Paradoxıcal effect of body mass index ranges on pedobarographic evaluatıon.

Although there is evidence suggesting that obesity alters plantar pressure distribution, the specific effects of certain body mass index (BMI) ranges on pedobarographic measures in healty individuals has been poorly investigated. A cross-sectional study with 167 healthy participants was conducted to assess plantar pressure changes across 4 BMI ranges: Under/normal weight (NW), overweight (OW), obese (OB), severe obese (SO). Subjects walked on the Footscan® pressure plate at a self-selected speed, and peak plantar pressure (PPP), load rate, and contact area values were recorded. The foot was di

Injury and performance related biomechanical differences between recreational and collegiate runners.

INTRODUCTION: Running related injuries (RRI) are common, but factors contributing to running performance and RRIs are not commonly compared between different types of runners. METHODS: We compared running biomechanics previously linked to RRIs and performance between 27 recreational and 35 collegiate runners. Participants completed 5 overground running trials with their dominant limb striking a force plate, while outfitted with standardised footwear and 3-dimensional motion capture markers. RESULTS: Post hoc comparisons revealed recreational runners had a larger vertical loading rate (194.5

The cold plate as a test of nociceptive behaviors: description and application to the study of chronic neuropathic and inflammatory pain models.

A cold plate apparatus was designed to test the responses of unrestrained rats to low temperature stimulation of the plantar aspect of the paw. At plate temperatures of 10 degrees C and 5 degrees C, rats with either chronic constriction injury (CCI) of the sciatic nerve or complete Freund's adjuvant (CFA) induced inflammation of the hindpaw displayed a stereotyped behavior. Brisk lifts of the treated hindpaw were recorded, while no evidence of other nociceptive behaviors could be discerned. The most consistent responses were obtained with a plate temperature of 5 degrees C in three 5-min testi

Misdiagnosis of Extraskeletal Osteochondroma in the Foot: A Report of 3 Cases.

CASES: Two women presented with newly growing callosities beneath the first and second metatarsal heads, initially believed to reflect gastrocnemius tightness and plantar plate pathology. In another man, swelling at the posterolateral aspect of the heel was mistaken for a Haglund deformity. Subsequent imaging of each patient led to delayed diagnosis of extraskeletal osteochondroma (ESO). Surgical excision resolved symptoms in all 3 with no recurrence over 12 months later. CONCLUSIONS: Whenever bony prominences newly develop in soft tissues of the foot, ESO should be suspected and appropriate …

Quality of Early Union After First Metatarsophalangeal Joint Arthrodesis.

The aim of the present retrospective cohort study was to assess the quality of union and the clinical outcomes in patients who had undergone first metatarsophalangeal joint (MTPJ) fusion using a dorsal plate and plantar lag screw. From March 2011 to December 2012, the clinical and radiographic data of 39 patients (41 feet) who had undergone first MTPJ fusion using a compressive locking plate were retrospectively reviewed. All patients had undergone postoperative computed tomography at 6 weeks postoperatively to assess union. The average metatarsophalangeal angles improved from 23° ± 16° preope

Mechanical comparison of cortical screw fixation versus locking plate fixation in first metatarsal base osteotomy.

The oblique closing base wedge osteotomy has been used for surgical treatment of moderate to severe hallux valgus deformities with an intermetatarsal angle typically greater than 15°. Several postoperative complications have been identified that relate to failure of the fixation construct used to fixate the osteotomy, especially when that construct has been subjected to a vertical load. We performed a mechanical analysis comparing 2 constructs used to fixate oblique osteotomies of the first metatarsal using composite first metatarsals. An oblique base osteotomy was uniformly performed on 40 co

Achieving a neutral hip-to-calcaneus axis in kinematically aligned total knee arthroplasty equalizes coronal hindfoot pressure balance at initial ground contact in the gait cycle.

AIMS: Evaluating plantar pressure distribution and coronal lower limb alignment, including the hindfoot, using the hip-to-calcaneus axis, known as the ground mechanical axis (GA), is valuable in total knee arthroplasty (TKA). This study aimed to compare postoperative changes in plantar pressure distribution and lower limb alignment between mechanically aligned TKA (MA-TKA) and ground kinematically aligned TKA (gKA-TKA), targeting neutral GA. METHODS: After 1:1 propensity score matching, 35 pairs of patients with end-stage osteoarthritis of the knee, who underwent gKA-TKA and MA-TKA with simil

Work and power in hemiplegic cerebral palsy gait.

The purpose of this descriptive study was to quantify the work that is accomplished by major muscle groups of the affected limb of 10 children with spastic hemiplegia secondary to cerebral palsy during walking. Cinematographic film and force-plate data were used in a biomechanical link-segment model to calculate the positive and negative work performed by the muscles around each joint. The results revealed that the ankle plantar flexors produced just over a third of the positive work for the affected limb instead of the normal two thirds. The greatest proportion of positive work was performed

Traumatic lateral dislocation of the great toe fibular sesamoid: case report.

Traumatic dislocation of the hallucal sesamoids is uncommon. This case involves a 17-year-old female driver involved in a head-on collision who sustained traumatic lateral dislocation of the fibular sesamoid associated with intersesamoidal ligament disruption, partial plantar plate avulsion, and impaction fracture of the metatarsal head. The diagnosis was delayed due to incorrect interpretation of initial radiographs. In addition, the severity of the soft-tissue injury was not appreciated, possibly further delaying the diagnosis. The patient was treated with open reduction of the fibular sesam…

Excellent Reliability for an Instrumented Test of Ankle Plantarflexion Force.

BACKGROUND/PURPOSE: The assessment of ankle plantarflexion force is commonly required in athletic performance and clinical rehabilitation settings to assess the integrity of the calf and lower limb musculature. The force generating capacity of the soleus muscle is thought to be important in many aspects of sporting and everyday function. Unfortunately, there are only a few reliable tests describing the assessment of the strength of the soleus muscle, especially in dorsiflexion greater than plantar grade/neutral which mimics ankle joint positions associated with higher ground contact forces. Evaluation of therapeutic benefits of botulinum toxin for foot dystonia associated with Parkinson's disease.

BACKGROUND: Foot dystonia occurs in patients with Parkinson's disease (PD) and leads to pain, malformation, and difficulty with walking. Botulinum toxin injections may be effective for foot dystonia, but the extent of improvement and effects on motor function are unclear. METHODS: In this study, we performed botulinum toxin injections for foot dystonia in 25 patients with PD. At 3 weeks and 3 months post-infection, we assessed changes in plantar pressure distribution utilizing the Pressure Plate system; dystonia using the Modified Ashworth Spasm score; pain using the visual analog scale (VAS)

Ankle flexor torque, size and density are differential determinants of distal tibia trabecular plate-rod morphometry and bone strength: The Ankle Quality Study.

HYPOTHESIS: Greater peak torque and higher myotendinous density at the ankle are associated with a more plate-like architecture at the distal tibia. METHODS: In this cross-sectional study, women and men ≥ 50 years old with no metal implants, reconstructive surgery, muscular dystrophies, or tendinopathies in any leg were recruited by convenience. Isometric ankle dorsi-plantar flexion and inversion-eversion peak torques were measured using dynamometry. HR-pQCT distal tibia scans were completed. Both assessments were completed on the same day on the non-dominant leg. Integral and trabecular vBMD

Irreducible Dislocation of the Great Toe Interphalangeal Joint Secondary to an Incarcerated Sesamoid: A Retrospective Case Series of Six Patients.

INTRODUCTION: Irreducible interphalangeal (IP) joint dislocation of the hallux is a rare injury, most often caused by interposition of the sesamoid-plantar plate complex. Diagnosis may be delayed due to subtle radiographs, and closed reduction is typically unsuccessful. MATERIALS AND METHODS: This retrospective case series reports six patients (ages 25-60 years, 4 males and 2 females) who sustained dorsal or plantar hallux IP dislocations following sports injuries, falls, or crush trauma during 2 years. Four patients presented acutely, while one presented 3 weeks after injury. Closed reductio…

Orthotic Heel Wedges Do Not Alter Hindfoot Kinematics and Achilles Tendon Force During Level and Inclined Walking in Healthy Individuals.

Conservative treatments such as in-shoe orthotic heel wedges to treat musculoskeletal injuries are not new. However, weak evidence supporting their use in the management of Achilles tendonitis suggests the mechanism by which these heel wedges works remains poorly understood. It was the aim of this study to test the underlying hypothesis that heel wedges can reduce Achilles tendon load. A musculoskeletal modeling approach was used to quantify changes in lower limb mechanics when walking due to the introduction of 12-mm orthotic heel wedges. Nineteen healthy volunteers walked on an inclinable wa
