Restoration of oral function after maxillectomy with osseous integrated implant retained maxillary obturators.

This study assesses the success rate of osseous integrated implantation in assisting the prosthetic obturation of maxillectomy defects. Twenty-three patients received a total of 85 osseous integrated implants used for retaining maxillary obturators between 1985 and 1993. Defects include 13 radical maxillectomies, 5 premaxillary resections, 4 subtotal maxillectomies, and 1 soft-palate resection. Thirteen patients (50 implants) received a radiation dose ranging from 5,040 to 7,940 cGy. Implants can be placed at the time of ablation or subsequently. Efforts were made to spare uninvolved segments

Immediate rehabilitation of a palatal defected edentulous patient by implant-supported overdenture: a case report.

BACKGROUND: Immediate rehabilitation is a considerable therapeutic challenge but is necessary for edentulous patients with oronasal fistulas, especially those with inadequate residual bone and a history of radiotherapy. CASE PRESENTATION: We report a rare case of a 63-year-old patient who was missing the majority of his maxillary teeth and who had a defect due to palatal mucoepidermoid carcinoma resection. The patient also received radiotherapy twice within one year postoperatively. An implant-supported prosthesis with an obturator was fabricated immediately. CONCLUSION: This technique impro

Infrastructure maxillectomy for maxillary sinus and hard palate neoplasms.

Infrastructure maxillectomy is a surgical procedure to remove the lower part of the maxilla and hard palate. The objective of the present study was to analyze clinical data and treatment outcome of patients who underwent infrastructure maxillectomy between 2011 and 2019. A total of 13 patients who underwent infrastructure maxillectomy for maxillary sinus and hard palate neoplasms between 2011 and 2019 were analyzed. These patients were subdivided into maxillary sinus neoplasm (n=5) and hard palate neoplasm (n=8) groups. All patients except one underwent infrastructure maxillectomy using the su

[Repair of palatal fistulae in cleft patients].

OBJECTIVES: Treatment of oronasal fistulae in cleft patients remains a surgical challenge because of its high failure rate. The authors report the results of an aggressive surgical technique using the total elevation of palatal mucoperiosteum, even for small fistulae. METHODS: This approach was used on twelve consecutive patients, from five to 33 years of age, presenting with a Pittsburgh classification type IV palatal fistulae. The surgical procedure was total elevation of the hard palate mucoperiosteum starting from the dental sulcus combined with sealed double layer sutures. Clinical and p

Modified Technique to Fabricate a Definitive Closed Bulb Hollow Obturator for Maxillectomy in a Patient Following COVID-Associated Mucormycosis: A Case Report.

Immunocompromised people developed mucormycosis as a result of the COVID-19 outbreak. Antifungal medications, surgical excision of infected tissues, and therapy of underlying metabolic problems are available forms of treatment. Usually, surgery entails completely excising the affected area. The patient is at risk for nasal twang, nasal cavity leaks, and impaired masticatory function because of these anomalies. The obturator prosthesis may form an oro-nasal seal in such problems. Additionally, lowering the prosthesis weight contributes to improved stability and retention. This case report expla

Rehabilitation Using Hybrid Technique for Feeding Plate Fabrication in a 3-Month-Old Infant with Cleft Palate Defect Utilising Digital and Conventional Method: A Case Report.

Cleft palate leads to difficulty in suckling, speech abnormalities, dental problems, hearing loss and middle ear infections. Feeding plate acts as a barrier between the oral and nasal cavities allowing the normal development of jaws and enhanced nutritional supply to the patient. Fabrication of the feeding plate using digital means provide better adaptability due to the engagement of all the possible favourable anatomic undercuts, thereby, providing better retention. This case report discusses the hybrid technique including both the digital and conventional means for fabrication of a feeding p

Hard palate perforation in cocaine abusers: a systematic review.

Cocaine abuse has increased in the past decade, with a rise in the reported cases of midpalatine perforations produced as a result. The vasoconstrictive and caustic effect of the drug can produce direct irritation and ischemia of the nasal and palatine mucosa, leading over the long term to the creation of an oronasal perforation secondary to maxillary bone destruction. The present study offers a systematic review of all the clinical cases of necrotic nasopalatine perforations attributed to inhaled cocaine documented in the PubMed literature database. The main clinical characteristics of the di

Effect of obturators on facial form following surgery for head and neck cancer and impact on the perception of appearance.

OBJECTIVES: To describe the correlation between changes in the surface area and depth of the face in individuals wearing an obturator compared with it not being in place, and self-reported Quality of Life in relation to appearance. METHODS: Difference images were created from stereophotogrammetry images recorded from research participants with and without their obturators in place. On the difference images of the face, surface areas and mean depths were calculated. Oral health impact profile questionnaires (OHIP-49) were completed. RESULTS: Data from 19 research participants were recorded. T

Primary veloplasty: long-term results without maxillary deformity. a twenty-five year report.

Primary veloplasty during the first year of life enables normal growth of the upper jaw and of the facial skeleton in patients with clefts. In spite of the remaining cleft in the hard palate, intelligible speech can be learned. The closure of the residual cleft of the hard palate is generally postponed until the age of 12 to 14 years, when the normal growth of the jaw is virtually completed. This method has been used in the clinic by the author and his father for thirty years. The results after primary veloplasty for some hundred adult patients show normal maxillary and cranial growth both cli

Pronator quadratus musculo-osseous free flap for wide hard palatal defect reconstruction: An anatomical study.

Wide hard palate defects include congenital and acquired defects that are six square centimeters or larger in size. Obturator prostheses and autologous soft tissue transfers have been used to reconstruct palatal defects. This study aims to repair wide, hard palatal defects by using a pronator quadratus musculo-osseous free flap to achieve subtotal reconstruction. Seventeen formalin-fixed cadavers were dissected. Free musculo-osseous pronator quadratus flaps were prepared after a 12 cm curvilinear volar skin incision. Standard 30 × 23 mm (690 ± 52.12 mm2) hard palate defects were made by chisel

Presurgical Nasoalveolar Molding Techniques for a Complete Unilateral Cleft Lip and Palate Infant: A Case Report.

The purpose of this article was to present a modified protocol and devices of presurgical nasoalveolar molding techniques to improve deformity of lip, nose and alveolus at Department of Orthodontic, Khon Kaen University (KKU). This new protocol was developed in order to address nasal problems as early as possible with a new extra-oral type of nasal molding device, "forehead type of nasoalveolar molding device". Extra-oral strapping was applied to approximate lip segments and also help to reduce alveolar cleft rapidly. The remaining alveolar cleft was reduced till completely, using alveolar mol

Primary (early) alveolar bone grafting.

Early primary bone grafting is successful when performed as a separate operative procedure after lip repair and before palate closure. It must be done in conjunction with neonatal orthopedics. The graft is placed only after the alveolar segments have molded and grown to a butt joint. Minimal soft-tissue dissection of the alveolus and maxilla are performed. The graft stabilizes arch form, allows for tooth migration and eruption through its site, and decreases anterior and posterior crossbite. There is no facial growth attenuation, and a good foundation for further soft-tissue growth is provided

[Palatal groove in premature newborns].

Prolonged intubation for very low birth weight neonates induces the formation of a palatal groove. Intraoral appliances are used to prevent this complication. However, it still exists inhalation and ingestion risks when an impression is taken. Irritation of the skin of the upper lip when using a surgical tape to secure the tube is usual. This article deals with modifications brought to avoid such disadvantages.

[Joint treatment of patients with clefts by orthodontists and logopedists].

Close cooperation between speech pathologist and orthodontist in management of CLP cases is described. Preoperative treatment aims at maintenance of initially favorable maxillary relations and compensation for imbalance of the oral musculature. Knowledge of growth and speech development is decisive for timing of primary surgery (lip about 6 months, soft palate about 18 months, hard palate 5 to 7 years). Speech assessment starts at age 2-2 1/2, followed by team controls once or twice a year with proper coaching of parents. Actual speech therapy is not stared before age 5. The approach described

Health-related quality of life in children with Robin sequence following Tübingen palatal plate treatment in infancy.

OBJECTIVE: Children with Robin sequence (RS) are characterised by micrognathia/retrognathia, glossoptosis and upper airway obstruction, often resulting in hypoxemic episodes and growth failure, potentially impairing their health-related quality of life (HRQoL). Few studies, however, have yet tried to objectively quantify HRQoL in children with RS. The aim of this cohort study was to investigate HRQoL in children with RS 7-11 years after treatment with the Tübingen palatal plate (TPP) in infancy. METHODS: Using two generic health-related QoL-questionnaires from the parental and child perspecti

Fully digital workflow for presurgical orthodontic plate in cleft lip and palate patients.

OBJECTIVE: In most cases, according to our treatment concept, a presurgical orthodontic treatment (POT) is performed on patients with cleft lip and palate (CLP). The aim of this case report is to demonstrate a completely digital workflow for the production of a palate plate. MATERIALS AND METHODS: For the assessment of the maxillary arch, a digital impression of the jaw was made on two patients with an intraoral scanner (Cerec Omnicam Ortho). After reconstruction of a virtual model from the scan data, appropriate areas of the jaw could be blocked out and a plate constructed. This was printed
