A Salter-Harris II Distal Radius Fracture Irreducible by Closed Methods.

CASE: A 10-year-old boy presented with a Salter-Harris II distal radius fracture that was irreducible by closed methods. An open reduction was performed in the operating room where a sleeve of periosteum was found interposed between the fracture fragments. Successful reduction was performed without difficulty after the periosteum was removed from the fracture. CONCLUSION: Soft-tissue interposition must be kept in mind when having difficulty performing closed reductions of pediatric distal radius physeal fractures to avoid excessive reduction attempts.

Function-preserving Surgery for Femoral Sarcoma by "In Situ Preparation" Technique with a Free Flap.

Resection of soft-tissue sarcomas near important tissues (major blood vessels, nerves, bones) is challenging. "In situ preparation" (ISP) technique enables the function of the affected limb to be maintained by preserving the tissue as much as possible. The technique is based on evaluation of the margin of resection of important tissues near the tumor during surgery. Postoperative fractures are known to frequently occur, however, in cases where bones were preserved and periosteum has been resected by the ISP. We present the case of a 51-year-old woman who required treatment for soft-tissue sarc…

Subperiosteal subtemporal approach: technique and applications.

The authors describe a craniofacial surgical approach that they have used since 1987. Based on existing anatomical continuity between the subtemporal plane and the cranial periosteum, it consists of a direct subperiosteal subtemporal cleavage. Preservation of continuity between the muscle and the periosteum allows the operator to return the temporalis muscle to a normal position at the end of the operation when the scalp is sutured. This approach avoids superficial cleaving of the temporalis muscle or section and reinsertion to the fronto-orbital rim. It also allows access to the upper third o…

The Effect of Round Window Sealants on Delayed Hearing Loss in a Guinea Pig Model of Cochlear Implantation.

AIM: To determine whether the type of material used to seal the cochlea after round window cochlear implantation influences delayed hearing … ectrode array was implanted in guinea pigs via the round window, which was then sealed with muscle, periosteum, or fibrin glue. Auditory brainstem responses (ABRs) to pure tones (2, 8, 16, 24, and 32 kHz) were recorded before surgery and 1, 4, and 12 weeks after surgery, with subjects then euthanized and their cochleae harvested for histological analysis. Lymphatic vessels invade bone in disease but are absent in health and regeneration.

A recent study reported the existence of lymphatic vessels in normal bone and suggested their involvement in bone regeneration after injury. However, this conclusion was based on approaches that do not allow unequivocal identification of the spatial localization of lymphatic endothelial cells (LECs). Here, we employed a Prox1-based genetic tool and a dual-recombinase-mediated LEC-specific labeling system to trace lymphatic vessels with high specificity. We found that LECs are present in the connective tissues, including the periosteum surrounding the bone. However, they do not reside within th…

The periosteal microcirculation in health and disease: An update on clinical significance.

Apart from its nutritive functions, the periosteum critically affects bone regeneration via its stem/osteoprogenitor cell content. Normal healing after bone fractures, trauma-orthopedic interventions and invasive dental procedures is critically linked to the reestablishment of the periosteal microcirculation, but the reconstruction, replacement or repair of lost tissues may also be performed with autologous periosteum. Besides the initiation of cell differentiation during bone repair and remodeling processes, the periosteum together with the endosteum plays significant roles in the pathogenesi…

Emerging functionalized artificial periostea: Diverse origins, multifaceted roles and therapeutic frontiers.

The periosteum, a specialized connective tissue layer enveloping bone surfaces, serves as a critical regulator of bone healing through its integrated neurovascular architecture, stem cell niche properties, and mechanical barrier function. Clinically, traumatic injuries or pathological conditions frequently result in periosteal tearing, avulsion, or complete loss, fundamentally disrupting its biological integrity. In practice, repairing a torn or absent periosteum is difficult, because existing options rarely recreate its coordinated biological cues and protective function. The limited availabi…

Ultrasound features of periosteal abnormalities in children.

Ultrasound is a good diagnostic technique for musculoskeletal system diseases in children. At present, musculoskeletal ultrasound is widely used in diagnosing soft tissue and joint diseases in pediatric patients, but its application for bone and cartilage diseases is limited. However, ultrasound examination is very important for the diagnosis of bone and cartilage diseases in children. Especially for lesions displaying periosteum, ultrasound has unique advantages, but there are few reports. This article summarized the normal, variant and abnormal ultrasound findings of periosteum on the surfac…

Modification of Periosteal Flap as Management of Cerebrospinal Fluid Leakage after Frontal Sinus Fracture Surgery in Moderate Traumatic Brain Injury Patients.

Main management for the frontal sinus fracture is using the pericranial flap. Pericranial flaps based on the supraorbital and supratrochlear vasculature have previously been used with significant success for the separation of intracranial and extracranial spaces after major trauma. Defect closure was modified due to lack of the frontal periosteum; the graft was made from the temporal side of periosteum to make primary periosteal flap longer. Defect closure could be optimum. Evaluation for 6 months showed a significant improvement without major complications. In this article, we propose a new m…

Local Delivery of Exosomes and Antibiotics in Hydroxyapatite-Based Nanocement for Osteomyelitis Management.

The management of bone and joint infections is a formidable challenge in orthopedics and poses a global health concern. While clinical management emphasizes infection prevention and complete eradication, an effective strategy for stabilizing skeletal tissue with adequate soft tissue coverage remains limited. In this study, we have employed a novel approach of using the local delivery of exosomes and antibiotics (rifampicin) using a hydroxyapatite-based nanocement carrier to manage the residual space created during debridement effectively. Additionally, we synthesized a periosteum-guiding antio…

Traumatic diaphyseal bone defects in children.

UNLABELLED: Traumatic bone defects (BD) are rare in children. There are no pediatric series in the literature on this topic. The aim of this … ave a better prognosis in children than in adults. The thicker, more active and richly vascularized periosteum in children is an important prognostic factor. Treatment of BD requires good initial bone stabilization. Reconstruction depends on the integrity of the periosteum. In case of an intact periosteum, bone reconstruction does not seem necessary in young …

Modification of the orbital rim anchorage method for surgical replacement of the gland of the third eyelid in dogs.

Excision of a prolapsed gland of the third eyelid predisposes a dog to develop keratoconjunctivitis sicca later in its lifetime, and replacement, rather than excision, of the gland is the preferred method of treatment. One method of gland replacement involves suturing the prolapsed gland to the periosteum of the ventral orbital rim. As originally described, however, the procedure required that the surgeon make a U-turn with the needle and suture within the confines of the ventral fornix. This report describes modifications to the original technique that eliminate this difficult step and make i…

Management of fractures of the supraorbital rim.

The frequency of supraorbital rim fractures is increasing as incidence of motorcycle and bicycle accidents increases. These fractures are frequently depressed, comminuted, and often extend into the frontal sinus. Open reduction and fixation are the treatment of choice. Adequate debridement and cleansing with preservation of periosteum and blood supply are recommended. Unstable fragments can be fixed to the frontal bone or stabilized by suturing the torn periosteum. Severely comminuted supraorbital frontal sinus fractures may require stabilization via frontal sinus pack or balloon. We feel the …

Limb ischemia-reperfusion differentially affects the periosteal and synovial microcirculation.

BACKGROUND: Joints are privileged compartments that enjoy increased protection against the inflammatory reactions affecting the extremities. We hypothesized that the functional characteristics of the microvasculature would contribute to the differential defensive potential of the synovial membrane. METHODS: We investigated the synovial microcirculatory reactions and compared them with those of the tibial periosteum in response to 60 min of total limb ischemia, followed by 180 min of ischemia-reperfusion (IR) in rats. Carrageenan/kaolin-induced knee monoarthritis, a neutrophil-driven synovial …

Diabetes impairs periosteal progenitor regenerative potential.

Diabetics are at increased risk for fracture, and experience severely impaired skeletal healing characterized by delayed union or nonunion of the bone. The periosteum harbors osteochondral progenitors that can differentiate into chondrocytes and osteoblasts, and this connective tissue layer is required for efficient fracture healing. While bone marrow-derived stromal cells have been studied extensively in the context of diabetic skeletal repair and osteogenesis, the effect of diabetes on the periosteum and its ability to contribute to bone regeneration has not yet been explicitly evaluated. Wi…

The strip gingival autograft technique.

In keeping with the concept of rapid epithelialization of close wound edges, the strip technique was developed to maximize the area of gingival grafting with less trauma to the donor site or the recipient site. An incision is made and a partial-thickness flap is reflected so that stable periosteum is left. The apical mucosal border of the recipient site is sutured to the periosteum. Donor tissues are obtained in 2-mm-wide strips, transferred to the recipient site, and sutured. Dry foil and surgical packing are used to stabilize and protect the site during healing. The donor site is rapidly epi…

Comparison of human stem cells derived from various mesenchymal tissues: superiority of synovium as a cell source.

OBJECTIVE: To compare the properties of human mesenchymal stem cells (MSCs) isolated from bone marrow, synovium, periosteum, skeletal muscle, and adipose tissue. METHODS: Human mesenchymal tissues were obtained from 8 donors during knee surgery for ligament injury. After collagenase digestion or gradient-density separation, nucleated cells were plated at an appropriate density for expansion at the maximum rate without colony-to-colony contact. Yield, expandability, differentiation potential, and epitope profile were compared among MSCs from the 5 different tissue sources. RESULTS: Colony num…

Identification of LRP1+CD13+ human periosteal stem cells that require LRP1 for bone repair.

Human periosteal skeletal stem cells (P-SSCs) are critical for cortical bone maintenance and repair. However, their in vivo identity, molecular characteristics, and specific markers remain unknown. Here, single-cell sequencing revealed human periosteum contains SSC clusters expressing known SSC markers, podoplanin (PDPN) and PDGFRA. Notably, human P-SSCs, but not bone marrow SSCs, selectively expressed identified markers low density lipoprotein receptor-related protein 1 (LRP1) and CD13. These LRP1+CD13+ human P-SSCs were perivascular cells with high osteochondrogenic but minimal adipogenic po…

Role of space provision in regeneration of localized two-wall intrabony defects using periosteal pedicle graft as an autogenous guided tissue membrane.

BACKGROUND: Marginal pedicle periosteum (MPP) has been used as a rigid membrane in guided tissue regeneration (GTR) for osseous defects. The present study aims to study the effect of space provision by an alloplastic graft material in bone defect area (BDA) reduction of 2-wall defects. METHODS: Twenty interproximal intrabony 2-wall defects in healthy non-smoking patients with chronic periodontitis were randomly divided in control (group 1, periosteum alone) and experimental (group 2, periosteum with alloplastic graft material) groups. Measurements of probing depth (PD), clinical attachment le…

A quantitative study of the routes of blood flow to the tibial diaphysis after an osteotomy.

Techniques of tissue exclusion have been used previously in qualitative investigations of the vascularity of long bones, after experimental fracture; we quantified their effects on bone blood flow in rabbits. Thirty-six adult rabbits were divided into three groups in which flow was measured, with the microsphere technique, 1 and 2 weeks after osteotomy. In Group 1, osteotomy of the tibial shaft only was done; in Group 2, osteotomy was done with exclusion of the periosteum and muscle by a silicone rubber sheath; and in Group 3, osteotomy was done with exclusion of the marrow by reaming and inse…
