We describe a rare case of intermittent dysfunction of a bileaflet mechanical aortic valve prosthesis. Subvalvular pannus overgrowth on the inflow aspect of the prosthesis impeded normal closure of one leaflet causing severe aortic regurgitation, which occurred intermittently. The complementary role of transthoracic and transesophageal echocardiography and cine-fluoroscopy in the diagnosis of this case is described.

Antigen from a Chlamydia trachomatis biovar, mouse pneumonitis agent (MoPn), was used to produce an inflammatory arthritis by inoculation in the knee joints of C57B1/6 mice. Panniculus morbidus (PM) is a presentation of severe chronic abdominal lymphoedema associated with obesity resulting in oedema and chronic fibrosis. It is a multifaceted condition with significant clinical and psychosocial implications. A case of heparin-induced cutaneous necrosis resembling necrotizing fasciitis occurred in a post-cesarean section patient. She presented with massive necrosis of the skin and abdominal pannus on the sixth postoperative day after a cesarean section for severe preeclampsia and failed induction. A biopsy showed findings characteristic of heparin-induced cutaneous necrosis, permitting conservative therapy.

Serefeddin Sabuncuoglu wrote Cerrahiyyetü'l Haniyye (Imperial Surgery) in 1465. It was the first illustrated surgical textbook in the Turkish-Islamic medical literature. Only three handwritten copies exist. Cerrahiyyetü'l Haniyye consists of three chapters (412 pp.) covering a wide range of surgical specialties, including medical and surgical management of such ophthalmic diseases as chalazion, symblepharon, lymphangiectasia, pannus, proptosis, and hypopyon.

An autosomal dominant syndrome of mild foveal hypoplasia (visual acuity, 20/50), congenital nystagmus, corneal pannus, and presenile cataracts was seen in four patients from a single family. There is variable expressivitly of this trait. The differential diagnosis includes aniridia, ocular albinism, and isolated foveal hypoplasia. Cataract surgery is indicated for affected patients with substantial visual impairment.

The Freestyle stentless bioprosthesis (Freestyle valve) is widely used throughout the world because of its durability and benefits for hemodynamic function. However, we encountered a patient who required reoperation for a cuspal tear 5 years after the implantation of a Freestyle valve. The histological findings suggested that tissue degeneration with pannus formation might play a critical role in cuspal tears.

The patient was affected by a lepromatous leprosy and had a unilateral oedema of the posterior layers of the cornea with a conjonctival hyperhemia. The authors review the various aspects of corneal changes in leprosy: -- paralytic and non specific changes in trigeminal paralysis; -- direct and specific changes: opacification of corneal nerves, a vascular keratitis, corneal leproma, disorders of the vascularisation either by pannus or by interstitial vascularisation. Rheumatoid arthritis is associated with several pathologic changes in the cervical spine, including loss of articular cartilage, ligamentous destruction, and bone erosion. These changes may lead to one or a combination of complications, including atlantoaxial subluxation, cranial settling, subaxial cervical subluxation, and periodontoid pannus formation. Surgical management of these problems should focus on relieving pain, stabilizing the spine, and decompressing neural elements.

We have observed seven patients with longstanding complaints of ocular irritation who had blepharitis and/or meibomitis associated with corneal involvement. Corneal lesions varied from severe pannus formation, destruction of the limbal structure to marginal infiltrates and peripheral corneal melting. The corneal lesions did not regress until blepharitis was treated. The pathogenesis of chronic blepharitis with corneal involvement is discussed.

Panniculus morbidus is a rare end stage complication of abdominal obesity characterized by excess abdominal skin and subcutaneous tissue induced by severe lymphedema. The aim of the study was to define clinical predictors of magnetic resonance imaging (MRI) findings of temporomandibular joint (TMJ) involvement in juvenile idiopathic arthritis (JIA). Forty-six patients, aged 2.08-36.7 years, with JIA (oligoartitular 18, polyarticular 17, systemic type 11) were examined with standard plain and contrast-enhanced sequences.
