi Arch Wire and Bone Remodeling to Simulate 3D Orthodontic Tooth Movement During Leveling Phase.

Orthodontic tooth movement results from alveolar bone remodeling induced by mechanical loads applied to the teeth over extended periods during the unloading phase of arch wires. In clinical practice, orthodontic arch wires are inserted into bracket slots without precise knowledge of the forces transmitted to individual teeth. Excessive loads may lead to undesirable side effects, whereas insufficient forces may fail to initiate bone remodeling and tooth movement. This study aims to develop a 3D numerical approach based on finite element method to simulate orthodontic tooth movement during the a

Periodontal implications of surgical-orthodontic treatment of an impacted dilacerated maxillary incisor: A case report with a 2-year follow-up.

The treatment of an 8-year-old girl with a dilacerated maxillary incisor began in the mixed dentition; a modified palatal arch attached to the molars served as anchorage for the forced eruption of the dilacerated tooth to prevent the intrusion of the adjacent teeth and reduce the risk of root resorption. Two surgical sessions were planned: the first to permit the closed eruption; the second was an apically positioned flap to add attached gingiva to the labial side of the erupting tooth. The result was an optimal periodontal outcome; moreover, the roots of the adjacent teeth did not show any si

Orthodontic treatment for an unerupted and severely rotated maxillary central incisor. A case report.

BACKGROUND: Many reports have indicated the treatment for permanent teeth with disturbed eruption due to the presence of supernumerary teeth. However, successful treatment for an unerupted maxillary incisor with severe rotation and another maxillary incisor with disturbed eruption by a supernumerary tooth is quite rare. CASE REPORT: We treated a 7-year-old Japanese male with an unerupted and severely rotated maxillary right incisor, along with a maxillary left incisor due to interference with eruption by an inverted supernumerary tooth. Orthodontic treatment using surgical exposure, traction

Combined treatment of surgical extrusion and crown lengthening procedure for severe crown-root fracture of a growing patient: a case report.

BACKGROUND: Preservation of a healthy periodontium is critical for the long-term success of restored teeth. In cases of extensive caries, tooth fracture, inadequate crown length, and increased esthetic demands, the restorative margins need to be placed apical to the gingival margin. Violation of the biological width due to dental trauma frequently appears in clinical practice. There are three treatment options for preserving biological width and the ferrule effect: crown lengthening, orthodontic extrusion, and surgical extrusion. This case report describes the surgical intervention and fixed p

A finite element analysis of the effects of semipontic design on tooth movement during mesialization of the mandibular second molar with clear aligners.

INTRODUCTION: Loss of the mandibular first molar is common in orthodontic patients. One treatment option is the mesialization of the second and third molars. This study aimed to investigate the displacement and type of movement in the second molar during mandibular second molar mesialization with clear aligner treatment using finite element analysis in configurations with or without pontic, semipontic, and anatomic pontic for the edentulous space. METHODS: Mesialization of the mandibular second molar with clear aligner treatment was simulated using the AlGOR Fempro program (ALGOR, Inc, Pa) wi

Surgical extrusion of multiple teeth with crown-root fractures: a case report with 18-months follow up.

The objective of this case report is to describe the treatment procedure involved in surgical extrusion of multiple crown-root fractures and review the critical factors to be considered for successful and predictable outcome. The treatment of complicated crown-root fracture in anterior teeth is likely to compromise function and aesthetics when approached with conventional surgical crown lengthening. Orthodontic extrusion has also been suggested; however, it is time-consuming, aesthetically compromising and hardly applicable on multiple anterior crown-root fractures due to the limited source of

An application of a splint purposeful resin-bonded fixed partial denture after orthodontic treatment: a case report.

An adult male patient who had lost his maxillary left central incisor seven years ago in a traffic accident presented with a chief complaint about his unaesthetic appearance associated with the loss of his maxillary left central incisor space, a Class III molar occlusion, and an anterior open bite malocclusion due to tongue-thrust swallowing. Fixed orthodontic treatment was rendered following fan-type expansion of the maxilla. A Maryland bridge as a minimally invasive dentistry approach was used as a retention appliance and the patient's aesthetic appearance was restored.
