Global report on preterm birth and stillbirth (4 of 7): delivery of interventions.

BACKGROUND: The efficacious interventions identified in the previous article of this report will fail unless they are delivered at high and equitable coverage. This article discusses critical delivery constraints and strategies. BARRIERS TO SCALING UP INTERVENTIONS: Achieving universal coverage entails addressing major barriers at many levels. An overarching constraint is the lack of political will, resulting from the dearth of preterm birth and stillbirth data and the lack of visibility. Other barriers exist at the household and community levels, such as insufficient demand for interventions

Advancing climate-resilient livestock systems: Next-generation emission mitigation strategies and integrated technological innovations.

Livestock production significantly contributes to global greenhouse gas (GHG) emissions, particularly methane (CH₄), nitrous oxide (N₂O), and carbon dioxide (CO₂), posing challenges to climate change mitigation and environmental sustainability. This review explores advanced, system-wide approaches to reduce emissions from livestock systems while enhancing productivity, resilience, and resource efficiency. It covers short-term mitigation strategies such as dietary interventions-including methane inhibitors, microbial modulators, and natural compounds-that target enteric fermentation. Long-term

Fighting COVID-19 in China's Greater Bay Area: how do different policy styles affect policy effectiveness?

BACKGROUND: The COVID-19 pandemic has highlighted how political systems influence public health policy effectiveness. This study examines how different governance styles within China's Greater Bay Area shaped pandemic responses, comparing Guangdong Province, Hong Kong Special Administrative Region (SAR), and Macau SAR as representatives of society-mobilizing, market-facilitated, and targeted-control styles, respectively. This quasi-natural experiment setting controls for cultural and geographic variables while allowing for meaningful institutional comparison. METHODS: Using a mixed-methods ap…

Lived Experiences of Coping With Type 1 Diabetes Among Palestinian Youth: A Qualitative Study.

BACKGROUND: Type 1 diabetes mellitus (T1DM) in adolescents requires continuous self-care and emotional adjustment. Palestinian youth with T1DM face unique challenges within the context of the West Bank healthcare system, where political instability, resource limitations, and cultural factors create additional barriers to optimal diabetes management. These youth experience heightened social stigma, cultural dietary pressures, and restricted access to specialized healthcare services. AIM: To explore the lived experiences, challenges, and coping strategies of Palestinian youth with T1DM in the W

Barriers and facilitators for strengthening primary health systems for person-centred multimorbid care in low-income and middle-income countries: a scoping review.

OBJECTIVE: To understand barriers and facilitators for strengthening health systems for person-centred care of people with multiple long-term conditions-multimorbidity (MLTC-M) at the primary healthcare (PHC) level in low-income and middle-income countries (LMICs). DESIGN: A scoping review. METHODS: We adopted a systematic scoping review approach to chart literature guided by Arksey and O'Malley's methodological framework. The review focused on studies conducted in LMICs' PHC settings from January 2010 to December 2023. Papers were extracted from the following databases: PubMed, EBSCOhost an

Community- and farmer-based management of animal African trypanosomosis in cattle.

Tsetse eradication is impossible in many parts of Africa given environmental, political, and economic circumstances. Animal African trypanosomosis (AAT) control then relies on implementation of local, integrated control strategies by communities or farmers that must take into account the eco-epidemiological context and the cattle rearing system to be sustainable.

Affective Polarization and Mental Health: The Case of South Korea.

CONTEXT: Existing studies find the effects of individual health on political outcomes, but relatively little attention has been paid to the opposite direction--that is, the effects of politics on health particularly in an era of affective polarization. While prior studies have examined both political and non-political consequences of affective polarization, they tend to focus on 'relational' aspects, rather than 'personal' ones. This study addresses this gap by examining mental health as a domain of the personal consequences of affective polarization. METHODS: This study uses public survey da

Evaluating post 2024 election scenarios for the UK based on political party manifestos.

Manifestos provide a vision for political parties to enact if elected to government. While manifestos may not be enacted in full, they provide some of the best information for the public in deciding how to vote. However, manifestos tend to contain both enactable policies (e.g., tax cuts) and outcomes or visions of what these policies may achieve for society (e.g., higher disposal income). These outcomes may be deliberately misleading, inaccurate or only a partial picture of how policies will materialise, as it is unlikely a single policy will influence a single outcome. This work creates a com

Oral health can be a driver of the UK health transformation agenda.

Oral health is a critical yet underutilised lever for health system transformation in the UK. Despite being among the most prevalent noncommunicable diseases (NCDs), oral conditions remain dominated by costly, intervention-based care models. The World Health Organization's Global Oral Health Action Plan (2023-2030) and recent UK health reform agendas present an opportunity to embed oral health within primary care and universal health coverage frameworks. This paper argues for a paradigm shift toward prevention, integration, and equity, addressing shared risk factors across NCDs through team-ba

Medical uncertainty, structural inequities and disease: The experiences of long-COVID among racialized immigrants and racialized non-immigrants in Canada.

Long COVID impacts millions of people worldwide, creating uncertain and varied symptoms that affect a range of bodily systems, with implications for individual and societal health and wellbeing. Yet, since there is no universal standardized way to diagnose long COVID, it has become a highly contested illness, with consequences for the recognition, resources, and responses directed towards the illness. While contested illnesses are known to foster discrimination via medical uncertainty, little is known about the synergistic effects of uncertainty, long COVID, and inequities. Research highlights

The Continuing Challenge of Reducing HIV Risk among Haitian Youth: The Need for Intervention.

There is a dire need for interventions that will address the multiple factors--poverty, substance use, early sexual debut, and violence--that influence Haitian youth's engagement in risky behaviors. The deteriorating socioeconomic and political state of the country has had a deleterious effect on the sociocultural milieu and on the boundaries that have heretofore kept risky behaviors in check. Historically, the lakou system, a community-based approach that supports the family unit, has disintegrated, leading to the disruption of traditional parenting patterns. The unstable economic system has

Central Asian republics: a case study for medical education reform.

Social, political, and economic changes in the former Soviet Union precipitated both the collapse of a once-centralized medical education system in the region and the development of individual models in its place. In the context of rapid globalization and international concerns about health, this development of "nation-based" models for the structure, content, language, and duration of instruction generated concerns about regional accreditation; workforce planning; student qualifications; residency training; continuing education; and infrastructure, such as access to literature, an adequate cl

Building reparative futures demands a reckoning with the crisis form of global health.

Recent cuts to global health funding and US withdrawal from the World Health Organization have generated claims that global health has entered a new crisis. This essay argues that such accounts misread the present moment. The better view is that global health operates through a 'crisis form', a modality of governance in which structural inequality is treated as ordinary, acute disruptions (Big Events) are framed as emergencies, and responses to Big Events restore institutional authority without transforming the historically continuous racialized political economy in which global health is embe

Transforming the health system in Gombe State: accomplishments, challenges and the next frontier.

Nigeria's health reforms aim to accelerate progress toward Universal Health Coverage (UHC). In alignment with these efforts, Gombe State has leveraged national policy frameworks to implement health sector reforms that have improved maternal, newborn, and child health outcomes. To consolidate gains and define future priorities, the State convened the Maiden Gombe State Health Summit in October 2025. The Summit was a two-day hybrid event with over 500 participants from government, development partners, civil society, academia, and the private sector. Data were drawn from rapporteur reports, reco

[State-sponsored doping in GDR elite sport: health consequences, reappraisal, and questions of legal compensation].

Against the backdrop of the ideological rivalry between communism and capitalism, sport in the GDR was exploited for political ends. This was evident, among other things, in a state-directed, controlled, hierarchically organized and, in parts, totalitarian competitive sports system. A key component was a conspiratorial, state-controlled program of compulsory doping, which was systematically implemented from the 1970s onwards and affected an estimated 15,000 underage athletes. Performance-enhancing substances, primarily anabolic-androgenic steroids, were administered covertly, without the knowl

Working towards affordable healthy diets: a review on innovations in food price monitoring, policy and research in Australia and beyond.

Healthy diets are unaffordable for billions of people worldwide, with food prices rising in high-, middle- and low-income nations in recent times. Despite widespread attention to this issue, recent actions taken to inform policy prioritisation and government responses to high food inflation have not been comprehensively synthesised. Our review summarises (i) innovative efforts to monitor national food and healthy diet price, ii) new policy responses adopted by governments to address food inflation and (iii) future research directions to inform new evidence. Evidence synthesis. Global. None. We

Presymptomatic detection of spinal muscular atrophy: Ongoing revolution for a devastating disorder.

Spinal muscular atrophy (SMA) is an autosomal recessive genetic disorder characterized by progressive degeneration of motoneurons, which used to lead in many cases to severe motor impairment and early death without treatment. In recent years, three disease-modifying treatments have dramatically changed the patient outcome, especially when initiated in the presymptomatic phase. This review examines the scientific rationale, practical implications as well as ethical, economic and political considerations of presymptomatic detection of SMA in the era of transformative therapies. The need for earl

Cross-cultural ethnobotany of the Baltis and Shinas in the Kharmang district, Trans-Himalaya India-Pakistan border.

Human communities that inhabit the political borders live a transitional life, which is due to several socio-political and geo-climatic factors. The current cross-cultural ethnomedicinal study was conducted at the highly elevated Pakistan-India borders of the Western Himalayas in order to address the medicinal flora and folk knowledge of Balti and Shina communities. Ethnobotanical field surveys were conducted from May 2021 to September 2022. We used semi-structured interviews in order to collect the ethnomedicinal data. The collected data was analyzed using the relative frequency of the citati

"Together on the Way": Occupational Therapy in Mainstream Education-A Narrative Study of Emerging Practice in Switzerland.

In Switzerland, recent changes in legislation have reformed special needs education; more children with special needs are now integrated into mainstream schools. Health professionals such as occupational therapists are not embedded in the Swiss education system, but pediatric occupational therapists are starting to work at schools, with the aim of enabling children's full participation as school students. This is bringing a change to the practice of pediatric occupational therapists. Cultural, political, and social factors differ in many ways from those of other countries where most of the cur
