[CME: Legionella pneumonia].

CME: Legionella pneumonia Abstract. Legionnaire's disease is a usually severe form of pneumonia caused by Legionella pneumophila, a Gram-negative bacterium with an airborne transmission. The infection is acquired in the community, but cases of hospital acquisition from hot water systems have been described. The most common clinical features are cough, fever, gastrointestinal symptoms, hyponatremia and altered liver function tests. The mainstay investigations to confirm diagnosis are urine antigen, sputum polymerase chain reaction, sputum or bronchial alveolar lavage cultures. Standard antibiot…

[Community-acquired pneumonia].

Important new developments in the understanding of CAP encompass new insights into pathogens, risk stratification and antibiotic treatment. Respiratory viruses are increasingly described as relevant pathogens in CAP, such as influenza and rhinoviruses. Risk stratification is central in the management of CAP. In addition to the classical scores blood glucose levels add to the risk prediction. Risk adapted antibiotic treatment is increasingly refined and the use of macrolides in patients with bacteremia or marked inflammation is advocated. Recently developed new antibiotics add to the therapeuti

An atypical pneumonia.

We describe a patient with underlying HIV presenting with progressive respiratory distress and acute renal failure. A unifying diagnosis of microscopic polyangiitis was made. Following immunosuppression induction with plasma exchange and intravenous corticosteroid and subsequent maintenance immunosuppression with intravenous cyclophosphamide in conjunction with renal replacement therapy he achieved remission. To our knowledge this is the first documented case of microscopic polyangiitis occurring in the context of underlying HIV, and raises interesting possibilities regarding the development o

[Cryptogenic organizing pneumonia].

INTRODUCTION: Organizing pneumonia is a particular type of inflammatory reaction of the lung which gives rise to a clinico-pathological syndrome. It is called "secondary" when a cause such as an infection, a drug toxicity, or a connective tissue disease can be identified, or "cryptogenic" when no cause is identified. The clinical picture is usually characterized by the subacute onset of fever, fatigue, cough and dyspnea, with multiple subpleural areas of consolidation on thoracic imaging. STATE OF THE ART: Organizing pneumonia is characterised by the presence of buds of endoalveolar connectiv…

Community-acquired pneumonia.

PURPOSE OF REVIEW: This review examines the epidemiology, diagnosis, prognosis, treatment and prevention of community-acquired pneumonia (CAP) in adults. RECENT FINDINGS: CAP is a significant cause of morbidity and mortality. Streptococcus pneumoniae is the most common CAP pathogen; however, microbial cause varies by geographic location and host factors. Identification of a microbial cause in CAP remains challenging - 30-65% of cases do not have a pathogen isolated. The use of molecular techniques in addition to culture, serology and urinary antigen testing has improved diagnostic yield. Scor…

Electrocardiogram in pneumonia.

Findings on electrocardiogram may hint that pulmonary embolism (PE) is present when interpreted in the proper context and lead to definitive imaging tests. However, it would be useful to know if electrocardiographic (ECG) abnormalities also occur in patients with pneumonia and whether these are similar to ECG changes with PE. The purpose of this investigation was to determine ECG findings in patients with pneumonia. We retrospectively evaluated 62 adults discharged with a diagnosis of pneumonia who had no previous cardiopulmonary disease and had electrocardiogram obtained during hospitalizatio…

'Non-resolving' pneumonia.

A 47-year-old man presented with symptoms of fever and productive cough secondary to a left upper lobe pneumonia. He had received more than three courses of antibiotics over a 2-year period. Review of serial radiographic exams including chest x-ray and CT scans revealed consolidation of the left upper lobe. Lack of response to antibiotics prompted invasive testing with bronchoscopy which revealed a growth in the left main bronchus. Histopathology revealed squamous cell carcinoma.

[Chronic eosinophilic pneumonia].

Chronic eosinophilic pneumonia. The chronic eosinophilic pneumonia is part of Pulmonary Eosinophilic Syndroms. It is presented a 33-years old man, Asmathic, with dry cough, fever, night sweats and fatigue of several weeks. The chest X-ray showed opacity in the right hemithorax. He was treated with antibiotics without response. A chest TC showed multifocal involvement. The patient refused bronchoalveolar lavage (BAL) so treatment antituberculostatic was started. Despite treatment the symptoms worsened. The Chest X-ray showed migration of the infiltrates and the blood smear marked eosinophilia. …

Pneumocystis jirovecii Pneumonia.

Pneumocystis jirovecii has gained attention during the last decade in the context of the AIDS epidemic and the increasing use of cytotoxic and immunosuppressive therapies. This article summarizes current knowledge on biology, pathophysiology, epidemiology, diagnosis, prevention, and treatment of pulmonary P jirovecii infection, with a particular focus on the evolving pathophysiology and epidemiology. Pneumocystis pneumonia still remains a severe opportunistic infection, associated with a high mortality rate.

Pneumonia in Goilala.

The clinical syndrome of pneumonia in adults in Port Moresby, the capital city of Papua New Guinea, has changed from the 1970s to the present. The severe lobar pneumonia commonly diagnosed in young adult men, characteristically from Goilala and living in settlements in Port Moresby, is no longer seen. Today pneumonia in adults is likely to be milder and bronchopneumonic in type. Possible explanations for the change include changes in immunity and in the bacteria found in the environment and carried in the nasopharynx of recent immigrants to the city. A change in treatment-seeking behaviour tog…

[Varicella zoster pneumonia].

Varicella (chickenpox) is a common contagious infection of childhood, with fewer than 2% of the cases occurring in adults. Since the early 1980s the incidence of chickenpox in adults has been increasing and only 7% of them are seronegative for Varicella zoster antibodies. Pneumonia, although rare, is the most common and serious complication of chickenpox infection in adults. The authors present an illustrative case of varicella pneumonia in an immunocompetent adult with smoking habits and make a brief thematic review.

Community-acquired pneumonia.

PURPOSE OF REVIEW: Community-acquired pneumonia (CAP) is a major cause of morbidity, mortality and expenditure of resources. When followed, guidelines for CAP management have been demonstrated to improve clinical outcomes; however, several issues are still open. This review summarizes the recent advances in this field and the priority needs for future research. RECENT FINDINGS: Recently identified clinical and biochemical tools promise to improve the assessment of CAP severity; however, definition of the most accurate and feasible rule(s) for clinical practice is now necessary. Some empirical…

Chronic eosinophilic pneumonia.

Chronic eosinophilic pneumonia is one form of eosinophilic lung disease that includes both idiopathic and known etiological cases. Chronic eosinophilic pneumonia has a characteristic clinical picture, along with a characteristic pattern on CT imaging. It is readily treatable with oral steroids, and although response is prompt with excellent resolution of the symptoms, chronic steroids are often necessary. The mortality from this disease is negligible.

Ventilator-associated pneumonia.

PURPOSE OF REVIEW: This review summarises some of the notable papers on ventilator-associated pneumonia (VAP) from January 2003 to October 2004. RECENT FINDINGS: Ventilator-associated pneumonia remains an important drain on hospital resources. All population groups are affected, but patients with VAP are more likely to be older, sicker, and male, with invasive medical devices in situ. Early VAP diagnosis is desirable to reduce VAP mortality and to retard emergence of multidrug-resistant microbes. This may be possible using preliminary culture results or intracellular organism in polymorphonuc…

[Imaging of pneumonia].

In patients with suspected pneumonia, chest radiography and CT play an important role in making the differential diagnosis and evaluating disease severity and progress. The radiologic patterns of both community-acquired and opportunistic pneumonias were reviewed and compared with those of non-infectious lung diseases. The important findings in the diagnosis of pneumonias were as follows: 1) the relationship between the extent of consolidation and segment or lobe; 2) homogeneity or heterogeneity of opacity; 3) presence of thickening of bronchi and bronchioli; centrilobular, acinar, lobular, and…
