[Postural kyphosis and morbus Scheuermann].

Scheuermann's kyphosis is the most frequent structural kyphosis in adolescents. There are gaps in the knowledge of epidemiology, aetiology and treatment. There are strong genetic and mechanical factors in the aetiology. Treatment options depend on the cobb's angle measured and the skeletal maturity. Training and brace treatment yield good results for milder curves, while surgical correction is the most effective for severe curves > 70°. Indications for surgery are subject of debate as complications are not uncommon.

Postural control in healthy individuals.

BACKGROUND: The aim of rehabilitation is to optimize functional recovery and frequently following injury, the uninjured limb is used as measure to compare with the injured limb. This assumes that symmetry existed prior to the injury. The aim of this study was to examine postural control in a healthy sample to ascertain if differences exist between the functionally dominant compared to non-dominant limbs. METHODS: Following ethical approval, 20 healthy active subjects (10 males and 10 females) mean age (SE) of 27.2 (1.4) years, with no previous history of lower-limb injury or pathology volunte

Postural induced-tremor in psychiatry.

Postural tremor is the most common movement disorder in psychiatry, and often a difficult problem for clinicians. It can be classified as physiological, essential, drug-induced, and postural tremor in Parkinson's disease. Drugs used in psychiatry that can produce postural tremor, include lithium, valproic acid, lamotrigine, antidepressants, and neuroleptics. Clinical characteristics of postural tremor induced by each of these drugs are described. Pharmacological strategies for therapy in disabling drug-induced tremor include beta-blockers, primidone, gabapentin, topiramate, and benzodiazepines

The importance of postural control for feeding.

Children with cerebral palsy and other neurodisabilities often have decreased postural control that exacerbates their feeding/swallowing disorders. Correct postural alignment is important in the normal feeding/swallowing process. In the child with cerebral palsy, the alignment and stability of the oral structures for feeding/swallowing may be compromised by abnormal muscle tone and movement patterns. Effective oral functioning for feeding begins with attaining better head stability to improve jaw control. Head control is influenced by trunk alignment, which depends upon the stability of the pe

Postural hypotension in hypertensive patients.

BACKGROUND: The association between orthostatic hypotension and supine hypertension (Hypo-Hyper) has been reported in previous studies on selected populations. The present longitudinal study aimed to characterize the haemodynamic patterns, comorbidity, prognostic implications and eventual effect of therapy in patients with the Hypo-Hyper pattern. METHODS: Ambulatory blood pressure monitoring (AMAP) and clinical evaluation were performed on 615 consecutive patients recruited from the Hypertension Clinics of five Italian Hospitals: 34 patents were identified as Hypo-Hyper, and underwent 10-mont

The neuropathic postural tachycardia syndrome.

BACKGROUND: The postural tachycardia syndrome is a common disorder that is characterized by chronic orthostatic symptoms and a dramatic increase in heart rate on standing, but that does not involve orthostatic hypotension. Several lines of evidence indicate that this disorder may result from sympathetic denervation of the legs. METHODS: We measured norepinephrine spillover (the rate of entry of norepinephrine into the venous circulation) in the arms and legs both before and in response to exposure to three stimuli (the cold pressor test, sodium nitroprusside infusion, and tyramine infusion) i

Postural myoclonus induced by phenytoin.

Involuntary movements are an infrequent complication of treatment with phenytoin and include tremor, asterixis, myoclonus, parkinsonism, and dyskinesias. The mechanism by which phenytoin exerts its actions is unclear. Phenytoin has been observed to exert variable effects on dopamine metabolites and also may induce changes in serotonergic activity. In this report, we discuss the available experimental evidence concerning the possible mechanisms of involuntary movements induced by phenytoin. We describe a case of postural myoclonus during treatment with phenytoin.

Decerebrate posturing in alcoholic coma.

Two cases of alcoholic coma are presented where extensor responses to noxious stimuli are demonstrated. Decerebrate posturing normally indicates severe structural or functional depression of midbrain function but can be caused by depressant drugs. Blood alcohol measurements are a vital test in the comatose patient as the clinical picture may be caused, or temporarily significantly worsened, by severe alcohol intoxication. The preservation of pupillary light reflexes in the presence of deep coma with decerebrate posturing should alert the clinician to a possible metabolic cause for the coma, in

Aging and postural hypotension. An update.

Postural hypotension is a common clinical problem in elderly patients. Reported studies have investigated the epidemiology and mechanisms of this disorder. However, many of these data are based on subjects with diseases and medications also known to cause this disorder, which confuses the relative contributions of normal aging, disease, and medication effects. This review critically evaluates published studies on measurement, epidemiology, and mechanisms of postural hypotension in screened and unscreened elderly populations. Pertinent age-related changes in physiology and the relationship of p

Postural stability and Colles' fracture.

We have tested the hypothesis that the Colles' fracture is due, not simply to bone loss at the menopause, but to postural instability in a subset of postmenopausal women such that they are rendered more liable to fall. We have measured bone mass by dual photon spinal densitometry and single photon wrist densitometry and measured postural sway in 19 postmenopausal women with a history of Colles' fracture. Our results show that not only do Colles' fracture subjects have a small reduction in bone mass but they have a significantly increased degree of postural sway, a finding which has previously

Ventricular arrhythmia with postural dependency.

Ventricular arrhythmias associated with recumbency were noted in a 17-year-old man with mild idiopathic myocarditis. These arrhythmias were refractory to several anti-arrhythmic drugs. Frequent ventricular arrhythmias were reproducibly recorded in the supine position and in a 15 degree tilt from the horizontal, while they were completely abolished in positions tilted more than 30 degrees from the horizontal, at which time sinus rate increased to more than 70 beats/min. The ventricular arrhythmias were suppressed during treadmill exercise and markedly aggravated when the underlying sinus rate w

Congenital postural deformity association.

The so-called congenital postural deformities were evaluated in the material of the Hungarian Congenital Malformation Registry, 1970-1976. Seven categories of postural deformities: dislocation of the hip, clubfoot, torticollis, musculoskeletal, other limb, face-nose and auricular were separated. The occurrence of the combination of dislocation of the hip, clubfoot and torticollis is manifold of random combination. These three postural deformities, however, are not associated more often with other nonpostural type abnormalities. Thus the congenital postural deformity association composed of two

Primate models of postural disorders.

Dorsal column lesions in the high cervical region of the monkey result in severe defects of movements projected into space and contactual orienting reactions of the forelimbs. The hindlimbs are less affected provided a pathway through the lateral columns, Morin's tract, remains intact. Interruption of this pathway results in a defect of hindlimb function similar to that of the forelimbs. Cerebellar ablations in monkeys result in postural and movement disorders, including hypotonia of limb extensor muscles. An important mechanism underlying the hypotonia is a depression of the responses to musc

Postural behavior and motion sickness.

(1) An interesting style of acting was demonstrated on a stage of a Kyogen, a classic comedy of Japan, titled "Funawatashimuko", i.e., "A boatman and a bridgegroom in a boat". Antagonistic postures which move toward the opposite direction were displayed by a boatman who is pulling an oar and a passenger who is being moved by the rolling of a boat. (2) Why doesn't one suffer from motion sickness when he drives a car but may suffer from it when he is a passenger? This question was answered, from the standpoint of human postures, by observing the antagonistic postures exhibited by a bus-driver an…
