[Bifocal contact lenses as a correction method in presbyopia].

A study aimed at assessing the efficiency of presbyopia correction by bifocal contact lenses (BCL) was undertaken; it envisaged a comprehensive evaluation of subjective data provided by patients and measurements of a number of functional parameters of the visual quality for far and near, including mono- and binocular measurements with BCL of different constructions versus a maximal sphero-cylindrical spectacle correction for far and for near. Soft Acuvue Bifocal BCL as well as soft and rigid BCL manufactured in the optical-and-mechanical laboratory of the Research Institute for Eye Disease of …

[Physiopathology in accommodation and presbyopia, clinical and surgical approach].

Accommodation is a complex of phenomena by which the refracting power of the eye changes for focusing on near vision. Accommodative amplitude declines progressively with increasing age. Until today, this phenomenon called presbyopia was explained by Helmholtz's hypothezis which involves only ciliary muscle. It seems that in accommodation are involved, besides the ciliary muscle, the lens and extraventicular elastic components, biophysical dynamics of the lens capsule, vitreous, iris. According with the role of these factors and theirs pathophysiological implications, new surgical technique has…

A complication of scleral expansion surgery for treatment of presbyopia.

PURPOSE: To report a patient who developed complications from an experimental technique using scleral expansion to treat presbyopia. METHODS: Case report of a 46-year-old woman who underwent scleral expansion surgery on her right eye. Postoperatively, the patient developed chronic pain and swelling that necessitated removal of the scleral expanders. RESULTS: After removal of the scleral expanders, the patient demonstrated a -1.4 diopter myopic shift in the right eye relative to her preoperative refraction. Axial length of the right eye was 1.15 mm longer than of the left, whereas preoperativ…

Validity of surveys reporting myopia, astigmatism, and presbyopia.

Validation of surveys is an important step in establishing the usefulness of questionnaires for gathering information in clinical studies. Often in large studies of refractive error, it is not possible to examine and refract all subjects and their relatives. Therefore, we used 3 methods to evaluate 112 consecutive clinic patients' abilities to report myopia through a survey: (1) The Lay Terms Method asks: "are you nearsighted?"; (2) The Direct Method asks: "are you myopic?"; and (3) The Indirect Method uses a series of questions about the use of eyeglasses and age at first dispensing. Patient

Pathophysiology of accommodation and presbyopia. Understanding the clinical implications.

As a consequence of a new understanding of the mechanism of accommodation, a unique surgical procedure has been developed which can reverse presbyopia. The basic pathophysiology involved in the development of presbyopia may have implications in the etiology and treatment of ocular hypertension and primary open angle glaucoma.

[Convergent strabismus in the age of presbyopia (author's transl)].

Based on 15 case reports it can be shown, that the onset of a convergent squint is even possible in the age of presbyopia, independent of the patient's refraction. The clinical picture resembles that of a divergence paralysis or the convergent strabismus of the Bielschowsky type. In 13 cases the decompensation of a previous esophoria may have caused this distance-esodeviation; a divergence paralysis is not probable. In two cases change of orthophoria to esotropia could be observed. Therefore we share the opinion of v. Graefe, that an increased tonicity of the internal recti induced for compens…

Presbyopia and the changes with age in the human crystalline lens.

1. A method of determining the distribution of capsular moulding pressure is described. This includes:(a) measuring the decrease in polar depth of the lens from photographed lens profiles before and after the capsule has been cut around the entire equator of the lens,(b) measuring the elastic properties of the lens substance by subjecting the lens to known centrifugal forces and photographing the polar strain so produced.2. A method of determining the relationship between capsular and lenticular strain is described. This includes (a) measuring the elastic properties of the capsule (b) measurin

Presbyopia and the water content of the human crystalline lens.

1. The water content of the human crystallaline lens nucleus is 63.4% S.D. +/- 2.9%, and cortex 68.6% S.D. +/- 4.3%.2. Neither the total water content of the cortex, nor that of the nucleus show any significant changes with age, so ;sclerosis' of the lens due to loss of water is not a cause of presbyopia.3. The initial loss of water from the nucleus of the lens substance obtained by drying in vacuo at 20 degrees C for 2 hr is related to age (P = 0.05) and deformability (0.02 > P > 0.01).4. The lens fibres of the ageing nucleus have an increased resistance to deformation associated with a decre…

Vergence-Accommodation Conflict: Increased Presbyopia in Virtual Reality.

BACKGROUND: Performance and symptoms in completing a visual search task on a PC monitor and using a head-mounted display (HMD) were compared for different viewing conditions and between users of different ages. PATIENTS AND METHODS: Twenty-three young (M = 30 y, SD = 7 y) and 23 older (M = 52 y, SD = 5 y) participants performed a visual search task presented on a PC monitor. The task was repeated using an HMD for a near and a far virtual viewing distance. Reaction times (RT), detection sensitivity (d'), and symptoms were recorded for the three different viewing conditions. RESULTS: RT and d'

Presbyopia developed earlier during the COVID-19 pandemic.

PURPOSE: The aim of this cohort study was to evaluate the development and progression of presbyopia and the status of dry eye-related symptoms from 2017 to 2020, to assess the impact of the COVID-19 pandemic. METHODS: Near add power at 30 cm was measured in 339 participants aged between 40 and 55 from 2017 to 2021 at Japanese eye clinics. Regression analysis of near add power and age was analyzed to compare 2017 with later years up to the pandemic. The prevalence of dry eye-related signs and six common symptoms were compared. RESULTS: The number and mean age (y) of participants were 183 (48.…

Non-lens-based surgical techniques for presbyopia correction.

INTRODUCTION: Presbyopia is the most prevalent refractive error. With a progressive aging population, its surgical correction is an important challenge for the ophthalmology community, as well as the millions of patients who suffer from it, and who are increasingly demanding alternatives to its correction with glasses or contact lenses. MATERIAL AND METHODS: A review is presented with a synthesised discussion on the pathophysiological theories of presbyopia and an updated and analytical description of the non-lens involvement surgical techniques used to treat presbyopia. RESULTS: Corneal pro…

Refractive enhancement following presbyopia-correcting intraocular lens implantation.

PURPOSE OF REVIEW: Presbyopia-correcting intraocular lenses are widely available. Residual ametropia is one of the most common issues that can result in patient dissatisfaction. Options for correcting refractive surprises include piggyback intraocular lens implantation, corneal incisional surgery and laser correction. Excimer laser surgery is a safe and predictable method to correct residual amounts of ametropia in pseudophakic patients with monofocal intraocular lenses; however, there is scant published literature regarding this technique. RECENT FINDINGS: Presbyopia-correcting intraocular l…

[The necessity of strengthening multispecialty management in presbyopia correction].

Presbyopia is one of the most common visual functional problems in middle-aged and older adults. With the increasing availability of correction options, presbyopia care has evolved from traditional near-vision compensation to comprehensive clinical decision-making that integrates visual quality, risk control, and long-term eye health management. However, the traditional single-specialty model is becoming insufficient to comprehensively address key aspects of presbyopia care, including assessment of visual needs, screening of ocular conditions, identification of systemic risk factors, selection…

Current Trends in Presbyopia Correction-A Comprehensive Review.

Presbyopia is a physiological phenomenon and one of the leading factors contributing to decreased near visual acuity. The prevalence of presbyopia, its social and economic consequences and the prolongation of human life place the correction of presbyopia among the top challenges in modern ophthalmology. Despite the numerous methods currently available for correcting presbyopia, there is still no ideal technique that, by restoring the eye's age-related loss of physiological accommodation, would provide long-term effectiveness without adverse effects. This article offers an overview of the exist…
