Volar Plate Fixation Versus Plaster Immobilization in Acceptably Reduced Extra-Articular Distal Radial Fractures: A Multicenter Randomized Controlled Trial.

BACKGROUND: There is no consensus as to whether displaced extra-articular distal radial fractures should be treated operatively or nonoperatively. We compared the outcomes of open reduction and volar plate fixation with closed reduction and plaster immobilization in adults with an acceptably reduced extra-articular distal radial fracture. METHODS: In this multicenter randomized controlled trial, patients 18 to 75 years old with an acceptably reduced extra-articular distal radial fracture were randomly assigned to open reduction and volar plate fixation or plaster immobilization. The primary o

Locking distal radius plate--early results from India.

BACKGROUND: Restoration of wrist function to close to preinjury levels of patients with intra-articular distal end radius fractures is of concern. Open reduction and internal fixation with angular stable screw fixation implants is coming in vogue but little literature evidence supports it. The objectives of this study are to assess the ability of volar locking plates to maintain fracture reduction when used to treat dorsally displaced intra-articular distal radial fractures and to assess the patient-related outcome after this procedure. METHODS: In a prospective study from March 2008 to Septe

[Is use of the fixateur externe no longer indicated for the treatment of unstable radial fracture in the elderly?].

BACKGROUND: In recent years there has been an increasingly marked shift in the operative treatment of unstable fractures of the distal radius. The introduction of locking compression plates has made it possible to extend the indications for palmar stabilisation according to the principles of internal fixation to extension fractures even in osteoporotic bone, and since then the new design has been used more and more widely. First clinical results show very good and good clinical and radiological outcomes in over 80% of cases after locking compression plate osteosynthesis. All this raises the qu

Surgery to remove the plate from the distal radius with simultaneous reconstruction of the lacerated flexor pollicis longus tendon performed under WALANT: A case report.

The tendon rupture was caused by the protruded distal edge of the palmar plate inserted for fixation of the distal radial fracture 2 years earlier. The patient was a 28-year-old patient who had stopped bending the distal phalanx of his right thumb 2 months prior to the presentation. Performance of anaesthesia and operation is presented in details. The operation went smoothly, bleeding was minimal despite the relatively extensive wound, and the patient felt only little pain at the end of the operation.

Radiographic Appearance of the Fore Digit and Carpal Joint in the Mule Foal from Birth to 3 Months of Age.

Few studies have established the normal radiographic anatomical development of the donkey foal and, to date, no data are available for mules. Our aim was thus to evaluate the radiographic development of the fore digit and carpal joint in the mule foal from 0 to 3 months of age. Ten forelimbs of five healthy full-term mule foals were included. Radiographs of the fore digit lateromedial and dorsopalmar and the carpus dorsopalmar were performed weekly for the first month of age, and bi-monthly during the following two months. Fore digit growth plate closure times, morphological, angular, and line

[Fractures of the distal radius in patients over 70 years old: Volar plates or K-wires?].

In a growing elderly osteoporotic population, the management of distal radius fractures remains without consensus as to volar distal plate versus K-wires. The goal of this retrospective study was to evaluate these treatments in elderly people. In a series of 38 patients over 70 years, 21 were treated by a volar plate and 17 by percutaneous K-wire fixation. Follow-up was at least 6 months. Results were analyzed using the disabilities of the arm, shoulder and hand (DASH), patient-rated wrist evaluation (PRWE) and Herzberg score by an independent operator. Radiological parameters were radio-ulnar

Surgical management of septic metacarpal physitis and concurrent serofibrinous fetlock arthritis: 2 cases.

The clinical, ultrasonographic, radiographic, cytologic and bacteriologic findings, diagnosis and surgical treatment of two heifers with septic metacarpal physitis (type-1 osteomyelitis) and concurrent serofibrinous arthritis of the metacarpophalangeal (MCP) joint are described. Osteomyelitis likely occurred by haematogenous spread following bronchopneumonia in one heifer and developed post-traumatically in the other. In both patients, ultrasonographic examination using the 7.5 MHz linear probe showed moderate effusion of the palmar and dorsal MCP joint pouches and highly irre-gular bone conto

A new treatment for parrot beak deformity of the toe.

Two cases of congenital parrot beak deformity of the toe were treated by pushing back the nail plate, nailbed, matrix, and proximal skin fold in one piece as a flap. The proximal skin portion of this flap was deepithelialized to facilitate this shift, and thus no dog-ear deformity was produced. The distal skin defect of the pulp was covered by a palmar advancement flap. This method does not require augmentation of the fingertip or toetip and is very useful for correcting parrot beak deformities of the toes.

Open reduction and internal fixation of intra articular and unstable fractures of the distal radius using the AO distal radius plate.

This study reports the results of open reduction and internal fixation of 25 dorsally displaced distal radial fractures using a specifically designed plate for the distal radius, the AO pi plate (Synthes Ltd, Paoli, USA). Twenty-one of these fractures were complex and intra-articular (AO Type 'C'). Measurement of range of motion of the affected wrist at an average follow-up of 16 months revealed a median return of 60 degrees of wrist extension, 40 degrees of wrist flexion, 90 degrees of pronation and 90 degrees of supination. Radiographic assessment revealed restoration of normal radial length

Effect of distal screw number on axial deformation and fragment-specific stability in AO/OTA 2R3 C2.1 distal radius fractures.

PURPOSE: To investigate the biomechanical influence of distal screw number in palmar plate fixation of unstable intra-articular distal radius fractures with radiodorsal metaphyseal comminution. METHODS: Nine matched pairs of cryopreserved human radii were prepared using a standardized AO/OTA 2R3 C2.1 fracture model with a defined radiodorsal metaphyseal defect zone. For each pair, specimens were randomized to palmar fixation using a 2.4-mm variable-angle locking plate with either four or six distal screws. After embedding, constructs were subjected to cyclic axial loading (150 N, 5,000 cycles…

[Distal radius fracture of the adult : Diagnostics and therapy].

Fractures of the distal radius are very common. The majority of patients are elderly females. High impact trauma are often responsible for fractures in young men. Clinical and radiological diagnostics, including computer-assisted tomography (CAT) scan, are generally sufficient. The indication for conservative treatment is still recommended for specific fracture patterns. Application of palmar locking plates after open reduction proved to be efficacious for the majority of fracture patterns. Furthermore, precise detection and treatment of concomitant lesions are mandatory in order to prevent co

Minimally invasive plate osteosynthesis for distal radius fractures.

BACKGROUND: Fractures of distal radius are common injury in all age groups. Cast treatment with or without close reduction is a viable option. However, the results are often unsatisfactory with restricted function. The open reduction and internal fixation often results in extensive soft tissue dissection and associated high rates of infect and delayed/nonunion. The distractor/external fixator have reported good functional and anatomical results but the incidence of pin traction infection nerve injury and cosmedic deformity are high. We introduced a modified operative technique for minimally in

Dorsal screw penetration following implant removal after volar locked plating of distal radius fracture.

Complications while removing implants are quite common. In rare cases screw shanks must be left in situ. We present a case of a 38-year-old patient who was treated using a palmar locking plate for a distal radius fracture. After implant removal, one of the screw shanks left in situ penetrated the dorsal cortex of the distal radius into the third extensor tendon compartment and led to irritation of the extensor pollicis longus tendon. We report the unexpected complication and analysis of possible errors of this case to avoid this kind of complication.

[Fresh injuries of the tractus intermedius of the extensor aponeurosis. Diagnosis, classification and therapy].

A test for early diagnosis of closed isolated ruptures of the central slip of the extensor hood is described. Five different lesions are described. Conservative treatment is adequate for ruptures with undislocated bone fragments. Operative treatment is required when the central slip has been avulsed with a dislocated bone fragment, in cases of combined lesions of the PIP joint (including central slip, collateral ligaments and/or the palmar plate) and finally open lesions. An isolated closed rupture of the central slip is a relative indication for surgery and is treated conservatively under spe…

[Functional results following surgical management of palmar fibrocartilagenous injuries].

Lesions of the palmar plate of PIP-joints were treated surgically during the past ten years in cases of dislocation of the PIP-joint, lesions to the collateral ligaments, or lesions to the extensor apparatus (according to types 3, 4 and 5 of Hintringer's classification). A followup of 97 patients showed nearly 90% good results in cases of ligamentous injuries, comparable to results after conservative treatment in other centres. In cases of fracture dislocation and open injuries, results were only fair. Conservative treatment is now preferred in most cases.

Analysis of Plate Position and Factors Associated with Time to Flexor Tendon Rupture Following Volar Plate Fixation of Distal Radius Fractures.

Background: Distal radius fractures are the most common fractures in the upper extremity. Volar plate fixation is a commonly performed surgical procedure for this fracture. Flexor tendon rupture is a serious postoperative complication due to attrition between the plate and tendon. This study aimed to analyse the factors associated with the time from surgery to tendon rupture, classify plate position and determine the incidence of flexor tendon rupture in relation to plate position. Methods: This multicentre retrospective study included 28 patients (24 women) with flexor tendon rupture followin

Repair of Pronator Quadratus With Partial Muscle Split and Distal Transfer for Volar Plating of Distal Radius Fractures.

Flexor tendon injury is a rare, but serious, complication after volar plate fixation for distal radius fractures. The plate position and prominence at the watershed line are contributing factors that cause flexor tendon injury. With the standard volar approach, the pronator quadratus (PQ) is typically elevated off the radial attachment. The distal part of the plate is often visible after repair of the PQ. We describe a "PQ-splitting" technique for covering the distal edge of the plate if primary PQ repair cannot completely cover the distal part of the plate. We also report the outcome of our s

Orthogonal plating of intra-articular distal radius fractures with an associated radial column fracture via a single volar approach.

INTRODUCTION: The purpose of this study was to investigate the radiographic and functional outcome of orthogonal plating (two plates at right angles) via a single volar approach for fixation of intra-articular distal radius fractures with an associated radial column fracture. METHODS: In a retrospective, chart-based review, we identified 14 consecutive patients with an intra-articular distal radial fracture who had been treated with locked volar plate fixation and an additional radial column plate. Radial column plates were LCP Distal Radius Plates 2.4; volar plates were LCP Distal Radius Pla

Salter-Harris type III dorsal fracture-dislocation of the second and third proximal interphalangeal joints: a case report.

BACKGROUND: Paediatric fracture-dislocation of the second (2nd) or third (3rd) proximal interphalangeal (PIP) joint is a rare injury and often associated with growth plate injury. There are few case reports regarding this uncommon injury, and no study has reported on Salter-Harris (SH) -type III injuries. CASE PRESENTATION: We present the case of a 6-year-old softball player who suffered from irreducible SH-type III fracture-dislocations of the 2nd and 3rd PIP joints at different time points, which were treated by open reduction without internal fixation. The injury mechanism for the fracture
