Triggering essential head tremor with sustained phonation: a clinical phenomenon with potential diagnostic value.

Here we report a clinical phenomenon that we have observed repeatedly in clinical research settings; namely, a triggering and/or exacerbation of head tremor during or immediately following sustained phonation. To our knowledge, it has not been reported previously nor has it been the subject of study. Here we: 1) report the phenomenon, 2) provide several visual illustrations, 3) estimate its prevalence, and 4) analyze its clinical correlates. Head tremor was assessed qualitatively, and scores were assigned pre-, during, and post-sustained phonation using the Tremor Research Group Essential Trem

Relationship between transglottal pressure and fundamental frequency of phonation--study using a rubber model.

The relationship between the change in fundamental frequency of phonation (F0) per unit change of transglottal pressure (dF/dP) was studied using a rubber model of the vocal folds that allows the stiffness and elongation of the vibrating part to be varied flexibly. Changes in elongation and stiffness successfully reproduced the negative relationship between dF/dP and F0 at low F0 seen in humans. The thyroarytenoid muscle appears to work concomitantly with the cricothyroid (CT) muscle to regulate F0, raising F0 by increasing the stiffness of the vibrating part at low F0, that is, when the CT mu

Theoretical analysis of maximum flow declination rate versus maximum area declination rate in phonation.

PURPOSE: Maximum flow declination rate (MFDR) in the glottis is known to correlate strongly with vocal intensity in voicing. This declination, or negative slope on the glottal airflow waveform, is in part attributable to the maximum area declination rate (MADR) and in part to the overall inertia of the air column of the vocal tract (lungs to lips). The purpose of this theoretical study was to show the possible contributions of air inertance and MADR to MFDR. METHOD: A simplified computational model of the kinematics of vocal fold movement was utilized to compute a glottal area function. The g

Adductor spasmodic dysphonia and muscular tension dysphonia: acoustic analysis of sustained phonation and reading.

Acoustic phonatory events were identified in 10 women diagnosed with adductor spasmodic dysphonia (ADSD) and compared to 5 women and 5 men diagnosed with muscle tension dysphonia (MTD). The three acoustic parameters examined during sustained vowel production and reading included phonatory breaks, aperiodicity, and frequency shifts. Intra- and intermeasurer correlations showed high reliability for the measures. Findings indicated that those with ADSD produced a greater number of aberrant acoustic events than those with MTD. The results suggested that: (1) only those with ADSD show evidence of p

Investigating the effects of vertical misalignment and stiffness asymmetry on phonation in a synthetic vocal fold model.

Blunt force trauma to the larynx can cause significant damage, resulting in displaced laryngeal cartilage fractures. Vertical misalignment of the left or right vocal fold (VF) in the inferior-superior direction and scarring of the VF tissue are common outcomes. The influence of inferior-superior VF displacement and VF scarring on phonation was investigated using synthetic, self-oscillating VF models in a physiologically-representative facility. Acoustic, kinematic, and aerodynamic parameters were assessed as a function of inferior-superior vertical displacement and asymmetric VF stiffness. The

Clinical Utility of Repeated Urimal Test of Articulation and Phonation for Patients with Childhood Apraxia of Speech.

Childhood apraxia of speech (CAS) causes inconstant oromotor production. We investigated the clinical efficacy of repeated urimal test of articulation and phonation (U-TAP) in CAS patients. Twenty-eight children were recruited: 19 with CAS and 9 with functional articulation disorder (FAD). Four age-matched typically developing children were also recruited. U-TAP was performed twice repeatedly, and the error rate of consonant accuracy (CA) was measured. Preschool Receptive-Expressive Language Scale (PRES) was also performed. The mean U-TAP CA showed a significant difference between the three gr

Respiratory kinematics and the regulation of subglottic pressure for phonation of pitch jumps - a dynamic MRI study.

The respiratory system is a central part of voice production as it contributes to the generation of subglottic pressure, which has an impact on voice parameters including fundamental frequency and sound pressure level. Both parameters need to be adjusted precisely during complex phonation tasks such as singing. In particular, the underlying functions of the diaphragm and rib cage in relation to the phonation of pitch jumps are not yet understood in detail. This study aims to analyse respiratory movements during phonation of pitch jumps using dynamic MRI of the lungs. Dynamic images of the brea

Measuring Voice Effects of Vibrato-Free and Ingressive Singing: A Study of Phonation Threshold Pressures.

BACKGROUND: Phonation threshold pressure (PTP), showing the lowest subglottal pressure producing vocal fold vibration, has been found useful for documenting various effects of phonatory conditions. The need for such documentation is relevant also to the teaching of singing, particularly in view of vocal demands raised in some contemporary as well as early music compositions. The aim of the present study was to test the usefulness of PTP measurement for evaluating phonatory effects of vibrato-free and ingressive singing in professional singers. METHODS: PTP was measured at a middle, a high and

The Effect of Pitch and Loudness Auditory Feedback Perturbations on Vocal Quality During Sustained Phonation.

OBJECTIVE: Dysphonia is a reduction in vocal quality that impacts communication and is often an early sign of a voice disorder. There is little information regarding the effects of auditory feedback control of loudness and pitch on voice quality. In this study, we used both loudness-shift and pitch-shift paradigms to study the relationship between auditory feedback control and vocal quality as measured by smoothed cepstral peak prominence (CPPS), which reflects the harmonicity of the voice signal. STUDY DESIGN: Experimental, mixed design. METHODS: We applied 200 ms loudness-shifts (± 0, 3, o

Botulinum toxin type A: an effective treatment to restore phonation in laryngectomized patients unable to voice.

We evaluated the efficacy of Botulinum toxin type A (BTXA) as an alternative to surgical intervention to facilitate phonation in 34 laryngectomized patients (31 males and 3 women) who were unable to produce tracheoesophageal voice because of spasm of the middle and inferior pharyngeal constrictor muscles (PCM). EMG was recorded to confirm activity in these muscles during attempted vocalization. Parapharyngeal nerve block (Carbocaine 2%, 5 cc) was used to demonstrate short-term fluent voice after relaxation of the pharyngeal constrictor muscle. At a later occasion, 100 U of Botox (Allergan) in

Comparison of acceleration and impact stress as possible loading factors in phonation: a computer modeling study.

Impact stress (the impact force divided by the contact area of the vocal folds) has been suspected to be the main traumatizing mechanism in voice production, and the main cause of vocal fold nodules. However, there are also other factors, such as the repetitive acceleration and deceleration, which may traumatize the vocal fold tissues. Using an aeroelastic model of voice production, the present study quantifies the acceleration and impact stress values in relation to lung pressure, fundamental frequency (F0) and prephonatory glottal half-width. Both impact stress and acceleration were found to

The effect of laryngeal nerve stimulation on phonation: a glottographic study using an in vivo canine model.

The present investigation was designed to examine the effect of change in vocal fold mass and stiffness on vocal fold vibration. To do this, the effect of variation in superior laryngeal nerve stimulation (SLNS) and recurrent laryngeal nerve stimulation (RLNS) was studied Photoglottography (PGG), electroglottography (EGG), and subglottic pressure (Psub) were measured in seven mongrel dogs using an in vivo canine model of phonation. The PGG, EGG, and Psub signals were examined at three frequencies (100, 130, and 160 Hz) for SLNS and RLNS, using a constant rate of air flow. Increasing SLNS, whic

Mechanisms of speaker adaptation to semi-occluded vocal tract exercises: Insights from computational phonation modeling.

Semi-occluded vocal tract exercises (SOVTE) are vocal exercises involving partial occlusion in the front part of the vocal tract, often through phonation into a narrow tube. Although SOVTE are widely used in voice therapy and pedagogy, the physiological mechanisms underlying their therapeutic benefits remain unclear. In this computational study, voice production at different vocal fold configurations with and without semi-occlusion was investigated in a three-dimensional model. Semi-occlusion significantly increased phonation threshold pressure under conditions of weak vocal fold adduction. Un

Dynamically Quantifying Vocal Fold Thickness: Effects of Medialization Implant Location on Glottal Shape and Phonation.

Unilateral vocal fold paralysis (UVFP) can lead to significant dysphonia. Medialization thyroplasty type 1 (TT1) is a common surgical intervention aiming at improving vocal quality by optimally positioning the paralyzed fold to generate the necessary vibrations for phonation. Implants are generally placed through the thyroid cartilage in a sedated patient and positioned either underneath the level of the vocal folds (infraglottal medialization or IM) or at the level of the vocal folds (glottal medialization or GM). Using high-speed three-dimensional digital image correlation (3D-DIC) in an ex
