Distinct metabolic characteristics and risk of stone recurrence in patients with multiple stones at the first-time presentation.

OBJECTIVE: To characterize the clinical and metabolic abnormalities of patients presenting with multiple stones and determine their risk of new stone formation. MATERIALS AND METHODS: This retrospective case-controlled study consisted of 911 patients who had ureter stones for the first time and 107 age- and sex-matched patients without stones. The patients were classified into 2 groups: those with a single ureter stone (n = 690) and those with 1 or more additional stones somewhere in the ureter or kidney (n = 221). All patients underwent 24-hour urinary metabolic evaluation. The 240 patients

Patient-centered medical therapy for nephrolithiasis.

OBJECTIVE: To elucidate the effect of a patient-centered combined nutritional and medical therapy approach on stone disease management, guided by 24-hour urinary stone risk. MATERIALS AND METHODS: We retrospectively analyzed the records of patients treated at our multidisciplinary stone clinic from July 2007 to February 2009. Included were adult stone formers who presented with severe urinary abnormalities or whose urinary parameters failed to improve with dietary changes. Urinary risk factors for stone disease were evaluated before and after intervention with 24-hour urine collections. Hyper

Role of citrate excretion in acid-base balance in diuretic-induced alkalosis in the rat.

Studies were performed to assess the role of changes in the excretion of citrate, a metabolic precursor of bicarbonate, in acid-base balance in diuretic-induced metabolic alkalosis. Rats on a low-chloride diet with sodium sulfate added were studied during a base-line period, 3 days of furosemide administration, and 4 days post-furosemide. During the period of furosemide administration, net acid excretion and plasma bicarbonate concentration increased. In the post-furosemide period, net acid excretion remained higher than base line but plasma bicarbonate concentration did not increase further.

Bone mineral density and growth changes in patients with distal renal tubular acidosis after two-years treatment with a new alkalizing drug (ADV7103).

BACKGROUND AND OBJECTIVES: ADV7103 is a new prolonged-release treatment for distal renal tubular acidosis (dRTA), containing potassium citrate and potassium bicarbonate. Since acidosis may affect bone mineral contents, the effects of ADV7103 on bone mineral density (BMD) and growth in patients with dRTA over 24 months were evaluated. PATIENTS AND METHODS: Thirty patients (24 paediatric patients and 6 adults) were included in an open-label extension study after a phase II/III trial. BMD, measured by densitometry, was assessed at baseline and at 24 months. Growth was evaluated throughout the st

Tamoxifen treatment of ovariectomized mice alters dopamine release from striatal tissue fragments superfused in vitro.

In this report we examined the effect of tamoxifen upon the nigrostriatal dopaminergic system. Ovariectomized mice were subjected to one of the following treatments: two subcutaneous injections administered on successive days of the sesame oil vehicle (control), estradiol benzoate (EB-10 micrograms), tamoxifen citrate (TMX 125 micrograms) or a combination of EB+TMX. At 24 h after the second injection, the caudate nucleus was superfused in vitro to evaluate the effects of these treatments upon basal as well as potassium stimulated (30 mM) dopamine release rates. In addition, uteri were weighed

Elevated serum levels of Vitamin D in infants with urolithiasis.

INTRODUCTION: The pathophysiology of urolithiasis in infancy is not well known. The aim of this study was to investigate whether infants with urolithiasis have higher serum levels of vitamin D, as a possible risk factor for urolithiasis, compared to infants without urinary calculi. MATERIALS AND METHODS: In this case-control study, 36 infants with urolithiasis (age range, 2.5 to 24 months) were enrolled as well as 36 age- and sex-matched infants without urolithiasis. Random urine samples were tested for calcium, phosphorous, oxalate, citrate, uric acid, sodium, potassium, magnesium, and creat

Medical and dietary therapy for kidney stone prevention.

The prevalence of kidney stone disease is increasing, and newer research is finding that stones are associated with several serious morbidities. These facts suggest that emphasis needs to be placed not only on stone treatment but also stone prevention. However, there is a relative dearth of information on dietary and medical therapies to treat and avoid nephrolithiasis. In addition, studies have shown that there are many misconceptions among both the general community and physicians about how stones should be managed. This article is meant to serve as a review of the current literature on diet

A rare case of congenital distal renal tubular acidosis combined with medullary sponge kidney.

Distal renal tubular acidosis combined with medullary sponge kidney (MSK) is not uncommon in adults, but is rare in infants. We report a 13-month-old boy with MSK who had features of distal renal tubular acidosis (nephrocalcinosis, hypercalciuria, hypocitraturia) and failed to thrive. Renal ultrasound revealed bilateral increased medullary echogenicity and nephrocalcinosis. Bilateral medullary nephrocalcinosis in the ultrasound was the first sign that alerted our pediatrician to the presence of MSK in infants. Earlier treatment may increase efficacy.

An uncommon cause of acute kidney injury in young children: cystinuria.

UNLABELLED: Bilateral obstructive nephrolithiasis is a rare cause of acute kidney injury (AKI) in early childhood. As soon as the identification of AKI secondary to ureteral stone is made, it will necessitate an emergency treatment. PATIENTS: We report three infants with AKI caused by bilateral obstructive ureteral cystine stones. They were diagnosed with acute post-renal injury due to obstructive bilateral ureteral stones based on ultrasound scan findings. Immediately, bilateral ureteral stents were inserted for urinary drainage. Once renal function recovered to normal, each patient underwen

Compounded Effervescent Magnesium for Familial Hypomagnesemia: A Case Report.

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) is a rare autosomal recessive disorder affecting <1/1,000,000 people. It is caused by mutations in the CLDN16 (FHHNC Type 1) or CLDN19 (FHHNC Type 2) genes, which are located on Chromosomes 3q27 and 1p34.2, respectively. There are no drug therapies for this condition. Although magnesium salts represent an important class of compounds and exhibit various therapeutic actions as a supplement for magnesium deficiency in FHHNC, various formulations on the market have different bioavailability. We report the case of a patient w

[Hypercalciuria].

Hypercalciuria is the most common metabolic abnormality that causes urolithiasis. The pathogenetic mechanisms responsible for hypercalciuria include enhanced gastrointestinal absorption of calcium, increased bone resorption and/or decreased renal reabsorption of calcium; the main dietary factors promoting hypercalciuria are high dietary sodium intake and protein-rich diet. The authors discuss pathophysiology of hypercalciuria and genetic factors behind 'idiopathic hypercalciuria'. The simplified diagnostic approach to hypercalciuria is outlined herein, and available therapeutic interventions o

[A case of urolithiasis associated with short bowel syndrome].

We report a case of urolithiasis associated with short bowel syndrome. A 56-year-old woman was admitted to our hospital for asymptomatic bilateral renal stones. She had received extensive resection of small intestine due to strangulating obstructive ileus 7 years ago (residual intestine, only 20 cm). Subsequently, she was in a state of short bowel syndrome. Plain film of kidney, uteter, bladder and computed tomography revealed bilateral renal stones (right 4 mm, left 10 mm). The left renal stone was successfully treated by extracorporeal shock wave lithotripsy. Since the right renal stone was

Development of a diagnostic test for Yersinia pestis by the polymerase chain reaction.

A 501 bp caf1 gene fragment and a 443 bp of pla gene fragment carried by 100 kb (pFra) and 10 kb (pPst) species-specific extrachromosomal replicons, respectively, were used as targets to study the conditions under which DNA amplification by polymerase chain reaction (PCR) may be applied to detect and identify Yersinia pestis DNA in cell lysates of pure cultures and biological samples. The sensitivity limit of PCR with the crude cell lysates of Y. pestis EV was estimated as 10-50 cfu in reaction mixture. When target Y. pestis EV cells were mixed with fresh blood of white mice, which contained 0

Absence of effect of steady state bempedoic acid on cardiac repolarization: Results of a thorough QT/QTc study in healthy volunteers.

Bempedoic acid is an inhibitor of adenosine triphosphate-citrate lyase approved for use in adults with hypercholesterolemia. Nonclinical studies assessed binding to the human ether-a-go-go-related gene (hERG) potassium channel in vitro and the effect of bempedoic acid on QT/QTc in cynomolgus monkeys. A randomized, double-blind, parallel-design clinical study assessed the effects of steady-state bempedoic acid at a supratherapeutic dose (240 mg/day, 33.3% higher the180 mg/day therapeutic dose), placebo, and moxifloxacin (400 mg) in healthy subjects. Effect of increased blood oxygen affinity on work performance of rats.

Influence of altered blood oxygen affinity on maximal performance ability was evaluated in trained rats exercising to exhaustion in a graded treadmill test. Modification of blood oxygen affinity was achieved both by 2,3-diphosphoglycerate depletion, accomplished by exposure of animals to CO(2) and by exchange transfusion with blood exposed to bisulfite or stored in acid citrate dextrose, and by carbamylation of hemoglobin, produced by exchange transfusion of blood incubated with potassium cyanate. A decrease in oxygen tension at half-saturation of hemoglobin (P(50)) from 36 to 23 mm Hg produce

Analysis of synaptic potentials in mitral cells in the isolated turtle olfactory bulb.

1. The synaptic responses of mitral cells have been analysed in intracellular recordings from the isolated olfactory bulb of the turtle. 2. The response of a mitral cell to a single volley in the lateral olfactory tract consisted of and antidromic impulse and a complex hyperpolarizing potential that had the properties of an inhibitory post-synaptic potential. The inhibitory response consisted of two successive components I1 and I2, followed by a prolonged hyperpolarization. 3. High-gain recordings revealed miniature hyperpolarizing potentials during the I1 and I2 responses. Both the miniature

Synchronous Metabolic Interference and Immune Reprogramming by a Self-Assembled Bismuth-Phenolic Nanoparticle against Pseudomonas aeruginosa-Induced Infections.

Effective eradication of Pseudomonas aeruginosa (P. aeruginosa) infection remains challenging due to its multifaceted antibiotic resistance mechanisms and biofilm-associated protection. Furthermore, inflammatory dysregulation within the infected microenvironment compromises both antibacterial efficacy and tissue repair. Therapeutic strategies capable of simultaneously eliminating pathogens and restoring microenvironmental homeostasis are therefore highly desirable. Herein, we report a bismuth-phenolic nanoparticle (BAQ NP) for synergistic antibacterial and immunomodulatory therapy of bacterial

Fanconi syndrome and CKD in a patient with paroxysmal nocturnal hemoglobinuria and hemosiderosis.

Fanconi syndrome and chronic kidney disease associated with paroxysmal nocturnal hemoglobinuria is rarely reported. We describe a 51-year-old woman with glomerular filtration rate decrease and hypokalemia, glucosuria, and proteinuria during a 4-year period. Paroxysmal nocturnal hemoglobinuria was diagnosed 17 years earlier, and she has received multiple blood transfusions because of hemolytic episodes during the last 5 years. Deteriorating kidney function and persistent Fanconi syndrome were accompanied by a progressive increase in serum ferritin levels. Laboratory studies showed proximal rena
