OBJECTIVE: This case report describes the esthetic-functional rehabilitation of a patient with unilateral complete cleft lip and palate who was managed multidisciplinarily from 10 to 18 years of age, and reports the clinical outcome 5 years after definitive prosthetic rehabilitation, which was undertaken after late secondary alveolar bone grafting had yielded insufficient bone volume for implant placement.

INTRODUCTION: Adult patients are seen more frequently in our orthodontic offices. Unlike the adolescent, the adult patient will often need a multidisciplinary approach due to tooth migrations secondary to extractions or periodontal disease.
MATERIALS AND METHODS: This article will address the indications and orthodontic solutions to various restorative problems relative to the adult patient. Specific orthodontic mechanics aimed at facilitating and improving the prosthetic outcomes will be described and illustrated with clinical cases.

Localized severe chronic periodontitis is often seen in middle-aged patients, particularly when plaque control for malaligned teeth is not satisfactory. While orthodontic treatment can realign such teeth with reduced periodontal support, esthetics may be compromised by gingival recession due to preexisting bone loss and resolution of inflammation after periodontal treatment. This clinical report describes how a malaligned maxillary lateral incisor with severe periodontitis was replaced by an immediately placed implant and implant-supported crown with a good esthetic outcome.

Various implant-supported restorations have been used successfully for several decades to rehabilitate edentulous patients. Telescopic crowns are a common treatment modality used to connect dentures to natural teeth. Although previous findings indicate that telescopic crowns can be placed successfully on implants to support overdentures, only limited data are available on this treatment approach.
