[Orthopedic Management of Skeletal Metastases].

Recent advances in drug therapy for metastatic bone diseases are changing the indications for surgical treatments. Bone modifying agents, including bisphosphonates and denosumab, have reduced skeletal-related events. Meanwhile, novel anticancer drugs that have been developed, especially molecular-targeted drugs, can improve the prognoses of patients with bone metastases, thereby potentially increasing the number of patients who could receive benefits from surgeries for metastases. A multidisciplinary approach and the early involvement of surgeons are required for the application of adequate or

Orthopedic aspects of collagen disorders.

PURPOSE OF REVIEW: The purpose of this article is to provide the pediatrician with a review of disorders that have the orthopedic manifestation of joint hypermobility. Hypermobility, also termed ligamentous laxity, may present in different parts of the body at different times throughout childhood and adolescence. It may be symptomatic or incidentally found on the physical examination. Many conditions that cause joint hypermobility resolve with nonoperative management, but occasionally operative intervention is required for the best patient outcome. RECENT FINDINGS: In addition, hypermobility

[Orthopedic management of spina bifida].

BACKGROUND: Spina bifida is associated with congenital deformities, such as kyphosis, spinal malformations, teratological hip dislocations, clubfeet, vertical talus and also with acquired deformities due to muscle imbalance and impaired biomechanics. The degree of the acquired deformities and the mobility of the patient depend on the level of the spinal lesion. DIAGNOSTICS: Neurological symptoms are mostly asymmetric and there is an inconsistent correlation between the anatomical level of the lesion and muscle function. Deficits of sensation are usually one to two levels lower than the motor

Orthopedic surgery in ancient Egypt.

BACKGROUND: Ancient Egypt might be considered the cradle of medicine. The modern literature is, however, sometimes rather too enthusiastic regarding the procedures that are attributed an Egyptian origin. I briefly present and analyze the claims regarding orthopedic surgery in Egypt, what was actually done by the Egyptians, and what may have been incorrectly ascribed to them. METHODS: I reviewed the original sources and also the modern literature regarding surgery in ancient Egypt, concentrating especially on orthopedic surgery. RESULTS: As is well known, both literary sources and the archaeo

[The beginnings of orthopedic surgery in Israel].

In early mandatory Israel, orthopedics was mainly conservative, The first modern orthopedic surgeon was Ernst Spira from Czechoslovakia who established an orthopedic service at the Beilinson Hospital in Petah Tikva and left in 1948 to establish the Orthopedic Department and the Rehabilitation Center in Tel Hashomer, which treated the War of Independence casualties including amputees and victims of spinal cord injuries. A second orthopedic department was opened in Tel Hashomer by Shmuel Weissman who left in 1961 to open the Orthopedic Department at the Ichilov hospital in Tel Aviv. Shmuel Weiss

Orthopedic management of spina bifida.

The management of orthopedic problems in spina bifida has seen a dramatic change over the past 10 years. The negative effects of spasticity, poor balance, and the tethered cord syndrome on ambulatory function are better appreciated. There is less emphasis on the hip radiograph and more emphasis on the function of the knee and the prevention of knee pain. The importance of the hip abductor muscle and its influence on gait and knee function has been realized. Important developments in the treatment of spinal deformity include the use of pedicle screws which allow better correction. The role of a

[Utilization of FRAX at orthopedic clinics].

A survey for orthopedic doctors was performed to evaluate the recognition and the usability of FRAX. As a result, a half of them have known FRAX; however, only a few recognized its advantage. Fracture risks were assessed by FRAX for out patients in the orthopedic osteoporosis clinic and also for subjects in the osteoporosis screening. Appropriate threshold of the major osteoporotic fracture for pharmaceutical intervention seemed to be 15% and be applied from 65 to 74 years of age based on the data.

Infections associated with orthopedic implants.

PURPOSE OF REVIEW: We review recent advances in the prevention, diagnosis and treatment of infections associated with joint prostheses and internal fixation devices. RECENT FINDINGS: The perioperative antimicrobial prophylaxis should be administered 60-30 min before incision or before inflation of the tourniquet. New diagnostic approaches include sonication of removed implants to dislodge adherent microorganisms growing in biofilms and the use of molecular techniques to improve diagnostic yield. Treatment of implant-associated infections without removal of the device is an established option

Thromboprohylaxis in orthopedic surgery and traumatology.

Orthopaedic and trauma surgery are classified according 3 groups of venous thromboembolic risk. Elective total hip replacement (THR) or total knee replacement (TKR), hip fracture surgery or trauma patients are at high risk. Isolated lower extremity injury with fracture is at moderate risk whereas this risk is low without fracture as well as with knee arthroscopy. In THR and TKR, low molecular weight heparin (LMWH), fondaparinux or melagatran-ximelagatran are strongly recommended. The routine use of other anticoagulants, in particular vitamin K antagonist are not recommended. In patients at hig

Orthopedic problems in sport climbing.

Sport climbing is associated with unique upper- and lower-limb injuries involving predominantly the hand, elbow, and shoulder, and to a lesser extent the foot. Many pathologic conditions are limited to sport climbing. Physicians treating sport climbers should be aware of these unique injuries and overuse syndromes. This article presents an overview of orthopedic problems (injuries, overuse syndromes, and fractures) resulting from sport climbing. Sport climbing is defined in the context of existing mountain sports, and its characteristics and technical terms are presented. The etiology, diagnos

[Expectations for alendronate in orthopedic surgery].

Alendronate is suggested not only to be useful for prevention of fracture as a treatment for osteoporosis but also to have an anti-inflammatory action. Moreover, as it is considered to be applicable for stress shielding, and prevention and treatment of osteolysis after arthroplasty, it is expected to be useful for the treatment of other orthopedic disorders as well as osteoporosis.

Orthopedic emergencies in cancer patients.

The most frequent orthopedic emergency in oncology patients is fracture. Stabilization of the entire fractured bone restores function and relieves pain. The site, quality, and extent of the lesion can identify impending fractures that should be stabilized. New methods of pelvic stabilization effectively bypass periacetabular bone deficiency. Spinal cord decompression is important to maintain neurologic function. Advances in segmental fixation of the spine have improved the outcome over what was achieved by radiation alone. Infection is common in neutropenic patients, and should be treated aggr

Orthopedic aspects of child abuse.

Fractures are one of the most common manifestations of child abuse. Pediatricians and other physicians caring for children must have a clear understanding of the injury patterns sustained by children in abusive situations and be able to recognize those injuries. A thorough history and physical examination and accurate radiographic interpretation are essential. Early identification of nonaccidental trauma is imperative because proper intervention has been shown to be effective.

[Antibiotic prophylaxis in orthopedic surgery].

The efficiency of prophylactic antibiotics in total hip replacement has been demonstrated by several studies. The rate of infection decreased approximatively from 3% to 1% with a cephalosporine 1 protocol. The rate of infection was not different after a five-day administration of cephalosporine 1 and a 2-day treatment of cephalosporine 2. In open fractures, the value of prophylactic antibiotics has to take in account the usual organisms of peroperative contamination as well as the anaerobes. Short administration can be used, however no randomized studies have proven their efficiency. For clean

[Orthopedic aspects of Rett syndrome].

Rett syndrome is a degenerative neurological disorder with onset between 6 and 18 months of age. Severity of motor impairment is variable; some patients lose the ability to walk whereas others walk nearly normally. Orthopedic problems, which mainly involve the lower limbs and spine, can increase functional impairment and cause discomfort. Abnormal joint alignment in the lower limbs due to muscle contractures requires physiotherapy and orthopedic appliances. Surgery may be necessary, in particular to prevent dislocation of the hip or to correct talipes equinus. Scoliosis is common and unrespons
