Medical and pharmacy coverage decision making at the population level.

Medicare is one of the largest health care payers in the United States. As a result, its decisions about coverage have profound implications for patient access to care. In this commentary, the authors describe how Medicare used evidence on heterogeneity of treatment effects to make population-based decisions on health care coverage for implantable cardiac defibrillators. This case is discussed in the context of the rapidly expanding availability of comparative effectiveness research. While there is a potential tension between population-based and patient-centered decision making, the expanded

Living in an older adult community: a pharmacy student's experience.

Interacting with older adults is a daily practice for pharmacists. It is important to understand how medications affect their wellbeing, but there are many other factors that affect quality of life. To truly understand some of the challenges facing older adults, Emily Anastasia, a sixth-year pharmacy student at the University of Rhode Island, moved into South Bay Retirement Living, a senior living community, for an eight-day immersion experience as a special project within one of her advanced pharmacy practice experience rotations. During her stay, she did not attend classes nor leave the faci…

Preparing pharmacy students and pharmacists to provide tobacco cessation counselling.

ISSUES: Tobacco use and abuse is a major health risk for people across the world and is responsible for nearly 500 000 deaths in the USA annually. Currently, the accepted role of pharmacists is the provision of pharmaceutical care, which includes health promotion and prevention of disease. Pharmacists should work collaboratively with other health-care professionals to provide tobacco-cessation counselling to smokers. APPROACH: Recently, in the USA, a curriculum has been developed by faculty at a school of pharmacy and distributed to pharmacy schools across the nation in a train-the-trainer fo…

Missed hypothyroidism diagnosis uncovered by linking laboratory and pharmacy data.

BACKGROUND: Although diagnostic errors are important, they have received less attention than medication errors. Timely follow-up of abnormal laboratory test results represents a critical aspect of the diagnostic process, and failures at this step are a cause of delayed or missed diagnosis, resulting in suboptimal clinical outcomes and malpractice litigation. We linked laboratory and pharmacy databases to (1) explore the potential for linking laboratory and pharmacy databases to uncover diagnostic errors, and (2) determine the frequency of failed follow-up of elevated levels of thyroid-stimulat…

Pharmacy-nursing facsimile systems: set-up considerations and operational issues.

The use of facsimile (fax) machines for the electronic transmission of physician's orders from nursing areas to a hospital pharmacy has become increasingly popular. Although the uses and benefits of fax machines are straightforward in this practice setting, namely a significant decrease in order processing turn around time, the problems that may develop are often not described in the literature. This article describes some of the set-up concerns, operational problems, forms considerations, staff training, paper costs, equipment needs, transmission volume, and equipment maintenance involved wit…

A Review of Medication Errors and the Second Victim in Pediatric Pharmacy.

The concept of the second victim, described as the sense of victimization of health care professionals following the exposure to a traumatic, unanticipated medical error, was first introduced in 2000 by Albert W. Wu. Since then, the concept has gained immense traction and inspired the generation of assistance programs for second victims. With most second victim occurrences resulting from medication errors, pediatric pharmacists are at a high risk of experiencing second victim phenomenon. Second victims may experience both psychological and physical symptoms of distress often akin to post-traum

Pharmacy access became available in California in 2016 and in Colorado in 2017. OBJECTIVE: To characterize how access to contraception products and services in pharmacies changed during the COVID-19 pandemic, including pharmacist pr…
