Considerable effort has been directed towards replacing lost teeth using tissue-engineering methods such as titanium implants. A number of studies have tried to modify bioinert titanium surfaces by coating them with functionally bioactive molecules for faster and stronger osseointegration than pure titanium surfaces. Recently, peptides have been recognized as valuable scientific tools in the field of tissue-engineering. The DLTIDDSYWYRI motif of the human laminin-2 α2 chain has been previously reported to promote the attachment of various cell types; however, the in vivo effects of the DLTIDDS

Although the current percutaneous osseointegrated (OI) prosthetic attachment systems are novel clinical treatments for patients with limb loss, there have only been limited translational studies undertaken to date. To bridge this knowledge gap, from a larger study group of 86 animals that were implanted with a novel percutaneous OI implant construct, 33 sheep were randomly selected from the 0-, 3-, 6-, 9- and 12-month groups for histomorphometric analyses of periprosthetic cortical bone tissue. At necropsy, implanted and nonimplanted limbs were harvested and processed for the evaluation of cor

The aim of the present review is to evaluate the English language literature regarding factors associated with the loss of oral implants. An evidence-based format in conjunction, when possible, with a meta-analytic approach is used. The review identifies the following factors to be associated with biological failures of oral implants: medical status of the patient, smoking, bone quality, bone grafting, irradiation therapy, parafunctions, operator experience, degree of surgical trauma, bacterial contamination, lack of preoperative antibiotics, immediate loading, nonsubmerged procedure, number o

BACKGROUND: The advent of osseointegrated prosthetic systems marks a significant milestone in the evolution of limb reconstruction and rehabilitation. Osseointegration implants are a direct skeletal interface for artificial limbs, introduced as bone-anchored limb prostheses that may improve many limitations of traditional socket-based prosthetics, such as discomfort, poor fit, skin irritation, and limited range of motion.
MAIN BODY: Since the discovery of the osseointegration phenomenon in 1940, several osseointegrated implant systems, such as OPRA, ILP, OPL, and ITAP, with varying biomechani

INTRODUCTION: Finger amputation significantly impairs hand function and self-image. When surgical reconstruction or replantation is not possible, a finger prosthesis can provide cosmetic and functional benefits. However, conventional prostheses are limited by poor retention and lack of sensation. In this study, we evaluate an alternative method using osseointegrated dental implants to anchor finger prostheses, which provide stable fixation and, in turn, improve function and sensory feedback.
METHOD: Four patients with traumatic amputations of a single finger each (thumb, index, middle, and ri

Osseointegration is the direct contact between living bone and a dental implant, with supporting evidence confirming the direct connection between bone and titanium, found using an electron microscope. However, the fundamental mechanisms and interconnections between the bone and titanium are not clearly understood. At present, osteoimmunology explores the interaction between bone and immune cells not only in the medical field but also in dentistry. Immunology in bone cell formation has long been a research topic; however, interest in these effects has recently surged. Through subsequent studie

BACKGROUND: Osseointegrated (OI) prostheses are a novel alternative solution for patients with lower-limb amputations. Plastic surgeons are becoming increasingly involved in providing concurrent thigh lifts (CTLs) to reduce soft tissue redundancy around the implant. Because of limited mobility, many patients present with higher body mass indexes (BMIs) and may benefit from CTL. This study aimed to evaluate outcomes in obese patients with OI prostheses who underwent CTL compared to those who did not. Additionally, we aimed to compare outcomes in patients with normal BMIs compared to obese patie

BACKGROUND: The transfemoral or transhumeral Osseoanchored Prostheses for the Rehabilitation of Amputees (OPRA) osseointegration technique is indicated in patients with transfemoral or transhumeral amputations who have difficulty with use of a conventional socket and traditional prosthesis. Augmentations to the procedure, such as targeted muscle reinnervation (TMR) and/or a regenerative peripheral nerve interface (RPNI), serve to enhance function (in the case of TMR) and/or improve residual-limb neuropathic pain (in the case of combined TMR and RPNI) in the same patient population.

PURPOSE: To evaluate the effect of continuous perioperative clopidogrel treatment on the osseointegration of titanium implants.
MATERIALS AND METHODS: A total of 32 New Zealand rabbits were randomly divided between two groups: a clopidogrel group (n = 16) and a control group (n = 16). For 1 week prior to the surgical placement of a titanium implant in their medial femoral condyle, rabbits in the clopidogrel group received 3 mg/kg of clopidogrel daily, and the control group received only the vehicle. This treatment was continued for another 6 weeks postoperatively. At 6 weeks, the rabbits were

Thumb amputations affect 50% of hand functionality. Common solutions consist of microsurgical treatments or silicone vacuum prosthesis. Not all patients are eligible for microsurgical treatment and the use of vacuum prosthesis is often discouraged because of their instability. On the contrary, osseointegrated prosthesis provide stable retention and osseoperception. This cadaveric study evaluated the process of a patient-matched osseointegrated prosthesis for the treatment of thumb amputees. Computed tomography (CT) medical images reconstruction provided information on metacarpal stump, used as

People with unilateral transfemoral amputation using socket prostheses are at increased risk for developing osteoarthritis in both the residual hip and intact lower-limb joints. Osseointegrated prostheses are a surgical alternative to socket prostheses that directly attach to the residual femur via a bone-anchored implant, however their multi-joint loading effect is largely unknown. Our objective was to establish how osseointegrated prostheses influence joint loading during walking. Motion capture data (kinematics, ground reaction forces) were collected from 12 participants at baseline, with s

BACKGROUND: The bone-implant gap resulted from morphological mismatch between cervical bony endplates and implant footprint may have adverse impact on bone-implant interfacial osseointegration of cervical disc arthroplasty (CDA). The purpose of the study was to evaluate the impact of bone-implant gap size on the interfacial osseointegration in a rabbit animal model.
METHODS: A series of round-plate implants with different teeth depth (0.5 mm, 1.0 mm, 1.5 mm and 2.0 mm) was specifically designed. A total of 48 New Zealand white rabbits were randomly categorized into four groups by the implants

BACKGROUND: The preparation of bone for the insertion of an osseointegrated transfemoral implant and the insertion process are performed at very low speeds in order to avoid thermal damages to bone tissue which may potentially jeopardize implant stability. The aim of this study was to quantify the temperature increase in the femur at different sites and insertion depths, relative to the final implant position during the stepwise implantation procedure.
METHODS: The procedure for installation of the osseointegrated implant was performed on 24 femoral specimens. In one specimen of each pair, th

Percutaneous osseointegrated (OI) implants are increasingly viable as an alternative to socket suspension of prosthetic limbs. Upper extremity prostheses have also become more complex to better replicate hand and arm function and attempt to recreate pre-amputation functional levels. With more functionality comes heavier devices that put more stress on the bone-implant interface, which could be an issue for implant stability. This study quantified transhumeral loading at defined amputation levels using four simulated prosthetic limb-types: (1) body powered hook, (2) myoelectric hook, (3) myoele

BACKGROUND: Percutaneous osseointegrated (OI) docking of prosthetic limbs returns loading directly to the residual bone of individuals with amputations. Lower limb diaphyseal biomechanics have not been studied during the wide range of daily activities performed by individuals with lower extremity amputations; therefore, little is known about the loads experienced at the bone-endoprosthetic interface of a percutaneous OI device.
RESEARCH QUESTION: Does residual limb length and/or gender influence loading magnitudes in the diaphysis of the femur or tibia during daily activities?
METHODS: This
