[Study of 357 cases of phlebitis of the lower extremities in 2 inter-regional samplings. I. Methods, epidemiological data and analysis of etiopathogenetic elements].

Using a specially designed record card, 357 cases of superficial and deep thrombophlebitis and phlebothrombosis of the lower extremities have been investigated. Of these 261 were observed in Piedmont and 96 in the Trentino region. A comparative study of aetiopathogenetic data in the two regions was made possible by means of punched cards and subsequent computer processing. The study was completed by statistical analysis. Apart from the region of birth and residence, sex (prevalently female) and age which in both samples mainly affects subjects aged between 30 and 45, the type of working activi…

Risk factors of phlebitis in patients admitted to the intensive care unit vary according to the duration of catheter dwelling: A post-hoc analysis of the AMOR-VENUS study.

AIM: This study investigated the risk factors of peripheral intravenous catheter (PIVC)-related phlebitis in critically ill patients according to the duration of catheter dwelling. METHODS: This was a post-hoc analysis of the AMOR-VENUS study involving 23 intensive care units (ICUs) in Japan. We included patients aged ≥ 18 admitted to the ICU and had PIVCs inserted during ICU admission. The primary outcome measure was phlebitis, and the risk factors of phlebitis were evaluated based on hazard ratios (HR) and 95% confidence intervals (CI). The duration of catheter dwelling was classified as (i…

Impact of peripherally inserted central venous catheter-associated phlebitis in neonate guided by intracavitary electrocardiogram: A systematic review and meta-analysis of randomised controlled trials.

Because the application of intracavitary electrocardiogram (IC-ECG)-guided peripherally inserted central catheter (PICC) in the treatment of neonates is controversial in terms of phlebitis reduction compared with traditional X-ray positioning technique, a systematical evaluation is needed on the impact of IC-ECG on this common complication following PICC. Literature retrieval was conducted on large databases including PubMed, Google Scholar, Cochrane library, and CNKI. Randomised controlled trials (RTCs) of intracavitary electrocardiogram-guided peripherally inserted central catheter tip place…

Impact of Noradrenaline Administration Dosage on the Occurrence of Peripheral Intravenous Catheter-Related Venous Phlebitis in Critically Ill Patients Using a Time-Dependent Multilevel Cox Regression Model.

Purpose: Peripheral intravenous catheter (PIVC)-administered noradrenaline offers faster treatment for septic shock but risks complications like phlebitis. We aimed to investigate the relationship between the total noradrenaline dose administered via PIVCs and the development of phlebitis by considering the influence of noradrenaline as a time-dependent covariate. Methods: A post hoc analysis was conducted on prospective multicenter cohort data from 23 intensive care units in Japan. The total noradrenaline dose was included as a time-dependent variable in a multilevel Cox regression model, and…

The incidence of phlebitis development of high concentration of continuous amiodarone infusion with in-line filter compared to the low concentration without in-line filter: a retrospective propensity score-matched analysis.

BACKGROUND: Phlebitis-associated peripheral infusion of intravenous amiodarone is common in clinical practice, with an incidence between 5% and 65%. Several factors, including drug concentration, catheter size, and in-line filter used, are significantly associated with phlebitis occurrence. We performed a retrospective propensity score-matched analysis to find out whether in-line filter will reduce the incidence of amiodarone-induced phlebitis (AIP) in high concentration of amiodarone infusion compared to low concentration without in-line filter. METHODS: Clinical records of all patients who …

Time to phlebitis onsite and its predictors among admitted patients with peripheral intravenous cannulas at Debre Markos Comprehensive Specialized Hospital, Northwest Ethiopia, 2024/ 2025: A prospective follow-up study.

INTRODUCTION: Phlebitis due to peripheral intravenous cannulation is a common issue in hospitalized patients which leading to serious complications such as deep vein and pulmonary thromboembolism, and septicemia. Understanding the time to phlebitis onset and its predictors is of clinical importance. However, data on its incidence, contributing factors, and time of onset remain limited in Ethiopia. Given the scarcity of compressive studies conducted in Ethiopia and the critical knowledge gap on the incidence, predictors, and timing of phlebitis onset, this study is significant in advancing clin…

Is the topical application of sesame oil (Sesamum indicum L.) combined with standard care valuable and safe for managing infusion-related phlebitis: Evidence from a systematic review with meta-analysis of randomized controlled trials.

OBJECTIVES: Recent randomized controlled trials (RCTs) have studied the potential effect of the topical use of sesame oil (SO), obtained from the sesame plant seeds (Sesamum indicum L., Pedaliaceae family), in preventing or alleviating the symptoms of infusion-related phlebitis (IRP); nevertheless, their data are inconsistent. Thus, this review sought to qualitatively and quantitatively synthesize data from all available RCTs concerning the effect of the topical administration of SO on managing IRP. METHODS: The online databases were searched up to July 13, 2024. Studies were eligible if they…

Simultaneous occurrence of superficial and deep thrombophlebitis in the lower extremity.

Forty-two consecutive patients diagnosed with superficial phlebitis were seen during a 5-year period. Thirty-five of the 42 patients were outpatients. The diagnosis of superficial phlebitis was made by the presence of palpable subcutaneous cords in the course of the greater saphenous vein or its tributaries in association with tenderness, erythema, and edema. The presence of concurrent deep venous thrombosis (DVT) was assessed by impedance plethysmography in 37 patients, compression venous ultrasonography in 3 patients, and venography in 8 patients. Five of the 42 patients (12%) had DVT. Four …

Midline catheter use for cancer patients receiving 5-FU chemotherapy: prospective study of safety and outcomes.

BACKGROUND: Peripheral intravenous 5-fluorouracil (5-FU) administration often causes phlebitis and necessitates catheter replacement, imposing burdens on both patients and healthcare providers. Insertion of a midline catheter (MLC) into the upper arm with tip positioned in the axillary vein may reduce the incidence of phlebitis. This study evaluated the safety and effectiveness of MLC use for continuous 5-FU infusion in cancer patients. METHODS: This prospective study included patients with cancer requiring at least 4 days of continuous 5-FU infusion. The primary endpoint was the incidence of…

Care bundle for the prevention of peripheral venous catheter blood stream infections at a secondary care university hospital: Implementation and results.

BACKGROUND: Venous catheterization for diagnostic and therapeutic purposes is part of routine hospital practice, as approximately 70% of hospitalized patients have a peripheral venous catheter (PVC). This practice, however, can lead to both local complications, (e.g., chemical, mechanical and infectious phlebitis) and systemic complications (e.g., PVC-related bloodstream infections [PVC-BSIs]). Surveillance data and activities are central to preventing nosocomial infections, phlebitis and improving patient care and safety. The aim of this study was to evaluate the impact of a care bundle on re…

Protective effect of isoliquiritigenin in amiodarone-induced damage of human umbilical vein endothelial cells.

OBJECTIVE: Amiodarone (AM) is a drug commonly used in patients with ventricular arrhythmias. It can damage vascular endothelial cells and easily cause phlebitis. At present, the prevention and treatment of phlebitis induced by the use of AM is not clear due to the lack of corresponding primary research. Isoliquiritigenin (ISL) has an anti-inflammatory effect, but until now, has not been explored much in the field of research in primary care nursing. The purpose of this study is to investigate the efficacy and mechanism of action of ISL in treating phlebitis induced by AM. METHODS: In our stud…

[Role of radioisotopic lymphoscintigraphy in postphlebitic syndrome of the legs].

Post-phlebitis syndrome of the lower limbs is a set of symptoms that follow a state of phlebostasis and phlebolymphostasis with consequent oedema and tissue distress. We submitted 36 patients with post-phlebitis syndrome of the lower limbs to radioisotopic lymphoscintigraphy for the purposes of evaluating the vicariant function of the lymphatic system and the possible role of the lymphatic system in the genesis of dermo-epidermal abnormalities. The average age of the patients was 53.2 years (range: 43-69 years; M:F ratio: 0.50). The control group consisted of 6 healthy subjects. The average du…

[The pathophysiology of post-thrombotic syndrome].

The acute stage of venous thrombosis is characterized by an inflammatory process, therefore the term phlebitis is often erroneously equated with venous thrombosis. In most cases, however, the inflammatory reaction or phlebitis is a secondary phenomenon, associated with the process of fibrinolysis and phlebitis as a primary cause is rare. Postthrombotic sequelae such as edema, varicose veins and ulcers usually are late complications and can take a long time to develop. The primary cause of the postthrombotic syndrome is related to an injured venous wall, including damaged valves or stenosis due…

Increase in venous complications associated with etomidate use during a propofol shortage: an example of clinically important adverse effects related to drug substitution.

Etomidate is a widely used intravenous induction agent that is especially useful for patients at risk for hypotension during anesthesia induction. Side effects limiting its use include adrenocortical suppression, acidosis, myoclonus, venous irritation, and phlebitis. The osmolality of etomidate prepared in propylene glycol appears to play a crucial role in causing phlebitis. The increased use of etomidate during the recent propofol shortage correlated with an increase in reported incidences of postoperative phlebitis and thrombophlebitis at Ochsner Clinic Foundation from October 2009 through A…

Recurrent environmentally triggered thrombophlebitis: a five-year follow-up.

Twenty disabled patients with recurrent intractable nontraumatic phlebitis were studied. The patients were divided into two groups and matched for age and severity. The control group was continued on their standard anticoagulant regime, bed rest and support hose. The other group was placed in an especially designed Environmental Control Unit (ECU) where all air, food and water could be controlled. These patients were taken off all medication and not fed until the leg pain and swelling disappeared, which was four to seven days. The patients then showed specific sensitivities to foods and ambien…
