A case of multiple endocrine neoplasia: hyperparathyroidism, insulinoma, GRF-oma, hypercalcitoninaemia and intractable peptic ulceration.

A 42-year-old woman with a family history of multiple endocrine neoplasia type 1 (MEN 1) presented with symptomatic hypoglycaemia and peptic ulceration. Investigation revealed an insulinoma, hyperparathyroidism, hypercalcitoninaemia with a positive pentagastrin stimulation test, acromegaly due to a GRF-oma, hyperprolactinaemia and normal serum gastrin levels. Five pancreatic tumours were removed at laparotomy and immunostaining was positive for insulin, calcitonin, somatostatin and glucagon. Post-operatively she developed elevated serum gastrin levels and gross peptic ulceration, despite H2-bl…

Inhibitory effect of isoprenaline on gastric acid secretion in the rat. The role of endogenous histamine.

In dogs beta-adrenoreceptor agonists inhibit gastric acid secretion stimulated by exogenous gastrin to a much greater extent than acid secretion stimulated by exogenous histamine. One possible explanation for this observation is that endogenous histamine is important in gastrin-mediated acid secretion and that isoprenaline and related beta-adrenoreceptor agonists block gastric mucosal histamine release. This possibility was tested in the present study in gastric lumen-perfused anaesthetized rats. Intravenous infusion of isoprenaline (12 microgram kg-1 h-1) inhibited maximal, pentagastrin-stimu…

Congenital hypertrophic pyloric stenosis: A gastrin hypothesis pursued.

Pentagastrin and acetyl choline were shown to contract isolated strips of the circular pyloric muscle from baboons. These findings are compatible with the hypothesis that gastrin may have a part to play in the development or maintenance of congenital hypertrophiic pyloric stenosis of infancy.

Primary hyperparathyroidism as the leading symptom in a patient with a Y791F RET mutation.

Primary hyperparathyroidism (PHP; serum calcium 2.75 mmol/L, PTH 226 pg/ml) had been the first clinical manifestation of MEN-2A in a female patient (aged 55 years) with a mutation (Y791F, TAT-->TTT) in exon 13 of the RET proto-oncogene. The patient has a pentagastrin-induced rise in serum calcitonin (up to 57 pg/ml) considered normal for noncarriers but abnormal in family members of MEN-2 patients. This is the first case of MEN-2 due to this specific mutation with primary hyperparathyroidism as the first manifestation of the disease. In addition, the patient harbored, within the Menin gene, a …

Release of somatostatin, neurotensin and vasoactive intestinal peptide upon inhibition of gastric acid secretion by duodenal acid and hyperosmolal solutions in the conscious rat.

The inhibitory effect of duodenal exposure to acid and hyperosmolal solutions on pentagastrin-stimulated gastric acid secretion was studied in conscious rats equipped with chronic gastric fistula and duodenal Thiry-Vella loop. The loop was challenged with saline, HCl or hyperosmolal polyethylene glycol. Gastric acid secretion was measured in samples from the gastric fistula. Gut peptide concentrations were measured in duodenal perfusates collected each 30 min, and in plasma samples collected both during stimulated acid secretion alone, and at the end of experiments in combination with luminal …

The inhibitory influence of the pyloric antrum on gastric secretion in dogs.

The effect of the pyloric antrum and of subsequent vagotomy on the secretory responses of an innervated or a denervated gastric pouch in the dog to histamine and pentagastrin was determined. The increased sensitivity of the gastric pouch to stimulation after these procedures is in keeping with the concept that an inhibitory reflex is concerned with the control of acid gastric secretion.

[Myoelectric activity of the rabbit stomach and duodenum following neurogenic and humoral effects].

Electrical activity of the duodenum increased after the immobilisation or haemorrhage stress to a greater extent than the stomach and pylorus myoelectrical activity. Pentagastrin or serotonine increased the electrical activity of the duodenum and decreased the stomach and pylorus myoelectrical activity. Chronophysiological mechanism of the gastro-duodenal discoordination is analysed.

Cerebroventricular bombesin inhibits gastric acid secretion in dogs.

Bolus injections of bombesin into a lateral cerebral ventricle of conscious beagle dogs inhibited pentagastrin-stimulated gastric acid secretion from both the innervated stomach (gastric fistula) and the vagally denervated Heidenhain pouch in a dose dependent manner. Bombesin (300 ng/kg), injected into the lateral ventricle, inhibited acid secretion from the gastric fistula and Heidenhain pouch by 66% +/- 5% and 93% +/- 4%, respectively. Central administration of bombesin also suppressed acid secretion in response to a meal by 52% +/- 15%; postprandial gastrin response was not affected. By con…

Effect of PACAP on gastric acid secretion in rats.

Pituitary adenylate cyclase-activating polypeptide (PACAP) is a new VIP-like brain-gut peptide. Its effects on the motility and secretory functions of the gastrointestinal system have been shown in previous studies. In this study we investigated the effect of intravenous PACAP on gastric acid secretion in conscious pylorus-ligated rats and in gastric fistula rats. PACAP showed significant inhibitory effects on pentagastrin- and histamine-stimulated gastric acid secretion, but no effect on basal or carbachol-stimulated secretion in pylorus-ligated rats. It did show dose-related inhibitory effec…

The effects of benzodiazepines on cholecystokinin (CCK) responses produced following activation of CCKB receptors by pentagastrin in the ventromedial hypothalamus (VMH) or CCKA receptors by CCK-8S in the dorsal raphe of the rat brain in vitro have been investigated. 2. The benzodiazepine agonist, flurazepam, at high concentrations, blocked pentagastrin-induced excitations in the rat VMH yielding an equilibrium constant (Ke) value of 12.5 microM. 3. In the rat dorsal raphe, where activation of CCKA receptors leads to neuronal depolarization, flurazepam also produced a weak block of the CCK r…

Symptomatic C-cell hyperplasia associated with chronic lymphocytic thyroiditis.

A 58-year-old euthyroid man with episodic flushing and a 2-year history of progressive wheezing was found to have a hypoechoic lesion in one lobe of his thyroid and hypercalcitoninemia in response to pentagastrin stimulation. Thyroidectomy revealed bilateral C-cell hyperplasia unexpectedly associated with chronic lymphocytic thyroiditis. The C-cells exhibited positive immunohistochemical staining for calcitonin and polyclonal carcinoembryonic antigen (CEA). Postoperatively, the wheezing and flushing subsided and the serum calcitonin level was not elevated with pentagastrin stimulation. The sub…

Effect of alpha and beta adrenoreceptor antagonists on glucagon-induced inhibition of gastric acid secretion in cats.

The aim of the present study was to examine the effect of alpha and beta adrenoceptor blockade on gastric acid secretion, mucosal blood flow (GMBF) and catecholamine content of the gastric mucosa during glucagon-induced inhibition of gastric acid secretion. The secretory response to continuous infusion of pentagastrin (6 micrograms/kg/h) was reduced by regitine (0.5 mg/kg/h) and propranolol (25 micrograms/kg/h). Glucagon (25 ng/kg/h) further slightly decreased HCl secretion. GMBF was also significantly inhibited by regitine and propranolol. Administration of glucagon continued decreasing of th…

The role of substance P in the control of gastric acid secretion.

Experiments were performed to determine whether substance P plays a physiologic role as an enterogastrone. Basal substance P levels averaged 6.4 +/- 0.2 pg/ml in dogs prepared with gastric and duodenal fistulas. Infusion of substance P at 5 ng/kg/min increased circulating hormone concentrations by more than 1 pg/ml and inhibited pentagastrin-stimulated (4 micrograms/kg/hr) gastric acid secretion by 44% (from 10.9 +/- 1.8 to 6.1 +/- 1.6 mEq/30 min); a similar dose of substance P (7 ng/kg/min) did not alter gastrin levels from the basal level of 19.8 +/- 1.2 fmol/ml. Duodenal acidification (with…
